The effect of occlusal relationships on the occurrence of sounds in the temporomandibular joint.

The aim of this investigation was to determine the influence of occlusal relationships on the occurrence of sounds in the temporomandibular joint. A group of 100 male subjects aged 24-52 years (X = 35.03+/-6.92) was examined. Analysis of occlusion included determination of the number of existing teeth, number of teeth in occlusion, overbite and overjet, type of occlusion, mediotrusion interferences, relationship of the retruded contact position (RCP) to intercuspal position (ICP), and the amount and direction of sliding from RCP to ICP. Sound was registered by means of a stethoscope and classified according to its character in click or crepitation. Sound was present in 29% of subjects. In 28% of cases it was registered as click and in 2% of cases as crepitation. One subject had simultaneous click and crepitation. The results of the statistical analysis indicate that overbite, type of occlusion, existence of mediotrusion interferences, the relationship of RCP to ICP, and the amount and direction of sliding from RCP to ICP do not have an influence on the occurrence of sounds. The risk of the occurrence of crepitation is significantly increased in the case of the loss of more than 5 teeth, and in the case of horizontal overbite larger than 7.5 mm (p<0.05).

Mandibular border positions in eugnathic subjects during the second bite lifting.

This research was aimed at performing a clinical functional analysis during the second bite lifting, in order to define mandibular border positions and possible differences by sex and/or age. Out of a total of 9680 examined children from the region of Medimurje, Croatia, 103 were found to be eugnathic, with dentition in occlusion during the phase of the second bite lifting. All the eugnathic patients belonged to the class I by Angle, without any caries, fillings or extracted teeth. They were divided into three predefined groups for the sake of comparison. The obtained values of mandibular border positions in eugnathic subjects during the second bite-lifting phase vary with age and with the number of present permanent teeth in occlusion. Generally, all the values are higher in girls than in boys, which speaks in favour of the assumption that mandibular border positions are determined by sex, hormonal activity, growth and intercuspidation. Also, girls show a higher TMJ flexibility as well as a higher mandibular mobility. The knowledge of these facts will contribute to a more precise treatment in child prosthetics and pedodontics.

Evaluation of craniometric methods for determination of vertical dimension of occlusion.

In clinical practice, fully precise method for exact determination of vertical relation of occlusion still does not exist. This study examines the relationship between different craniofacial distances and the distance subnasale-gnathion (Sn-Gn), which represents the lower third of the face in vertical relation determination. The highest coefficient of correlation was (r = 0.8678, p < 0.05) between the distance eye-ear (E-E = lateral border of the ocular orbit-medial opening of the meatus of the external auditory canal) and Sn-Gn. The prediction of the distance Sn-Gn could be determined through the formula: Sn-Gn = E-E/1.08 or through the regression analysis: Sn-Gn = 1.9197 + 0.6449 x E-E. None of the calculated coefficients of correlation was 1, but was < 1, so that the prediction of the distance Sn-Gn by craniometric distances is not absolutely reliable, although it is considerably helpful. Our results point at the variations of craniofacial distances in the Croatian population. Yet, craniometry could still be recommended in everyday clinical practice for prediction of vertical relation of occlusion, as it is a simple, economic and non-invasive method, however in combination with some other methods, which have proved to be helpful.

Estimation of referent cephalometric parameters in dental prosthetics.

Estimation of the inclination of the occlusal plane and its spatial orientation during the insertion of plaster casts in the articulators space, and control during the clinical phase of the prosthetic work, is one of the key problems in dental prosthetics. The aim of this investigation was to compare the relations of the basic roentgencephalometric parameters applied and to determine their reliability. Five angular variables were analysed: OP-MdP, OP-PP, OP-CP, OP-FP and OP-SNP, on a sample of 86 radiographs of subjects with almost normal occlusion by applying basic statistical parameters and the correlation's analysis. The results gave mean values of the investigated variables with estimation of their variability and level of mutual correlation, which can be used when determining individual inclination of the occlusal plane. Parameters FP and SNP showed significant stability and reliability which can be successfully applied in prosthodontics.

Modification of the angle of the lower third of the face.

Determination of vertical dimension of occlusion by roentgencephalometric method, with the help of the angle of the lower third of the face, has been described in literature. The aim of this study was to modify the angle of the lower third of the face and correlate its determination with reference planes for orientation of casts in the articulators space, to determine mean value and measures of variability of the modified angle in a random sample, and the possibility to use this angle as a control value for establishing the height of the lower third of the face. On a sample of 131 roentgenograms in L-L projection, with teeth in maximal intercuspation, points: N, IS47, THA, SNA, SNP, Xi and Pg were traced down. By the use of this points the angle of the lower third of the face (angle K) was constructed. The values of the angle were measured and statistically analyzed. Mean value of the angle was 48.5 degrees, with standard deviation of 4.41 and coefficient of variation 9.09%. The investigated angle can serve as a control value for determination of intermaxillary relationships and should not be accepted as an strict norm.

