External valvuloplasty technique in deep venous valve insufficiency of the lower limbs.

OBJECTIVE: To evaluate the therapeutic effects of external valvuloplasty technique in deep venous valve insufficiency of the lower limbs. METHODS: External valvuloplasty of the femoral vein valve was performed in 30 patients(41 limbs) with deep venous valve insufficiency of the lower limbs. The patients ranged in age from 16 to 69 years (mean +/- s, 50.8 +/- 9.2). External valvuloplasty of the first pair of superficial femoral vein valve was carried out in all limbs. External valvuloplasty of the second pair of superficial femoral vein valve was added in 7 limbs and valvuloplasty of the common femoral vein valve in 6 limbs. RESULTS: Venous claudication, swelling, aching disappeared in 78.0% limbs (32), obvious improvement was seen in 22.0% (9), and varicosis disappeared in all limbs. Ulcer healing was observed in 80% (8/10) limbs. Deep venous valve sufficiency was shown in 90.2% limbs, and reflux I degree-II degree in 4 by color Doppler. CONCLUSION: External valvuloplasty is an effective method to treat deep venous valve insufficiency of the lower limbs.

Isolation and identification of Mycoplasma genitalium from high risk populations of sexually transmitted diseases in China.

OBJECTIVE: To investigate Mycoplasma genitalium (Mg) infection in high risk populations of sexually transmitted diseases (STDs) in China by means of culture, Mg-DNA detection, observation under an electron microscope (EM) and an atomic force microscope (AFM). METHODS: 227 urogenital specimens taken from both patients in clinics and high risk populations of STDs in Beijing, Nanjing and Shanghai areas were cultured in SP-4 medium for isolation of Mg. Suspicious positive strains were identified first by PCR technique which could amplify the special DNA fragments of Mg adhesion protein (Mg-Pa) and 16S-rRNA sequences. The morphologic features of Mg were observed under EM and AFM. RESULTS: Eight strains were isolated from the 227 specimens, the total positive rate was 3.52%. The isolated strains met the criteria of growth and metabolic conditions required by Mg. Specific fragments of Mg-DNA (374 bp based on Mg-Pa and 402 bp on 16S-rRNA primers, respectively) could be amplified from both the isolates and the genital secretions. Under EM and AFM, Mg showed different shapes, including flask, pear, spindle and globe shape. The typical shape of Mg was flask or pear shape with projecting neck portion and broadened terminal structure under EM and with constrictive ring between the neck and body under AFM. CONCLUSION: Mg infection exists in high risk populations of STDs in China. We highly suggest the use of AFM in dermatological study.

[The role of mitogen-activated protein kinases family in ischemic preconditioning of rat heart].

OBJECTIVE: To investigate the cardioprotective effect of mitogen-activated protein kinases (MAPKs) of ischemic preconditioning (IPC) in rat heart. METHODS: In the model of ischemia/reperfusion (I/R) of isolated perfused SD rat heart, the effect of IPC on I/R injury was observed. The activities of MAPK family members, ERKs, p38 and SAPK, and their relationship with IPC phenomenon were also observed. RESULTS: IPC significantly improved myocardial functions in heart subjected to I/R injury, and attenuated leakage of myocardial creatine kinase and depletion of ATP. IPC activated ERK and p38 kinase. Their activities were increaed by 73% (P < 0.01) and 69% (P < 0.01) respectively. However, IPC showed no effect on myocardial SAPK activity. Preparation with genistein (an inhibitor of tyrosine kinase) and PD098059 (an inhibitor of ERK) abolished IPC cardioprotection, while p38 kinase inhibitor SB202190 had no effect on IPC. CONCLUSIONS: ERKs, but not p38 and SAPK of MAPK, mediates the IPC cardioprotective effect.

[In situ localization of mRNA gelatinases in cerebral aneurysms].

OBJECTIVE: To explore the role of gelatinases A and B and their tissue inhibitors, TIMP-1 and TIMP-2, in the formation of cerebral aneurysm. METHODS: In situ hybridization with digoxigenin RNA probes localized cells responsible for gelatinases A and B, TIMP-1 and TIMP-2 mRNA production in histologic sections of aneurysmal (n = 12), and normal (n = 6) cerebral arteries. RESULTS: The levels of gelatinase B mRNA were found very high in cerebral aneurysms, especially in the site of internal elastic lamella. Gelatinase B mRNA was not found in normal cerebral arteries. Gelatinase A TIMP-1 and TIMP-2 mRNA were not found in both cerebral aneurysms and normal cerebral arteries. CONCLUSIONS: Differential patterns of expression can be seen in situ, and tissue gelatinase B mRNA levels are elevated in aneurysms, suggesting that gelatinase B maybe responsible for the destruction of internal elastic lamina or even all laminae of cerebral arteries, leading to cerebral aneurysms.

