Choroid

Automated phakotomy and aspiration of soft congenital and traumatic cataracts.

The design and operation of an automated instrument for removal of a soft cataract through a small corneal incision has been described in a small series of young patients. The distinct advantages of the instrument and technique are the small single-entry corneal incision, the minimal manipulation on the part of the surgeon, the relative constancy of both the anatomical shape of the globe and of a predetermined intraocular pressure during the procedure, a decreased inflammatory response, and a rapid wound healing.

Adolescent

A fiber-optics cyclodialysis spatula.

A cyclodialysis spatula for glaucoma surgery with a fiber-optics bundle incorporated within its thickness is described. The instrument is used as is commonly done at present with the standard cyclodialysis spatula. In addition, the fiber-optics bundle delivers cold light to the tip of the spatula which is useful in helping the surgeon locate the spatula within the sclera and the anterior chamber.

Dialysis

Dacryoscopy.

A miniature caliber endoscope is described for introduction into the lacrimal excretory passages. Direct observation and diagnostic inspection of the contents and lining of the lacrimal sac and naso-lacrimal duct are possible with the instrument, and complement standard examination procedures of the lacrimal outflow system.

Endoscopes

Choroidography and photography of the long ciliary nerve and artery.

A noninvasive technique that allows photography of the choroidal vasculature and the long ciliary nerve and artery is described. It does not make use of any injectable contrast material. A simple single lens reflex camera with its close-up lens, mounted on a stand, was used to take illustrative choroidograms.

Angiography

A fiber-optics pig-tail probe.

A pig-tail probe with a fiber-optics bundle incorporated within its thickness is described. The instrument is used as is commonly done at present with the standard pig-tail probe. In addition, the fiber-optics bundle delivers cold light to the tip of the probe which has been found very useful for locating the torn ends of a canaliculus, as well as delineating the anatomy of the lacrimal sac.

Fiber Optic Technology

Biomicroscopical choroidoscopy (uveoscopy) and transillumination gonioscopy.

A fiber optic sleeve that fits over a Goldmann three-mirror lens, as well as an independent cable used in conjunction with the Goldmann lens, are described for transillumination biomicroscopical examination of the anterior chamber angle and the uvea. High-intensity retroillumination is provided to the anterior and posterior segments of the globe without production of heat to the eye. Present clinical uses as well as possible future research with the instruments are discussed.

Choroid

Transillumination of the angular and frontal veins.

A 500-W fiber optics cold light used to transilluminate the angular and frontal veins allowed quick, accurate delineation of the veins for percutaneous orbital venography. It also allowed the surgeon to avoid cutting the angular vein when operating on the lacrimal sac.

Face

Optic discoscopy through a miotic pupil.

A technique is described whereby visualization of the posterior pole of the retina is accomplished through a very miotic pupil. The technique is simple and quick to perform. A wide field of view is maintained as well as stereopsis.

Constriction
