Audiologic manifestations in fetal alcohol syndrome assessed by brainstem auditory-evoked potentials.

In 29 of 36 children with fetal alcohol syndrome brainstem auditory-evoked potentials were performed and the auditory threshold was determined. The hearing of patients with a pathologic finding was then subject to a thorough ear-nose-throat examination. Additionally the IQ was determined and an audiolinguistic examination was carried out. Seventy-five % of the children showed a peripheral hearing disorder on the basis of conductive hearing loss. In two children (7%) sensorineural hearing loss was located to the inner ear. A dysfunction of the auditory pathway was found in 6 patients (21%). The results of an isolated ear-nose-throat examination in 7 further children supported these findings. The mean performances in the linguistic tests were far below average and displayed a strong relation to the IQ. This study shows that a peripheral hearing disorder, especially an obstruction of the auditory tube as well as dysfunctions of the inner ear and the central hearing pathway, are different manifestations of the fetal alcohol syndrome. An etiologic connection between craniofacial abnormalities in these children and an increased susceptibility to peripheral hearing disorders could not be demonstrated. To avoid a retardation of the linguistic development in addition to the intellectual deficits a strong emphasis must be put on an early and comprehensive auditory examination of children with FAS.

[Haemophilus influenzae meningitis in childhood].

From January 1971 until the end of 1989, 94 infants suffering from meningitis caused by Haemophilus influenzae were treated in the Department of Pediatrics of the University of Leipzig. The anamnestic data, signs and symptoms, laboratory findings, the courses and final results of treatment are described. 88 children survived; in eight of them neurological or mental sequelae were found on discharge. Generally, a certain stagnation regarding treatment results is observed over the observation period. Therefore vaccination against Haemophilus influenzae type B should be encouraged.

[Clinico-neurologic and neurophysiologic findings in hemophilic boys and their effect on social integration].

Prophylactic substitution of severe (sometimes even moderate) haemophilia (home self-treatment) has, besides other measures, not only had a positive effect on the findings in the joints but also on the danger of bleeding in the CNS. The injuries of the head in haemophiliacs are analysed and the social integration is shown (school, occupation, spare-time planning). Compared with previous information in literature the high incidence of pathological EEG findings has decreased. Without observing clinically perceptible deficits, the acoustically evoked brain stem potentials were often found to be altered.

[The EEG of patients with tetralogy of Fallot with and without hypoxemic seizures].

Electroencephalograms were recorded of 135 children with TOF. They were normal in 51%, slightly disturbed in one-third, and severe disorders were seen in 16% of patients. Although not statistically significant, cyanotic spells tend to be associated with more severe impairment of the bioelectrical brain function. As far as patients develop additional neurological complications, these result in severe electro-encephalographic changes. Thus, electroencephalographic findings suggest early corrective surgery.

[Effect of dietary non-compliance on development in phenylketonuria--studies of 14-year-old patients].

25 children with early treated PKU were studied at the age of 14 years. The IQ was higher at the age of 6-8, 10 and 14 years if the dietary control was good (75% of the control values up to 10 mg/dl) compared to children with poor control. The IQ however decreased up to the age of 14 years in both groups. Discontinuation of the diet in children with a good dietary control at the age of 6 years because of a normal EEG after phenylalanine loading causes a decrease of the IQ from 100 at 6 years to 90 at 10 years. The IQ remains stable thereafter up to 14 years. The IQ ist lower at the age of 7 or 8 years in those children in whom the discontinuation of diet is delayed because of abnormal EEG after phenylalanine loading at the age of 6 years, but remains stable up to the age of 14. According to these results a discontinuation of the diet at the age of 6 years can not be recommended even if the EEG is normal after phenylalanine load.

[Results, problems and consequences of early treatment of phenylketonuria].

Screening of PKU was started in GDR by means of a napkin-test in 1967 and by Guthrie-test in 1971. A normal development is possible if treatment is started within the first 3 months of life and if the dietary control is good. Maintenance of a dietary control becomes difficult with growing age. The diet, however, should strictly be kept up to the age of 8 to 10 years. Thereafter there should be a low protein nutrition possible with supplementation of tyrosine. Prenatal diagnosis, which is possible in most of the families by RFLP technique, is justified despite the good results because of the social impact onto the family caused by the treatment. Treatment again must be started before pregnancy and must strictly be kept up to birth to avoid fetal damage by maternal PKU.

[Listeriosis of the central nervous system in children beyond the neonatal period].

Observations on three children with neurolisteriosis (one case of meningitis, two cases of meningoencephalitis, each Serovar 4 b), show that even after the neonatal period, listeriosis must not be ignored in the process of diagnosis and therapy. It is the bacteriological examination of the cerebrospinal fluid, together with the blood culture, and not clinical symptoms and serology that guarantee a timely diagnosis and therapy (ampicillin and gentamicin).

An epidemiologic study of febrile seizures and epilepsy in children.

Prevalence and annual incidence rates of febrile seizures and types of epilepsy were investigated in all children of the Altenburg district (German Democratic Republic) being 0-14 years old. The mean total population for the period 1982 through 1985 was 108,905. The proportion of children aged 0-14 years was 20,132. The average annual incidence of febrile seizures in children being 0-5 years old was 240.8/100,000. For children between 0 and 14 years the rate of epilepsy 51.1/100,000. Based on 87 epileptic patients, the exact point prevalence rate per 1000 on December 31, 1985, was 4.4.

[Pathogen spectrum in bacterial meningitis in childhood and subsequent therapeutic strategy].

In a total of 223 children over one month old suffering from purulent meningitis, there was a predominance (n = 96) of meningococci over hemophilus influenzae (n = 68) and pneumococci (n = 59). Crucial to therapeutic strategy for purulent meningitis is early diagnosis, in our laboratory covering both liquor and blood cultures. Initial therapy has to take account of these three chief causal agents. We have not as yet observed any resistance to penicillin from meningococci or pneumococci, and none of the liquor-cultivated hemophilus influenzae stock has been resistant to ampicillin. In the first two years of life, initial therapy for bacterial meningitis should include ampicillin, a liberal (300-400 mg/kg KM/d) dosage continuing to be important after the onset of improvement. In view of the lack of resistance of the causal agents cultivated, we had hitherto no cell to deploy modern cephalosporins in cases of bacterial meningitis in children.

[Hematological and hemostatic findings in Kawasaki's syndrome].

Hematological and coagulative-analytical findings are represented in 10 patients aged between 3 months and 12.9 years affected with a typical Kawasawski syndrome, a disease which, etiologically, has not been elucidated and which will predominantly occur at the early childhood. During the hemostaseological examinations the significantly increased fibrinogen level at the initial stage was particularly striking in 6 patients. Partially a considerable number of leukocytoses with a shift to the left appeared almost constantly during the acute stage of the disease. At the subacute stage, however, mostly moderate thrombocytoses could be detected. The differential-diagnostic as well as the therapeutic significance of such alterations and the present possibilities of treating them with acetylsalicylic acid or prednisolone respectively are evaluated.