[ABO-incompatible allogeneic bone marrow transplantation].

OBJECTIVE: To explore the immunohematological problems in ABO-incompatible allogeneic bone marrow transplantation (allo-BMT). METHODS: Among 250 HLA-matched allo-BMTs, 61 were performed between ABO incompatible siblings. Out of these 61 BMTs, 38 were major ABO-incompatible and 23, minor. Control group included 61 HLA-matched Allo-BMTs with ABO compatible grafts. The major ABO incompatible patients received marrow grafts depleted of erythrocytes by hydroxyethyl starch (HES) sedimentation. The minor or bidirectional received marrow grafts depleted of plasma to reduce anti-A or anti-B agglutinins. ABO antigen and antibody in the recipients sera were determined once a week. RESULTS: ABO-incompatible grafts for BMT had no adverse effect on engraftment, recovery of platelets, incidence of GVHD or survival. No recipient developed clinically hemolysis during or after marrow infusion. In ABO major compatible group, the onset of erythropoiesis after BMT was delayed and more RBC transfusions were required. Five of 10 recipients with blood group "O" in this group developed pure red cell aplasia (PRCA), lasting 7-24 months. Two of them recovered without therapeutic intervention. In the PRCA patients, the RBC antibody titers remained high for a longer time after BMT than in those whose erythropoiesis reconstituted timely. Serious morbidity related to ABO incompatibility did not occur. CONCLUSION: ABO-incompatible allo-BMT is fairly safe if there is indication, however, PRCA may develop in isolated cases.

Morphometric and histological study of coronary plaques in stable angina and acute myocardial infarctions.

OBJECTIVE: To compare the morphological difference of coronary plaques in acute myocardial infarction and stable angina to clarify the occurrence basis of these two different myocardial events. METHODS: Sixty-eight acute myocardial infarction cases and 34 stable angina postmortem cases were studied. The coronary arteries were dissected en block, fixed, decalcified, sectioned at a 3 mm interval and processed routinely for histologic examination. A computor-aided planimeter was used to measure plaque and lipid core area. RESULTS: Among the total 4089 coronary blocks in acute myocardial infarction, 2087 (51.0%) had a lipid core over 40% of the plaque area; while among the total 2030 blocks in stable angina, only 209 (10.3%) had a lipid core over 40% of the plaque area (P < 0.05). Sixty-one of the 68 (89.7%) acute myocardial infarction cases had thrombi among which 56 occurred on ruptured plaques; while no thrombus was found in stable angina. In addition, atheroemboli were found in most of the thrombus-negative acute myocardial infarction cases (6/7) but none in SA cases. CONCLUSIONS: Acute myocardial infarction and stable angina had different morphologic basis of the coronary plaques. The former had more plaques with a large lipid core than the latter. Acute myocardial infarction was caused mainly by plaque rupture and associated thrombus. But atheroembolus could be another very important cause of acute myocardial infarction.

[Relationship between extent of tumor resection and prognosis: an evaluation in 533 cases of gastric cancer].

OBJECTIVE: To evaluate prognosis of patients with stomach cancer based on the extent of tumor resection. METHODS: Five hundred and thirty-three cases of gastric cancer resected from 1980 through 1990 were divided into 3 categories A, B, and C, in the order of decreasing completeness of tumor resection as set forth in the 12th edition of "Rules for Gastric Cancer in Japan". RESULTS: There were 157 cases in category A, 209 cases in category B and 167 cases in category C. Their survival rate was 80.9%, 34.9%, and 9%, respectively. In category A and B, the depth of tumor invasion and lymph node metastasis were the major factors affecting survival rate. In the presence of lymph node metastasis, prognosis was dependent on which station (D1-D3) of lymph nodes dissected. In category B, if dissection covered D3 but metastasis was limited to I or II, the 5-year survival rate was 48%; if lymph node dissection performed was limited to the same station of lymph nodes where metastasis occurred, the 5-year survival rate dropped to 22.4% (P < 0.05). In case cancer left over at the cutting edge, the completeness of resection might drop from category A or B to C, and the 5-year survival rate correspondingly decreased to 6% and 12.2%, respectively. CONCLUSION: In order to ensure completeness of tumor resection, lymph node station dissected should be farther than that with metastasis, and the distance between the tumor and the cutting edge should be long enough to prevent residual tumor left behind.

