The GRIT: a quantitative measure of ulnar impaction syndrome.

The gripping rotatory impaction test (GRIT) is performed with a standard grip dynamometer and provides a quantitative measure for identifying ulnar impaction. The purpose of this study was to determine whether patients with unilateral ulnar impaction syndrome (UIS) have a GRIT ratio on the involved side greater than 1.0 while the GRIT ratio on the uninvolved side is no different from 1.0. Twenty-four patients with unilateral UIS were tested with the GRIT on both the symptomatic, involved side and the asymptomatic, uninvolved side. The GRIT was performed with a standard grip dynamometer, and testing alternated between each patient's involved and uninvolved sides, with the wrist in the neutral, supinated, and pronated positions, in that order. The GRIT ratio on the involved side, at 1.37, was significantly greater than 1.0, while the GRIT ratio on the uninvolved side, at 1.03, was not significantly different from 1.0. The GRIT ratio is a quantitative measure that identifies UIS and can be used as an adjunct to imaging studies and qualitative clinical tests for UIS. The GRIT may also be helpful in determining which patients with UIS might benefit from surgical ulnar shortening.

Interaction between rod and cone signals in responses of lateral geniculate neurons in dichromatic marmosets (Callithrix jacchus).

Parvocellular (P-) and magnocellular (M-) cells in the marmoset LGN can receive prominent rod input up to relatively high illuminance levels (Kremers et al., 1997b). In the present paper, we quantify rod and cone input strengths under different retinal illuminance levels. The stimulus was based on the so-called "silent substitution" method. The activities of P- and M-cells of dichromatic animals were recorded extracellularly. We were able to adequately describe the response amplitudes and phases by a vector summation of rod and cone signals. At low retinal illuminance levels, the cells' responses were determined by rod and cone inputs. With increasing illuminances the strength of the cone input increased relative to the rod strength. But, we often found significant rod inputs up to illuminances equivalent to 700 td in the human eye or more. Rod input strength was more pronounced in cells with receptive fields at large retinal eccentricities. The phase differences between rod and cone inputs suggest that the rod signals lag about 45 ms behind the cone signals.

Glutamate stimulates inositol phosphate formation in striatal neurones.

The major excitatory amino acids, glutamate (Glu) and aspartate (Asp), are thought to act at three receptor subtypes in the mammalian central nervous system (CNS). These are termed quisqualate (QA), N-methyl-D-aspartate (NMDA) and kainate (KA) receptors according to the specific agonist properties of these compounds revealed by electrophysiological studies. Although Glu has been shown to stimulate cyclic GMP formation in brain slices, direct regulation of second messenger systems (cyclic AMP, Ca2+ or inositol phosphates) subsequent to activation of excitatory amino-acid receptors, has not been extensively studied. Here we demonstrate that in striatal neurones, excitatory amino acids, but not inhibitory or non-neuroactive amino acids, induce a three- to fourfold increase in inositol mono-, di- and triphosphate (IP, IP, IP) formation with the relative potency QA greater than Glu greater than NMDA, KA. The Glu-evoked formation of inositol phosphates appears to result principally from actions at QA as well as NMDA receptors on striatal neurones. Our results suggest that excitatory amino acids stimulate inositol phosphate formation directly, rather than indirectly by the evoked release and subsequent actions of adenosine or acetylcholine.

Transfer of specificity by murine alpha and beta T-cell receptor genes.

T-cell receptor alpha- and beta-chain genes were isolated from a class I major histocompatibility complex-restricted cytotoxic T-cell clone and transferred by protoplast fusion into another cytolytic T-cell clone of different specificity. Expression of the transfected alpha and beta genes endowed the recipient cell with the specificity of the donor cell.

Primary choriocarcinoma of the vulva.

