Cleidocranial dysplasia. A family study.

The incidental observation of Wormian bones in the skull and defective ossification of the public symphysis in a mildly scoliotic 12-year-old girl prompted a clinical and radiological study of members of her family. The diagnosis of cleidocranial dysplasia (CCD) was established by observing similar skeletal abnormalities in eight out of ten of them but the classical clavicular defect in only one, the last to be examined. Abnormal modelling of the medial end of both clavicles was observed in all those affected. A modelling defect of the public bones is suggested as an additional, hitherto undescribed, radiological sign. The progression with age of certain radiological features of CCD is discussed. It is suggested that this family represents an atypical variant of CCD. Such variance may be more prevalent in patients with mild scoliosis than presently suspected. A complete skeletal survey with espeical attention to the skull, pelvis and medial aspect of the clavicle and family studies may be essential for the definitive diagnosis of CCD in certain patients.

The cystometric nuclear cystogram.

Vesicoureteral reflux is a common clinical problem that we monitor by interval nuclear cystography. Of the children having nuclear cystograms 40% also have voiding abnormalities, including incontinence (damp pants), urgency and infrequent micturition. We have evaluated these symptoms by recording the intravesical pressure during the nuclear cystogram. This combined examination, the cystometric nuclear cystogram, has been done on 46 children. An abnormal cystometrogram was found in 61% of the children with a voiding abnormality and helped to establish a basis for successful therapy. The cystometric nuclear cystogram aids in the diagnosis and rational therapy of childhood voiding abnormalities. It is a practical method to obtain a cystometrogram in children with voiding abnormalities who are being evaluated for ureteral reflux and it has facilitated the management of childhood ureteral reflux.

Light chain heterogeneity of type lambda anti-streptococcal group A-variant polysaccharide antibodies in rabbits.

BASILEA rabbits lack the expression of chi-polypeptide chains and compensate for this lack with expression of lambda polypeptide light chains. These rabbits were immunized with streptococcal group A-variant vaccines. The heterogeneity of the lambda polypeptide chains of specific antibodies was analyzed and compared with that of chi light chains. No significant difference was found by SDS-PAGE and IEF in the number of light chain bands of high affinity antibody expressed. This suggests that in the rabbit the size of the variable region repertoire is similar for lambda and chi light chains.

A new cytogenetic aspect of polycythemia vera.

The cytogenetic findings in a group of 15 polycythemic patients are reported. G and C banding techniques were used on bone marrow and peripheral blood preparations. Major chromosomal aberrations were found in four out of the 15 patients, an incidence similar to that found in other studies on polymorphism. The most interesting finding concerned the chromosomal polymorphism of the pair 19. A possible relation to the etiology of the disease is discussed.

Ultrastructural features of the plasma cells in "non-secretory" myeloma.

A case of "non-secretory" multiple myeloma is described. The diagnosis was based on the clinical picture, typical radiological findings, and infiltration of the bone marrow by myeloma cells which showed specific immunofluorescence staining mainly with antisera for IgM and kappa light chains. An attempt is made to explain the absence of pathological proteins in the serum, based on the ultrastructural findings of the myeloma cells, which showed "buddings" of the cell membranes containing endoplasmic reticulum and cytoplasmic material. It is suggested that the cells of the "non-secretory" type of multiple myeloma possess a normal excretory mechanism, but the pathological proteins are prevented to be secreted in the serum being surrounded by portions of the cell membrane.

Quantitative nuclear cystogram. Aid in determining spontaneous resolution of vesicoureteral reflux.

The quantatitative data derived from serial nuclear cystograms over a five-year period in 37 children were compared with the clinical course. A direct correlation (92 per cent) between an increasing bladder volume at which reflux occurs was observed in the clinically stable group. In the clinically unstable group, there was a 75-per cent correlation of an unchanged or decreasing bladder volume at which reflux occurs. The quantitative nuclear cystogram provides an objective as opposed to subjective data base for management of patients with vesicoureteral reflux.

Antibody response to the streptococcal group A-variant polysaccharide in BASILEA rabbits lacking kappa-polypeptide chains.

Rabbits from a variant strain called BASILEA, in which homozygotes express only lambda-type chains and heterozygotes have normal kappa/lambda ratios, were hyperimmunized with a streptococcal group A variant vaccine. Homozygotes (bas/bas) produced antibodies with lambda-chains, heterozygotes, however, produced predominantly antibodies with kappa-chains. The incidence of restricted high responders in the BASILEA strain was high; it was probably introduced by the original mutant rather than by the loss of kappa-chains (founder effect). The degree of heterogeneity of homozygotes is similar to the heterogeneity of heterozygotes, and to that of rabbits expressing kappa-chains. This suggests that in the rabbit, the repertoire of lambda-chain genes is of similar size to that of kappa-chain genes.