[Is the asthma caused by self-diagnosed "common cold" reliable].

OBJECTIVE: To investigate the incidence of allergic rhinitis (AR) in the patients with asthma induced by the common cold, diagnosed by the patients themselves, and the relationship between AR and asthma. METHODS: All the patients were inquired and examined in details, including counting the eosiphils in the smear of the nasal secretion and allergen skin test. RESULTS: 49(47.6%) out of 103 asthmatics with so-called common cold were diagnosed as AR. The average of the initial asthmatic episode in asthmatics with AR were significantly younger than that in patients without AR, (28 +/- 13) and (36 +/- 16) years, respectively (P < 0.01). There was concordance between asthma and AR on seasonal regularity of episode. CONCLUSIONS: (1) In large portion of asthma, common cold diagnosed by patients themselves actually is AR; (2) There is a close relationship between AR and asthma; (3) It is important for physicians to distinguish AR from common cold in asthmatics.

[Study on epidemic of drug-resistant tuberculosis in Shandong province, China].

OBJECTIVE: To establish the surveillance system for drug resistance of tuberculosis according to WHO and IUATLD guidelines, and to evaluate the initial and acquired drug resistance in Shandong province. METHODS: 30 of 141 counties/cities were randomly selected as surveillance points by cluster sampling from April to October 1997, in each of which 36 cases with smear positive pulmonary tuberculosis were continuously observed. Identification and susceptibility tests were done in all culture positive samples. RESULTS: The total frequency of drug resistance was 23.4%, initial drug resistance 17.6%, initial multi-drug resistance 2.9%, acquired drug resistance 50.0%, acquired multi-drug resistance 19.6%. CONCLUSIONS: The results suggest that the initial drug resistance still ranks high in Shandong province, National TB Control Program should be further strengthened.

[Relevance of Fc gamma R polymorphism to the susceptibility of early-onset periodontitis].

OBJECTIVE: To investigate the polymorphism of the Fc gamma R genotype and its association with the susceptibility of early-onset periodontitis (EOP). METHODS: DNA of the white blood cells from 33 subjects with EOP and 27 healthy controls was extracted. Genotypes of the Fc gamma RIIA and Fc gamma RIIIB were determined by PCR followed by BstUI restriction endonuclease digestion and DNA sequencing, respectively. Then compared the differences in distribution of each genotype. RESULTS: No statistical difference in distribution of Fc gamma RIIA was observed between patients and controls. However, a significant over-representation of the Fc gamma RIIIB NA1/NA1 was found in EOP. CONCLUSION: These results suggest that the Fc gamma RIIIB NA1/NA1 may be a risk indicator for the susceptibility of the EOP.

[A study of clinicopathology and lectin immunohistochemistry in unicystic ameloblastoma].

OBJECTIVE: To investigate clinicopathological characteristics of unicystic ameloblastoma and histological markers for differential diagnosis with odontogenic cyst. METHODS: HE staining and histopathological observation, and lectin UEA-1, BSA-1 immunohistochemistry staining. RESULTS: 40 cases of unicystic ameloblastoma were divided histologically into 3 groups. In odontogenic cysts, most of epithelial lining showed positive binding with UEA-1 and BSA-1, whereas negative reactions were obtained for these two lectins in the epithelial components of unicystic ameloblastoma and solid or multicystic ameloblastoma, except for limited UEA-1 or BSA-1 binding to markedly keratinized tumor cells in several cases of ameloblastomas. CONCLUSION: The unicystic ameloblastoma is a distinct entity which has characteristic clinicopathology. The lectin staining for UEA-1 and BSA-1 is a useful histological aid to differential diagnosis between unicystic ameloblastoma and odontogenic cyst.

[Influence of pre-operational medicated dachengqi granule on inflammatory mediator in tumor patients].