Primary extrauterine choriocarcinoma is very rare, found mostly in the genital tract (tube, cervix, ovary, vagina). Other sites such as lungs, gastrointestinal tract, heart, brain and submentum also have been reported. A 31-year-old woman presented with abnormal uterine bleeding 6 weeks after her last menstrual cycle. Her HCG titer level was 900 mIu/ml. She underwent dilation and curettage (D & C). Pathology failed to reveal any chorionic villi. Further evaluation was done, including a second D & C, laparoscopy, exploratory laparotomy, CT, MRI and ultrasound which all failed to define a source for the elevated HCG titer which subsequently rose to 95,000 mIu/ml 130 days after her last menstrual cycle. At that time a vulvar mass was observed. Fine needle aspiration (FNA) was positive for choriocarcinoma. The patient was treated with one course of methotrexate 60 mg intramuscularly every other day for 5 days. Actinomycin-D 0.5 mg given intravenously daily for 5 days was added to courses 2-7. Radiotherapy (4000 rads) was applied concomitant with the chemotherapy. The remainder of the mass was subsequently excised. The patient has remained with no evidence of disease for 10 years. This is the first case report of primary vulvar choriocarcinoma.

[Induction of bone tissue on different matrices: an in vitro and a in vivo pilot study in the SCID mouse].

AIM: Three resorbable biomaterials were evaluated regarding proliferation and osteogenic differentiation of human bone marrow stromal cells (BMSC) in vitro. In a second step, the new biomaterial, calcium-deficient hydroxyapatite (CDHA), was tested in a pilot in vivo study by subcutaneous implantation in the severe combined immunodeficiency (SCID) mouse. METHODS: CDHA, beta-tricalcium phosphate (beta-TCP), and demineralized bone matrix (DBM) were seeded with human BMSC and cultured in osteogenic supplements for 3 weeks. In the pilot in vivo study, CDHA was seeded with BMSC and kept in osteogenic media for 2 weeks (group A) before subcutaneous implantation in 8 SCID mice for 3 and 8 weeks. In addition, CDHA seeded with BMSC without prior osteogenic induction (group B) and empty ceramics were implanted in each mouse. RESULTS: Total protein content and the values for specific alkaline phosphatase (ALP) increased significantly in vitro on all matrices, but no significant difference between the groups was noted. In the pilot in vivo study all ceramics were well penetrated by cells. After 8 weeks 2 of 4 samples in group B and 1 of 4 samples in group A revealed cells resembling hypertrophic chondrocytes. Specific ALP was higher in the group B (p = 0.012, Z = - 2.5) compared to empty ceramics. There were no significant differences between groups A and B. Differences between group A and the empty control did not become significant (p = 0.069, Z = - 1.8). CONCLUSION: All three matrices promoted BMSC proliferation and differentiation to osteogenic cells in vitro. Human BMSC on CDHA showed signs of osteogenic differentiation after subcutaneous implantation into SCID mice.

[Evaluation of force and mobility following the open Bankart operation for treatment of recurrent dislocation of the shoulder].

AIM: The purpose of this study was to evaluate the isokinetic muscle strength, range of motion and radiological signs of arthopathy of recurrent dislocation of the shoulder after an open Bankart procedure. METHODS: This retrospective study is based on the analysis of isokinetic muscle strength, range of motion, function and arthropathy of the shoulders of 81 patients at an average of 2.3 years (range 1-4 years) after a Bankart procedure. Isokinetic muscle strength testing was performed using the Cybex 6000 dynamometer. RESULTS: Clinical testing at follow-up revealed no significant loss of range of motion of the operated shoulder compared to the healthy contralateral shoulder. The deficit of muscle strength (average peak torque, total work, total power) of the involved side averaged 10 % of the not-involved side and was not statistically different. Follow-up radiographs demonstrated an increase of arthropathy in 35 % of the shoulders after the Bankart procedure. CONCLUSION: At an average follow-up of 2.3 years after an open Bankart repair for treatment of recurrent dislocation of the shoulder we found no significant difference between the operated shoulder and the healthy contralateral side concerning range of motion and isokinetic muscle strength. From that we conclude that the Bankart procedure offers an excellent clinical outcome combined with a low degree of morbidity.

