Facial nerve function following temporomandibular joint surgery using the preauricular approach.

Sixty-eight patients (83 temporomandibular joints) consecutively operated on who had a variety of temporomandibular joint operations using a preauricular approach were assessed for facial nerve function following surgery. Nine patients (10.84%) showed signs of facial nerve injury in which the temporal and zygomatic branches were involved. The incidence of facial nerve injury was greater in patients who had undergone previous temporomandibular joint surgery (17.64%) than in patients with previously unoperated joints (9%). Normal facial nerve function returned in 9 to 14 weeks except in one patient who showed a mild deficit of the zygomatic branch at 20 weeks. The nature and duration of the surgical procedure did not correlate with facial nerve injury. Scarring of tissues as a result of previous temporomandibular joint surgery may significantly increase the risk of facial nerve injury during subsequent temporomandibular joint surgery.

Rembrandt toothpaste stain prevention with and without the use of Peridex.

Sixty-five subjects were assigned to use Crest or Rembrandt dentifrice with either a placebo or Peridex rinse for eight weeks. One group, using Peridex, started with Crest and switched to Rembrandt at week four of the study. There were no significant differences among the groups on gingivitis, plaque, or calculus by the conclusion of the study. On the stain intensity index there was a significant statistical difference on buccal surfaces between Crest and Rembrandt with placebo compared to Crest with Peridex. On lingual surfaces, Rembrandt with placebo rinse was significantly lower than all Peridex rinse groups by the end of the study. From baseline to the conclusion of the study, Crest with placebo had no change in stain intensity, while Rembrandt with placebo had a significant drop on both buccal and lingual surfaces. The Peridex groups had increases on buccal stain intensity. On stain area scores, by the conclusion of the study both Crest and Rembrandt with placebo were significantly different from Crest with Peridex on buccal and lingual surfaces. Over time there was no significant change in stain area for the Crest with placebo groups. Rembrandt and placebo was significantly lower on stain area from baseline to week eight on buccal surfaces. The Peridex groups had increases in stain area over the study period.

Disseminated histoplasmosis with embolic endovascular complications: a case report.

A 57-year-old man had subacute embolic ischemia of his right foot and subsequent acute embolic ischemia of his left foot after angiography. Thrombus removed at the time of the left femoral thromboembolectomy grew Histoplasma capsulatum confirming the diagnosis of disseminated histoplasmosis. Surgical revascularization of the right leg and parenteral amphotericin B was followed by chronic ketoconazole therapy for 16 months. The patient has remained asymptomatic at 30 months after operation. Effective treatment of endovascular infection with ischemic complications of Histoplasmosis requires surgical revascularization and intensive chemotherapeutic intervention. Histoplasmosis is a ubiquitous infection in endemic areas that often has an asymptomatic subclinical course. Involvement of the cardiovascular system is rarely reported. Previous case reports have described infected cardiac valves and aortic aneurysms. This report describes the uncommon presentation of disseminated Histoplasma capsulatum infection as a peripheral embolic event and the successful management with revascularization combined with systemic amphotericin B followed by ketoconazole therapy.

Histoplasmosis

An assessment for auriculotemporal syndrome following temporomandibular joint surgery through the preauricular approach.

Twenty temporomandibular joints (TMJs) were operated on for the correction of arthrographically confirmed internal derangements using a modified preauricular approach. Postoperatively, three of the 20 patients tested exhibited auriculotemporal syndrome as deduced from a starch-iodine test. The manifestation was subclinical and consisted of sweating only over the distribution of the auriculotemporal and great auricular nerves in response to a gustatory stimulus. The response was variable from patient to patient.

Effects of microamperage, medium, and bacterial concentration on iontophoretic killing of bacteria in fluid.

Prevention of nosocomial urinary tract infections by iontophoresis is addressed. An iontophoretic generator was used to provide microamperage (10 to 400 microA) to vials containing either synthetic urine or supplemented synthetic urine. Bacteria were added to vials, and parameters of growth, bacterial killing, and multiple electrode materials were examined. Escherichia coli and Proteus species were both inhibited and killed at various microamperages and with several electrode types, the most efficient being gold-gold as the anode-cathode combination. Klebsiella pneumoniae in supplemented synthetic urine was least inhibited in growth, and higher microamperage (200 to 400 microA) was most effective in killing the bacteria. Bacterial growth reduction and killing were directly related to increasing microamperage and were inversely related to bacterial concentration.

Bilateral mandibular hamartomas and familial natal teeth.

The case of a newborn girl with two mandibular incisor natal teeth and bilateral mandibular odontogenic harmartomas is presented. The hamartomas were evident as pedunculated masses on the posterior mandibular gum pads. The natal teeth were extracted at birth, and the hamartomas were excised at 5 months of age. Microscopic investigation of the hamartoma demonstrated the presence of all odontogenic tissues with the exception of an enamel organ. In addition, there was a strong family history of natal teeth, which may suggest a hereditary basis for the development of the odontogenic hamartoma.

Arthrogryposis multiplex congenita of the temporomandibular joint.

A case of arthrogryposis multiplex congenita involving the temporomandibular joint is reported. This is only the second reported case in which surgery was used in an attempt to alleviate the limited mandibular opening that may be associated with this uncommon condition. The clinical findings at the time of surgery differed significantly from those classically associated with other joints afflicted with this condition, suggesting that a different pathophysiologic mechanism may be involved.

Oral submucous fibrosis.

2 cases of oral submucous fibrosis have been reported. In each case, the diagnosis was confirmed histologically, and the treatment instituted was identical. Excision of buccal fibrotic bands was carried out, with subsequent placement of skin grafts. The patients were followed postoperatively to monitor results and particularly, to screen for malignant changes. The literature concerning oral submucous fibrosis is reviewed.

Hemodynamic effects of metocurine during isoflurane anesthesia.

Anesthesia was induced in 42 adults with thiopentone 3-7 mg/kg i.v. and maintained with isoflurane at a constant inspired concentration of 1-2%. After 30 min of hemodynamic stabilization with continuous muscle relaxation and an absence of surgical stimulation, each patient was randomly assigned to one of four metocurine dosage groups: I - control (n = 11); II - 0.2 mg/kg (n = 10); III - 0.3 mg/kg (n = 10); and IV - 0.4 mg/kg (n = 11). There were no significant hemodynamic changes in Groups I or II. In Groups III and IV mean arterial pressure (MAP) decreased 32% and 26% respectively, and systemic vascular resistance (SVR) decreased 42% and 36%, respectively (P less than 0.01). In Group IV, an increase of 24% in cardiac output was also significant (P less than 0.05). These results, especially in Group IV patients, stand in marked contrast to the lack of hemodynamic effects produced by metocurine during balanced anesthesia.
