A dilutional immunoperoxidase study of carcinoembryonic antigen (CEA) reactivity of mild and severe epitheliosis as well as malignant ductal and lobular lesions of the breast was performed. Intraduct carcinoma with a cribriform and clinging patterns showed intracytoplasmic, glycocalyceal and intraluminal staining for CEA at higher dilutions of antiserum than cases with mild and severe epitheliosis. Also, many intraduct carcinomas associated with infiltration stain at a lower concentration of antiserum to CEA than pure intraduct lesions. Staining is seen at lowest concentrations in the infiltrating components. This suggests that as intraduct carcinoma becomes invasive, it loses some of its ability to store and possibly to synthesize CEA. The dilutional immunoperoxidase method could be applied routinely to cases of severe epitheliosis to differentiate them from intraduct carcinoma. However, relatively few cases were studied in this preliminary series and a further study is being carried out. The difficulties encountered using CEA as a tumour marker are outlined.

The chemotactic responsiveness of peripheral blood monocytes was measured in 194 individuals: 37 patients with breast cancer, 17 patients with a history of breast cancer but clinically free of disease after surgery, 42 patients with benign breast masses, and 98 normal controls. Monocyte chemotactic responsiveness (MCR) in vitro was not significantly different from normal [mean = 72.8 migrating monocytes/oil immersion field, +/- 9.3 (1 SD)] in 2 groups of patients: a) those with benign breast masses (mean = 72.6 +/- 15.1; P greater than 0.3) and b) those previously having breast cancer resected and remaining clinically free of disease (mean = 69.0 +/- 12.5; P greater than 0.4). However, MCR was significantly depressed in the group of patients with active breast cancer (mean = 57.2 +/- 20.7; P less than 0.0025). Resection of malignant breast masses resulted in a significant change in MCR (P less than 0.0025), whereas resection of benign lesions did not (P greater than 0.4). MCR was abnormal in all clinical stages of breast cancer, including breast cancer without evidence of metastasis to regional lymph nodes. These data supported the hypothesis that neoplasms adversely affect monocyte function and may thereby hinder immunologically mediated destruction of malignant cells.
Three examples of primary signet ring cell carcinoma of the breast are described. This form of breast carcinoma deserves recognition as an entity because its prognosis may differ from colloid carcinoma of the breast and because of potential difficulty in distinguishing it from metastatic carcinoma.
The ultrastructural characteristics of mucin production in mammary mucoid carcinoma, signet ring cell carcinoma (Harris, Wells & Vasudev 1978), papillary carcinoma and lobular carcinoma are compared. The mucin in lobular carcinoma is confined to intracytoplasmic lumina whereas it is present as membrane bound granules in the other three types, although intracytoplasmic lumina also occur in the latter. A possible origin of intracytoplasmic lumina from distended Golgi cisternae is proposed. Possibly, extracellular mucin acts as a mechanical barrier between tumour cells and lymphatics, thus explaining the good prognosis of typical mucoid carcinomas.
Amyloidosis, either primary or related to other pathologic states, may mimic a variety of disorders and produce a multitude of appearances demonstrable on radiographic examinations of the chest. All intrathoracic tissues and organs may be affected, but the major abnormalities most commonly involve the heart and lungs. The radiographic appearances in ten patients are described and correlated with the underlying gross and microscopic pathologic findings. No specific pattern of pulmonary involvement could be determined, aside from tracheobronchial amyloidosis. The final diagnosis is dependent upon the awareness of the clinical and radiologic spectrum and the final microscopic examination of involved tissue.
Hemoglobin Hope (beta(H14)136gly leads to asp), a mildly unstable variant, was found to have decreased oxygen affinity, a normal Bohr effect and diminished cooperativity. Decreased oxygen affinity of hemoglobin Hope may explain the previous failure to find an appropriate response to hemolysis in individuals studied who were heterozygous for both hemoglobin Hope and sickle hemoglobin. Salt bridge formation between NA1 valine and H14 aspartic acid may stabilize the beta Hope subunit in its deoxy form thus producing intrinsically low oxygen affinity and reduced cooperativity.