[Presentation of a treatment concept of Duchenne muscular dystrophy with the aim at improvement and restoration of walking and standing ability].

A concept for treating progressive muscular dystrophy type Duchenne is presented. It aims at the improvement of quality of life by surgery of the lower extremities. 60 Duchenne patients were treated by a standard operating regime. The different stages of the disease required different postoperative rehabilitation measures.

The out-of-hospital use of a domestic violence screen for assessing patient risk.

OBJECTIVES: To determine the following: 1) whether a Domestic Violence Scene Assessment Screen (DVSAS) is accurate at predicting domestic violence (DV) when compared with results on the validated Abuse Assessment Screen (AAS), and 2) whether EMTs can perform accurately on a DVSAS after they have finished a transport so as not to interfere with routine care. METHODS: All patients transported by ambulance from domestic environments (i.e., home) by an urban EMS system were included in the study. A ten-question screen was developed to assess whether a domestic environment had a high risk of having DV (DVSAS). A positive answer to any of the questions was considered to be a positive result on the screen. A trained observer with no clinical duties rode on the ambulance for randomized shifts during a two-month period. The observer completed the DVSAS while at the scene, then the patient, if able, completed the AAS. After finishing the transport, the EMT completed the DVSAS based on his or her memory of the scene. Results of the observer DVSAS were compared with the results of the EMT DVSAS and with the AAS. RESULTS: A total of 43 transports from domestic scenes were included in the study. The observer DVSAS alone was positive in five cases (12%), the EMT DVSAS alone was positive in five cases (12%), and both were positive in seven cases (17%). Agreement between the EMT and the observer yielded a kappa of 0.56 adjusted for chance. Of 15 (42%) patients able to complete the AAS, one (7%) was positive on the AAS alone, four (27%) were positive on the observer DVSAS alone, and three (29%) were positive on both. The observer DVSAS agreed with the AAS results in ten of 15 (66%) of cases. When compared with the AAS, the observer DVSAS had a sensitivity of 75%, specificity of 55%, positive predictive value of 38%, and negative predictive value of 86%. CONCLUSION: Emergency medical technicians can complete the DVSAS at the end of a transport with good agreement with results obtained by an independent observer at the scene. The DVSAS is able to reflect the results of the AAS with moderate to good agreement.

Israeli Arab and Jewish youth knowledge and opinion about alcohol warning labels: pre-intervention data.

This article presents baseline data on the opinion toward alcohol beverage warning labels and on levels of knowledge of the risks discussed in the contents of the labels prior to the labels' introduction, and on levels of knowledge of additional alcohol-related hazards not included in the proposed warning labels, among a sample of 3065 adolescents of four religions living in the northern region of Israel. About 2220 Arab participants (Moslems, Christians and Druze) and 845 Jewish respondents answered in the winter of 1996 a Hebrew version of an American questionnaire, which had been used to measure levels of knowledge of the label in the United States. More respondents were in favour of warning labels on alcohol containers than on advertisements. Arabs as a group were more in favour of warning labels on alcohol containers than Jews. The initial knowledge levels among the participants were not very high, especially concerning the item 'Drinking impairs the ability to operate machinery' (74.4%) which is included on the proposed warning label, and concerning two hazards which are not included: 'Drinking increases risk of cancer' (54.6%) and 'Drinking increases risk of high blood pressure' (60.4%). Abstainers knew more than drinkers that 'Pregnant women should not drink', 'Drinking increases risk of cancer' and 'Alcohol in combination with other drugs is hazardous'. Implications for public health are discussed and alternative warning messages that might be used to inform the Israeli public of several less well-known hazards are suggested.

The presence of hepatic intramitochondrial crystalline inclusions in routine liver biopsies.