OBJECTIVE: To find a simple, effective, with few side effect method of pre-cleaning intestine for surgical operation of abdominal malignancy. METHODS: Thirty-five patients of abdominal malignant tumor were divided into two groups randomly and treated with Dachengqi Granule (DCQG) and routine western management (including oral taking antibiotics and enema with soap solution) respectively. Serum level of nitric oxide (NO), tumor necrosis factor (TNF), intercellular adhesive molecule-1 (ICAM-1), and enteric function recovering of patients were observed before operation, and 1, 3 and 7 days after operation. RESULTS: Time of borborygmus recovering and gas discharging after operation in the DCQG group was earlier than that of the control group, P < 0.05. Serum NO level was raised in both groups after operation, and reaching peak level at 3rd day post-operationally, as compared with pre-operation, P < 0.05, the difference between the two groups was insignificant. Serum TNF level lowered in both groups, but more apparent in the DCQG group, the inter-group difference was significant at the first day of operation, P < 0.05. Serum ICAM-1 level in the DCQG group decreased at the day 3 after operation with significant difference in comparison with pre-operational level, P < 0.05, but in the control group, change of ICAM-1 before and after operation was insignificant. The mean ICAM-1 level of the DCQG group at the day 3 post-operationally was significantly lower than that of the control group, P < 0.05. CONCLUSIONS: Compared with the routine management of western medicine, using DCQG for intestine pre-cleaning before operation could obviously lower the operation induced inflammatory reaction of tumor patients, reduce the secretion of inflammatory mediator, improve the recovery of enteric function and reduce the post-operational complication. Therefore, applying DCQG for pre-operational enteric preparing could provide a satisfactory effect in surgical operation of abdominal malignancy.

[Effect of integrated Traditional Chinese and Western medicine on immune function in patient of acute cerebral infarction].

OBJECTIVE: To investigate the change of serum soluble vascular cell adhesion molecule-1 (sVCAM-1) in acute cerebral infarction patients treated with integrated traditional Chinese and western medicine (TCM-WM). METHODS: Serum sVCAM-1 level of patients treated with TCM-WM or western medicine alone was measured by ELISA, and compared with that of healthy person. RESULTS: TCM-WM could reduce the sVCAM-1 level in patients with cerebral infarction (P < 0.05). The therapeutic effect of TCM-WM was better than that of western treatment alone, P < 0.05. CONCLUSION: The therapeutic mechanism of TCM-WM may be related with the modulation on immune status of patients.

[A study on the pathogenicity of acute hepatitis G infection].

OBJECTIVE: To study the infection due to hepatitis G virus (HGV) in liver tissues of patients with acute hepatitis and to investigate its pathogenicity. METHODS: HGV NS5 antigen was detected by using immunohistochemical method in the liver biopsy tissues of patients with acute hepatitis but serologically negative to hepatitis virus antigens from A to E. The clinical and pathological data were also analysed. RESULTS: 37 samples were tested, HGV NS5 antigen was detected in 14 (37.8%). 4 of the 14 were positive for HGV NS5 antigen alone (acute hepatitis G group) and 10 other cases were also positive for HBsAg, hepatitis C virus (HCV) NS3 antigen or EB virus antigen (superinfection group). 7 cases were HBsAg positive alone (acute hepatitis B group) and the remaining 16 cases were negative for all the antigens mentioned above (non-A - G hepatitis group). There was no difference among the 4 groups in the detecting rate of Fas antigen, histological activity index (HAI), serum alanine transferase (ALT) and total bilirubin (TBil). CONCLUSION: It is suggested that HGV appears to play some role in the pathogenesis of acute viral hepatitis and the degree of liver injury in acute hepatitis G may be as severe as that of hepatitis caused by other types of viruses.

[Dynamic changes of neurotrophic factors in different cerebral areas after full cerebral ischemia and reperfusion in old rats].

OBJECTIVE: To explore the relationship between vascular dementia and nerve growth factor (NGF) and brain-derived neurotrophic factor (BDNF). METHODS: 42 male and female senile Wistar rats were randomized into control and experimental groups. The control group was subdivided into A, and C three groups and the experimental group into 6 subgroups, i.e 15 min of ischemia alone and ischemia combined with reperfusion of 1 h, 6 h, 2 d, 4 d and 9 d. Swimming maze was performed before and after operation. The contents of NGF and BDNF in different parts of brain were examined in different periods of ischemia and reperfusion with ABC method of immunocytochemistry. RESULTS: NGF expression in frontal lobes reached a peak level in 2 days after reperfusion and then disappeared quickly, that in parietal lobes showed a moderate level in 15 minutes of ischemia and reached a peak in 9 days of reperfusion. However, there is no expression in thalamus in early ischemia and reperfusion, but reexpression occurred after 2 days of reperfusion. BDNF expression showed a peak level after 2 days of reperfusion and then decreased in frontal lobes, but a high level of expression that was unchanged in parietal lobes and hippocampal areas. The increasing expression in thalamus occurred only 9 days after reperfusion. CONCLUSION: The distribution of BDNF was wider than that of NGF. There was a good protective action of NGF and BDNF in frontal lobes in the early period of ischemia with reperfusion and there existed persistent and prompt action of the neurotrophic factors in parietal lobes and hippocampal areas. The protective action of NGF in thalamus was poor, but that of BDNF was good.
