Synthesis and pharmacological evaluation of 2, 3-dihydro-3-oxo-4H-thieno[3,4-e][1,2,4]thiadiazine 1,1-dioxides as voltage-dependent calcium channel blockers.

The synthesis of a novel series of 2,3-dihydro-3-oxo-4H-thieno[3, 4-e][1,2,4]thiadiazine 1,1-dioxides and their pharmacological evaluation as drugs with effects on the rat cardiovascular system are described. The compounds under study were synthesized via Curtius rearrangement of appropriate sulfamoylacylazides which, in turn, were prepared from known starting materials. In isolated rat portal vein, these thienothiadiazines, like verapamil and diazoxide, inhibited the spontaneous motility produced by KCl (20 mM). In addition, the new compounds, like verapamil and unlike diazoxide, also exhibited inhibitory effects in the same preparation when the cell membrane was depolarized by an increased extracellular KCl concentration (80 mM) and, consequently, the membrane potential approached a level close to the K(+) equilibrium potential. Further characterization of this inhibitory activity led to the identification of a selective inhibitory effect of the new compounds on KCl (80 mM)-induced 45Ca(2+) uptake in the same vascular tissue. When tested in vivo (anaesthetized normotensive rats), acute administration of verapamil, diazoxide and some of the most in vitro potent compounds in 45Ca(2+) uptake experiments produced a gradual, dose-dependent and sustained decrease in diastolic arterial blood pressure, devoid of cardiac effects. These results suggest that, like verapamil, the cardiovascular effects produced by the new thienothiadiazines seem to be due, at least in part, to a blockade of transmembrane voltage-dependent calcium channels present in vascular smooth muscle cells and not to an activation of ATP-sensitive K(+) channels. Compounds 5b, 5e and 5i have been selected for further studies as antihypertensive agents.

Long-term follow-up of epidural blood patch.

Epidural blood patch (EBP) was performed for the treatment of severe postlumbar puncture cephalalgia in 118 young patients. Following the first EBP, 105 patients had relief of headache. Eleven of the 13 in whom it failed had a second EBP, with adequate relief in 10, giving an overall success of 97.5 percent. Lumbar epidural, caudal, and spinal procedures were successful in 3 patients 105 to 380 days after EBP. Soon after EBP, one patient developed facial paralysis and one complained of episodes of vertigo, dizziness, tinnitus, and ataxia without headaches. Residual complications included backache and/or back stiffness in 22 patients and paresthesia in two. Two-year follow-up revealed 95 percent patient acceptance of the procedure. EBP was found to be a safe, effective method for treating severe postlumbar puncture cephalalgia, provided a proper diagnosis is made and there is no contraindication.

[Serum pro-oxidant and antioxidant factors and risk of Parkinson's disease: population study].

INTRODUCTION: Several studies suggested a role of 'oxidative stress' (increased production of prooxidants, antioxidants deficiencies or both) in the pathogenesis of Parkinson's disease. In this study we have measured the serum levels of a number of prooxidant and antioxidant substances to evaluate their possible relation with the risk for Parkinson's disease. PATIENTS AND METHODS: We assessed the serum levels of iron, ferritin, ansferrin, ceruloplasmine, vitamin A, alpha-carotene, beta-carotene, and alpha-tocopherol, in 28 patients with Parkinson's disease and 85 matched controls. All of them were recruited from a population study. RESULTS: None of the values studied differed significantly between the two study groups, and none of them were correlated with age at onset, duration of the disease, scores of the Unified Parkinson Disease Rating Scale or the Hoehn and Yahr staging in the Parkinson's disease group. CONCLUSIONS: These results confirm the previous findings of classic case-control studies, suggesting the absence of relationship of the studied values with the risk for Parkinson's disease.

[Antibiotypes of bacterial strains isolated from patients at the intensive care units].

The indiscriminate use of antibiotics in intensive care units has permitted the selection of multiple resistant bacterial strains in the hospital setting. Antibiograms present the sensitivity or resistance of these strains to drugs in current use. In this study, 100 bacterial strains were isolated from patients on medical and surgical ICU wards. The strains were identified by API-20E and sensitivity testing was performed by the E-test procedure. MIC were further analyzed with the WHONET software wich permits the identification of antibiotypes. Acinetobacter anitratus, Pseudomona aeruginosa and Enterobacter cloacae were the most resistant strains. Nine antibiotypes were determinated for A. Anitratus. Predominant strain presented antibiotypes with multiple resistance to ten antibiotics. P. aeruginosa presented 10 antibiotypes, the predominant one being CTX, CRO, CTX. Antibiotype determination allows phenotypic identification or resistance patterns in particular species as well as facilitates follow up and recognition of its epidemiological distribution.

[The nutritional status and immunological situation of cancer patients].

A study was performed on the nutritional state of a sample of 138 patients with digestive and extradigestive neoplasias. The study protocol was based on anthropometric, analytical and immunological tests. The main criteria for malnutrition were as follows: loss of 10% or more in normal weight and/or albumen levels of less than 3.5 gr/dl. A global malnutrition percentage of 56.5% was obtained. The highest malnutrition percentages were found among patients with neoplasia of the oesophagus (78.9%) and the lowest in neoplasias of the breast (9.1%). From the nutritional standpoint, neoplasias of the rectum showed an extradigestive behaviour, with a low malnutrition percentage (40%) compared to digestive tumors. The global malnutrition level for the sample studied was 56.6%. The muscular compartment was the most affected in cases of malnutrition. In the PCHR results, no evidence of the effect of the nutritional state on the alteration of these was recorded (Chi 2 = 6.42, p = 0.207).

[Comparison of 4 microscopy techniques for the diagnosis of leptospirosis in wild rodents in a rural area of Valdivia, Chile].

Kidneys of six different species of 93 wild rodents captured in the rural area of Valdivia (Chile) were simultaneously examined for leptospira infection by means of Levaditi silver stain, dark ground microscopy in wet smears, and immunofluorescence and immunoperoxidase techniques using pooled antiserum against hardjo and pomona serovars. Leptospira was shown to be present in 40 (43.0%) rodents. They were detected in five of the six species: Akodon olivaceus, Akodon longipilis, Rattus rattus, Oryzomis longicaudatus and Mus musculus. Levaditi's technique detected the highest number of positive samples (67.5%) and the dark field microscopy the lowest (32.5%). The detection of leptospira in the kidneys of these wild rodents suggests that their urine may play an important role in the dissemination of leptospirosis in this area of Valdivia.