Electron microscope examination of the liver biopsies of 33 patients revealed the presence of intramitochondrial crystalline inclusions (IMCI) in ten of them. Three patients suffered from iron deficiency anemia, porphyria, and malignant melanoma, conditions not reported to be associated with IMCI. Comparison of the clinical and laboratory findings of patients with and without IMCI did not show significant difference, or a clue for clarifying the nature of this phenomenon. Although these inclusions were reported in normal individuals, the possibility that they are the result of liver damage is not excluded.

Clinical impact and prognostic value of metaiodobenzylguanidine imaging in children with metastatic neuroblastoma.

PURPOSE: The clinical value of metaiodobenzylguanidine (mIBG) scintigraphy in patients with disseminated neuroblastoma (NB) at the time of diagnosis and after induction chemotherapy was evaluated. PATIENTS AND METHODS: The medical records and imaging studies of 30 patients with stage 4 NB who underwent mIBG scintigraphy and 99mTc hydroxy methylene diphosphonate bone scintigraphy at the time of diagnosis were reviewed. Scores were calculated for the mIBG and bone scintigrams, and outcome according to the initial and follow-up imaging studies was determined. RESULTS: Discrepancies between bone scintigraphy and mIBG osteomedullary localization were seen in six patients. For the entire cohort, 2-year event-free survival did not significantly differ for the group of patients with initial mIBG or bone scintigraphy scores > or = 10 compared to those with scores < 10 (P = 0.23 and 0.61, respectively). However, for patients older than 1 year, a trend associating worse outcome with mIBG scores > or = 10 at diagnosis was seen (P = 0.08). A trend correlating abnormal mIBG scintigraphy after induction therapy and poor outcome was also observed (P = 0.09). Outcome did not correlate with the results of the bone scintigram studies performed after induction chemotherapy (P = 0.68). CONCLUSION: Because a discordance between mIBG and bone scintigraphy results were seen in a subset of stage 4 NB patients, both imaging studies should be performed at the time of diagnosis. mIBG imaging studies performed at the time of diagnosis and after induction chemotherapy may be of prognostic value, particularly in stage 4 patients older than 1 year.

Soluble CD44 variants in the serum of patients with urological malignancies.

In patients with malignant diseases, characteristic alterations in the expression of CD44 protein and their variants were found. For the present study, the serum concentrations of the standard isoform CD44 std and the two variant isoforms CD44 v5 amd CD44 v6 were measured by ELISA in patients with prostate cancer (n = 49), benign prostatic hyperplasia (n = 30), renal cell carcinoma (n = 31) and bladder cancer (n = 29). The data were compared with the results of 30 healthy men and 30 healthy women. The sCD44 v5 concentrations in patients with prostate cancer (p < 0.01), benign prostatic hyperplasia (p < 0.01) and men with renal cell cancer (p < 0.01) were significantly lower than those measured in the male control group. The sCD44 v5 concentrations observed in male patients with bladder cancer were lower than in the male control group (p < 0.05). Only in one group the concentration of sCD44 std differed significantly from the others. The sCD44 std concentration in male patients with renal cell cancer was significantly lower than in the male control group (p < 0.01). Other significant differences were not found. In contrast to results observed in other carcinomas, the determination of soluble CD44 proteins in serum cannot be recommended as a marker for urological malignancies.

Histoplasmin reaction. Comparison of a polysaccharide antigen to the filtrate antigen.

This work was planned by taking into account all the knowledge accumulated from the immunological study of paracoccidioidomycosis. It aimed at comparing a polysaccharide antigen from Histoplasma capsulatum to a classic histoplasmin with the help of intradermal tests of delayed type of hypersensitivity. Tests were applied to 115 individuals in Santo Amaro, a town in the State of São Paulo. Positive results using classic histoplasmin were obtained in 46.0% cases whereas positive results using the polysaccharide antigen at its highest concentration were obtained in 51.30% cases. The major conclusion in this investigation is that it is possible to use the polysaccharide antigen as histoplasmin instead of the filtrate antigen.
