Guillain-Barré syndrome associated with acute hepatitis A.

A case of Guillain-Barré syndrome in a 49-year-old male is described. The syndrome presented as tetraplegia with sensory impairment and the pre-icteric phase of acute hepatitis A. Both viral hepatitis and neurologic disturbances improved, although neurologic changes lasted for 3 months after the onset. HA IgM antibody in cerebrospinal fluid was first manifest with onset of neurologic symptoms. On the basis of this finding and some reports in the literature it seems possible that the Guillain-Barré syndrome may be related to the immunopathogenetic mechanism of hepatitis A virus infection.

Transformation of human embryonic kidney cells by a viable deletion mutant of BK virus.

Infection by a viable deletion mutant of BK virus (pm-522) of human embryonic kidney (HEK) cultures originating from different fetuses generated cell lines, designated pmHEK, resistant to superinfection by the virus. However, infection of HEK cultures by a cloned wild-type BK virus (wt-501) did not generate a cell line. In medium with 10% calf serum all pmHEK cells reached saturation densities significantly higher than those of HEK cells and could grow in medium containing 1% serum. They did not form colonies in soft-agar medium, and had limited life-spans greatly extended beyond that of HEK cells. These results suggest that pmHEK cells are partially transformed. T antigen was uniformly expressed by all pmHEK cells, while V antigen was present in only a small minority of the cells. Thirty to 5,000 copies of the viral DNA per cell were detected, primarily in a nonintegrated form, in all pmHEK cell lines and the clones isolated from one of them. Since at least in two pmHEK lines, the actual quantities of free viral DNA per cell were significantly greater than those estimated on the basis of the assumption that free viral DNA was produced only by the V-antigen-positive cells, it can be concluded that free viral DNA is also present in the V-antigen-negative cells predominant in the cell populations. Although the existence of a few copies of integrated viral sequences in pmHEK cells was not ruled out, the large amount of free viral DNA present in each cell probably plays a prominent role in the production of T antigen required for maintenance of transformation.

Thymidine factor in human cord sera and relationship to birth size, gestational age and insulin.

Thymidine factor was studied in human cord sera from 44 neonates with gestational ages ranging between 36 and 43 weeks. Thymidine factor was determined by the uptake of 6-3H-thymidine using isolated chick embryo cartilage cells. The thymidine factors increased with gestational age and those of neonates at 40 weeks of gestation were 0.89 +/- 0.17 U/ml (mean +/- SD), but were significantly lower after 42 weeks of gestation. There was no correlation between thymidine factor in cord sera and birth weight or size. There was a positive correlation (p less than 0.02) between insulin level and thymidine factor in cord sera. Our data suggest that somatomedin measured as thymidine factor might not have a major role in fetal growth and insulin might be an important factor in the synthesis of somatomedin by the fetus.

Nutritive quality of leaf proteins coagulated at different pH.

Leaf proteins obtained by coagulation at different pH were examined for their chemical composition and nutritional quality. Green juice was extracted from alfalfa, red clover, Italian ryegrass and oats and leaf protein was coagulated by heating the juice after adjusting the pH to 4 or 8-8.5, or without any adjustment of the pH (about pH 6). The mild alkaline juice from Italian ryegrass and oats did not cause the satisfactory coagulation but it was achieved with the addition of Ca salt to the juice before heating. There were no important differences in the amino acid compositions of the leaf proteins coagulated at different pH. Crude ash, Ca and Mg contents increased with an increase in pH of coagulation and the protein coagulated at pH 8 had remarkably high contents of crude ash, Ca, Mg, Na and P in each crop. The leaf protein coagulated at pH 6, on the contrary, had a high content of true protein and low contents of nitrogen free extracts and nucleic acid as compared with those at pH 4 and 8. The pH of coagulation of leaf protein from alfalfa and red clover had no effect on the nutritional quality of the respective proteins. In Italian ryegrass and oats, however, the leaf protein coagulated at pH 8 was found to be nutritionally inferior to those coagulated at a lower pH. The data presented in this work support that in general, the leaf protein produced by heating the green juice without any adjustment of pH may be suitable for protein resources because of its desirable properties, i.e. high true protein content and good nutritional quality.

[Clinical evaluation of cefroxadine in the field of obstetrics and gynecology].

Cefroxadine (CXD), an oral cephalosporin antibiotic was studied in the field of obstetrics and gynecology and the following results were obtained. CXD was orally given to 22 cases at daily dose of 1,500 mg 3 times a day. CXD administration was given to 22 cases in all; 4 with cervicitis, 6 with endometritis, 2 with puerperal fever, 4 with bartholinitis, 5 with adnexitis and 1 with vulvitis, respectively. Overall efficacy rate was 77.3% (17/22) (excellent 4, good 13, fair 5). As for side effects, a slight diarrhea was observed. CXD was considered to be a useful antibiotic in the field of obstetrics and gynecology by above the results.

Molecular mechanism of the effects of guanine nucleotide and sulfhydryl reagent on muscarinic receptors in smooth muscles studied by radiation inactivation.