[Comparison of location between nasion and orbital points on soft tissue and on bony structures, in articulatory technique].

A skin projection of the orbitale as palpated through soft tissues was designated on the left cheek of 22 subjects. The nasion, orbitale and cephalostat nasion direction medium were plotted on transparent paper after teleroentgenography. Mutually parallel lines were drawn through the points, the line obtained on the construction of the nasion direction medium used as a basic one. Distances between the lines were measured and results statistically processed. Transfer of the model into the articulatory region by means of the orbitale and buccal arch nasion direction appeared not to orientate reliably the model according to reference planes of the head. Our results are consistent with literature data available.

[Evaluation of the reconstruction of occlusal plane by use of Ponans angle].

In this study, teleroentgenocephalograms of 34 subjects with at least 20 natural teeth were used. Correlation between the angle formed by linking the PORION-NASION-SPINA NASALIS ANTERIOR (PONANS angle) points and the angle made by Frankfort horizontal and occlusion plane was studied. The coefficient of correlation was 0.1263. It is considered that there is no correlation if the coefficient of correlation is less than 0.5. Thus, there was no correlation in the study sample, which is consistent with the results reported by Karkazis and Polyzois, but in disagreement with those published by Monteith. In our sample, the occlusion plane could not be oriented in the articulatory space or in toothless mouth by means of the method proposed by Monteith.

[Radiographic analysis of condyle position in patients with TMJ dysfunctions].

Decreased joint space is one of the basical radiographic signs of joint disease and correlation with crepitus the real clinical symptom of structural joint damage. The aim of this investigation is to find out if there is any correlation between the intrajoint condyle position and joint dysfunction symptomatology, by means of TMJ tomography evaluation. Among the patients with characteristic symptoms of TMJ dysfunction examined in our Departments, a group of hundred were tomographed (Polytom--Philips-Massiot). Thirty students not showing any anamnestic or clinical symptoms formed the control group. In the evaluation of radiographic findings we accepted the static relation between condyle and glenoid fossa in the maximal intercuspal position, where the joint space was divided in three almost equal parts. The radiographic findings of the relationship between condyle and glenoid fossa in the maximal intercuspal position are presented on with the intention of establishing the most characteristic differences between the groups. The statistical elaboration has been carried out by multivariante analysis of variance. The characteristic radiographic findings in our group of patient are: --excentric condyle position, --posterior condyle displacement and --marked asymmetry. Therefore, these findings represent efficient aid in TMJ dysfunction diagnosis, and show direction for repositioning of condyle in therapy dysfunction.

[Glass-ionomer cements].

Glass-ionomer cements represent a new type of dental materials widely clinically applied owing to their good adhesion to dental tissue, caries-protective effects, biocompatibility and simple handling. They are two-component materials commercially available in forms of powder or liquid. The two components are mixed manually or automatically in a vibrator to produce a compact mixture. In addition to chemical, light-polymerizing hybrid glass-ionomer cements are also available. A description is given of physical and chemical features of these materials, their classification and use in particular clinical cases.

[Positions of the central relation and central occlusion as recorded by a functiograph in patients with a stomatognathic system dysfunction].

Positions of the central relation and central occlusion were intraorally graphically recorded in 100 patients with a stomatognathic system dysfunction and in 0 control subjects. Results of the study showed asymmetry of these two positions to play a major role in the etiology of dysfunction. Occlusal program had an almost two-fold impact on the graphic tracing of the central occlusion position, as compared to the influence exerted by the articulation and muscle on the central relation tracing. Instrumental functional analysis and the instrument used alike, are simple to use and allow an objective assessment of the state to be made.

[Design of bond cast bridges].

A technique of etching was employed in 50 patients for bridge retention. The Rexillium III (Jenevie Industries, Wallingford Co.) alloy and Comspan bond (L. D. Caulk Co., Milford Del.) were used. Therapeutic results obtained with adhesive (etched) bridges depend on a variety of factor: careful and accurate indication, precise work plan, minimal preparation of the supporting tooth, appropriate method of retention of the attachment metal portion, and the type of bond and of dental alloy. Clinical application of bond cast bridges has a quite good prognosis. A prolonged period of observation should provide more data on the behavior of such appliances in the oral cavity and allow us evaluations.
