Fine needle biopsies (FNB) of the thyroid were examined from 860 patients. In 703 cases follicular cells without atypia were found and in this group of patients the clinical diagnosis was nodular goitre. Operations were performed in 138 patients and in 97 cases the cytological finding could be correlated to the histopathological diagnosis. In 33 of these patients thyroid carcinoma was histologically verified. In 26 of the carcinoma cases cytologic examination showed grave atypia or changes indicating carcinoma. The cases in which the cytological diagnoses were falsely negative are discussed. Moderate cellular atypia occurred in one case with papillary carcinoma. In two cases the cytological examination gave a false positive diagnosis of cancer, both representing thyroiditis of the lymphoid type. The scintigrams in patients with thyroid carcinoma are also presented. Cold nodules were found in 10/19 patients and a hot nodule in .1 patient. In 3 patients the scintigrams were normal and in another 5 inconclusive. The results indicate that thyroid scintigrams can only be used as a supplement to the physical examination and a guidance for FNB. The contribution of FNB in the decision to operate is discussed and it is concluded that FNB is a valuable adjunct in preoperative diagnosis of thyroid lesions. The best diagnostic results are obtained when there is a close cooperation between clinician, radiologist, cytologist and pathologist.
The inter- and intraobserver reproducibilities of the histopathological systems of breast cancer classification suggested by the World Health Organisation (WHO), the Armed Forces Institute of Pathology (AFIP) and Ackerman have been analysed. The reproducibilities of the three classification systems were only "fair" to "moderate" and no correlation with the five-year recurrence rate was found. Our results indicate that these classification systems are without biological significance and are useless for prognosis in the individual patient. When the tumours were classified according to degree of differentiation (high, moderate, low) or graded according to WHO (which includes both differentiation and nuclear atypia), however, there was a significant correlation with the five-year recurrence rate. Yet even such "reduced" subdivisions are of no value in judging prognosis for the individual patient at the time of diagnosis; rather, they are useful only in the follow-up analysis of groups of patients.
An evaluation of cytometric features of 142 human breast cancers was performed. Twenty-two cytometric features describing density, structure and shape were correlated with mitotic frequency and labeling index of some of the tumors and with the observed recurrence rate of the tumors within 3 years after removal of the breast. A new cytometric measure was also created by combining the four features most strongly correlated with mitotic frequency of the tumors. It was found that the variance of the nuclear area was the feature that was most strongly correlated with mitotic frequency and recurrence rate. No new cytometric measure created by combining the four features most strongly correlated with mitotic frequency, correlated as well as variance of nuclear area even though the four features were optimized by multiple regression analysis for their correlation with mitotic frequency.
The present study comprises 173 breast cancers in women living in Sweden. Mastectomy was performed and the surgical specimens were thoroughly scrutinized histopathologically with special attention given to the mammary tissue outside the dominant mass. Fifty-two patients (30 percent) had multifocal growth in the same breast as the dominant breast cancer. The patients have been followed up for 3 years after operation and compared with age-matched controls. The multifocal growth was not correlated with age, size of the tumor or death of the patient within 3 years. A previous diagnosis of benign breast disease was significantly more common among the cancer patients than among the controls, although it was unrelated to multifocal growth. This study stresses the importance of considering the high incidence of multifocal growth of breast cancer when discussing treatment by operation less radical than mastectomy.