The molecular sizes of the units concerned in 3-quinuclidinyl benzilate (QNB) binding and in the effects of guanine nucleotide and sulfhydryl reagent on the inhibition of QNB binding by carbachol in smooth muscle of guinea pig ileum were determined to be 76,000, 179,000 and 107,000, respectively by the radiation inactivation method. One or more subunits (GTP subunit) other than the receptor subunit in a muscarinic receptor appeared to be involved in the effect of guanine nucleotide. When guanine nucleotide was present, the receptor subunit seemed to be dissociated from the GTP subunit.

Effects of trypsin-treatment on agonist binding affinity to the muscarinic acetylcholine receptor.

Trypsin-treatment of the microsome fraction of the ileum and the synaptic membrane fraction of the cerebral cortex of guinea-pig caused selective reduction in the apparent affinity of an agonist (carbachol), but not an antagonist (atropine), to muscarinic acetylcholine receptors (mAChR), measured as inhibition of binding of 3H-quinuclidinyl benzilate (3H-QNB). This effect was similar to that of Gpp(NH)p. The effects of trypsin and Gpp(NH)p were not additive. On the other hand, treatment of these fractions with 5,5'-dithiobis (2-nitrobenzoic acid) (DTNB) increased the apparent affinity of agonist, but not antagonist. The effect of DTNB predominated over those of trypsin and Gpp(NH)p, when the fractions were treated with two reagents simultaneously.

Desensitization of the alpha adrenergic receptor system in guinea pig vas deferens.

Desensitization induced by alpha adrenergic (alpha-Ad) stimulation was investigated in organ cultured vas deferens of guinea pig. Brief exposure (1-2 min) of the muscle to noradrenaline (NA) caused short-term desensitization to both NA and acetylcholine (ACh), but not to high K+. After removing the agonist this desensitization completely disappeared within 15 min. Prolonged exposure to NA (i.e., cultured with NA for 3-24 hr) elicited long-term desensitization to NA, ACh and K+ (50 mM), but it did not change the maximal contraction by high K+ (154 mM). After removing NA from the culture medium the response to the agonist was restored to normal within 24 hr, but not within 15 min. The number and affinity of alpha-Ad and muscarinic ACh receptors, which were measured by the binding of 3H-WB4101 and 3H-QNB, respectively, were not changed in the muscle during these treatments. Moreover, long-term desensitization, but not short-term desensitization, was depressed by the concomitant presence of cycloheximide. The possible mechanisms of desensitization were discussed in comparison with those of various receptor systems.

Tumorigenicity of human BK papovavirus plaque isolates, wild-type and plaque morphology mutant, in hamsters.

The polyoncogenic prototype human papovavirus BKV (Gardner's strain) produced clear, small and large plaques. Two small-plaque isolates (wt-500 and wt-502), as well as a large-plaque-forming isolate (wt-501), induced frequent brain tumors and occasional osteosarcomas (but no insulinomas) in hamsters. The large-plaque former (wt 501) was more tumorigenic than the two small-plaque mutant (pm-525), which had been rescued from hamster tumor cell lines and have small deletion near the origin of DNA replication, induced frequent brain tumors and insulinomas. Mutant 522 was about five times more tumorigenic than wt-501. These results support the hypothesis that the polyoncogenicity of prototype BKV is accounted for by the presence of BKV mutants.

Mechanism of agonist-induced degradation of muscarinic cholinergic receptor in cultured vas deferens of guinea-pig.

The mechanism of agonist-induced degradation of muscarinic cholinergic receptors (mAChRs) was examined by means of [3H]QNB binding by organ-cultured guinea-pig vas deferens. Long-term exposure to ACh decreased the surface mAChR. This so-called 'down regulation' of mAChR was significantly inhibited by anti-endocytotic drugs. Anti-microtubular agents also strongly inhibited the decrease of mAChR remaining on the cell surface. Ammonium chloride and protease inhibitors, which are known to inhibit lysosomal enzymes, had little effect on the decrease and no increase in intracellular mAChR could be detected under these conditions. Chloroquine blocked the decrease in mAChR remaining on the cell surface. Based on these findings, the involvement of clustering and endocytosis in mAChR degradation were considered as possibilities. Furthermore, contraction of the smooth muscle to ACh in long-term desensitization was also examined in relation to the number and nature of the receptors. When the muscles were cultured with ACh and chloroquine or vinblastine, there were indications that the surface mAChR and the contractile system were uncoupled.

Postoperative results of Yoshino total knee prosthesis. A report of 264 arthroplasties performed between 1974 and 1980.

A review was made of 264 Yoshino total knee arthroplasties performed on 154 patients between February 1974 and July 1980. The average duration of the follow-up was 3 years and 2 months. Eighty-six percent were rheumatoid patients. Complications occurred in seven patients. The evaluation results indicated excellent relief of pain and improvement in walking ability. However, pain of the patellofemoral joint was noted in a number of osteoarthritic patients. This implies that a patella implant is necessary in the case of osteoarthritis. In addition, a new surgical technique was performed on "stiff" knees to improve their degree of movement; the average range of movement achieved in these patients was 70 degrees.