The inter- and intra-observer reproducibility of three grading systems for breast cancer has been analysed: those of the World Health Organisation, of Black, and of Hartveit. In addition, the components forming the basis of these grading systems were analysed separately with regard to reproducibility and interrelationship. In this analysis, degree of differentiation was also included as a parameter. The grading systems of WHO and of Black gave a stratification of the material into three categories of tumours, but that of Hartveit did not. All three systems had a low inter- and intra-observer reproducibility. Truncated component analysis indicated that the grading systems of WHO and of Black are closely related to each other and to the 'nuclear lobulation' component of Hartveit's system. The components 'pleomorphism', 'mitotic frequency', and 'hyperchromasia' of the WHO system were closely interrelated. Descriptors such as 'differentiation' and 'tubule formation' were interrelated but acted in a different direction from the others.
In an enzymatic study of livers from obese hyperglycemic mice (obob) and their lean litter mates, microdissected freeze-dried fine needle biopsies were used. Carbohydrate, fatty acid and ketone body metabolism was examined through assays of phosphofructokinase (PFK), 3-hydroxyacyl-CoA dehydrogenase (HOADH) and D-3-hydroxybutyrate dehydrogenase (HBDH). At 2 months of age the obob-mice showed a significant increase in PFK activity as compared to their lean litter mates. No such difference was found for the activity of HOADH and HBDH. An early increase in the initial glycolysis in the livers of obob-mice might thus be one of the factors responsible for the accelerated synthesis of lipids resulting in the fatty livers of the adult obob-mice. The present results corroborate previous studies indicating no major derangement in the fatty acid and ketone body metabolism in the livers of obob-mice. In a methodological evaluation of the technique for tissue sampling and preparation, freeze-dried fine-needle punctates showed significantly higher enzyme activities of PFK and HOADH than freeze-dried liver sections or crude homogenates. Freeze-drying of fine needle biopsies appears to be a technique with good preservation of the enzyme activity. This is of significance for future metabolic studies also in humans.
Fine needle aspiration biopsies were used for the diagnosis of orbital tumours. Examples of benign and malignant primary and metastatic orbital neoplasms, as well as inflammatory lesions diagnosed in this way are described. The method has proved of great value. This was specially true for processes located posteriorly in the orbit. The necessity for close cooperation between the ophthalmological and clinical cytological departments is emphasized.
A newly developed in vivo method, using the ob-ob strain of obese-hyperglycaemic mice with permanently very high serum insulin values, makes it possible to detect more prolonged serum insulin lowering properties than in normal animals. Two newly synthesized analogues of somatostatin, D-alanine-somatostatin and des-alanine-des glycine-des-amino-somatostatin produced a more prolonged and greater decrease in the serum insulin values of ob-ob mice than did somatostatin. Our new in vivo method makes it possible to investigate the duration of insulin suppression of new derivatives.
A new method for measuring differences in nuclear detail in chrome alum gallocyanin-stained nuclei of cells from human breast cancers was compared with conventional subjective grading and classification systems. The new method, termed computerized nuclear morphometry (CNM), gives a multivariate numerical score that correlates well with nuclear atypia and gives a higher reproducibility of classification than do subjective observations with conventional histological preparations. When 100 individual nuclei from each of 137 breast cancers were examined by CNM, there was a broad CNM score variation between patients but a good reproducibility for each tumor. When different parts of the same tumor were sampled, there was good reproducibility between samples, indicating that some breast cancers at least are "geometrically monoclonal." When these cancers were compared by the grading systems of WHO and Black, correlations of 0.43 and 0.48, respectively, were found. There was a poor correlation between CNM and classifications of tumor type, but in general there were high values for CNM in medullary tumors and low values in mucous tumors. Correlations between CNM and tumor progression and prognosis await future study of patients participating in the study.

A case of hepatoma in a 79-year old woman is reported. The patient had an enlarged liver and a pathological liver scintigram. Percutaneous liver biopsies were performed both with Menghini-and fine needles. The most prominent feature was the presence of hyaline cytoplasmic PAS-negative inclusions in liver parenchymal cells. There was no nuclear atypia. Electron microscopy disclosed two types of cytoplasmic changes. One consisted of a lamellar ultrastructure and was interpreted as a hyperplasia of smooth-surfaced endoplasmic reticulum. The other change consisted of smooth globular non-membrane limited regions containing amorphous or finely fibrillar material. This was interpreted as corresponding to the hyaline inclusions visible in the light microscope. The presence of PAS-negative hyaline cytoplasmic inclusions may thus be a sign of hepatoma. This may be of relevance for the diagnostic considerations of material obtained by fine needle biopsy.

Patients on digitalis medication at the time of diagnosis of breast cancer seem to develop tumors that have a lower growth potential than do patients not on such medication. These tumors are photometrically characterized by small nuclei with lower DNA-RNA content and less variation in morphometric characteristics.
