[Comparison of densitometric and morphometric measurements of global and regional functions using a fully digital subtraction unit].

The value of digital subtraction angiocardiography for evaluating left ventricular movement by a morphometric and densitometric technique was examined by animal experiments (dog). Reversible reduction of global and regional left ventricular function can be quantified by both methods equally well. For judging regional ejection fraction, sector display is more easily comprehended than an ejection-fraction map. For the estimation of global cardiac ejection fraction in 49 patients, morphometric diagnosis was clearly superior to cine cardio-angiography (rmorp. = 0.91; rdens = 0.65). For recognising and quantifying regional abnormalities of cardiac movement, semi-quantitative visual gradation is superior to densitometric measurements.

Prognostic value of chest radiograph, serum-angiotensin-converting enzyme and T helper cell count in blood and in bronchoalveolar lavage of patients with pulmonary sarcoidosis.

We studied the prognostic value of the initial radiologic stage, the serum-angiotensin-converting enzyme (SACE) and T helper cells in blood (OKT-4-Bl) and in bronchoalveolar lavage (OKT-4-BAL) for patients with biopsy-proven pulmonary sarcoidosis. Thirty-seven patients without prior treatment were followed up for a period of 2 years. A BAL was performed in 22 of them as part of the diagnostic workup. Clinical examination, chest radiographs, vital capacity, diffusion capacity for CO, airway resistance and PaO2 at rest and during exercise were determined initially and after 6, 12 and 24 months. According to these results, patients were classified as having progressive or nonprogressive disease. The radiologic stage and the initial SACE (38.3 +/- 10.2 vs. 43.7 +/- 13.0 nmol/ml/min) could not discriminate between the two groups. Patients with progressive disease had significantly fewer OKT-4-Bl cells (403.3/microliter +/- 146.7/microliter vs. 842.0/microliter +/- 430.1/microliter) and more OKT-4-BAL cells (24.5 +/- 15.4% vs. 7.0 +/- 2.6%) than patients with stable disease (p less than 0.01). A negative correlation between OKT-4-Bl and OKT-4-BAL cells was shown (Rs = -0.79; p less than 0.001). We conclude that the number of OKT-4-Bl and OKT-4-BAL cells can be used as prognostic parameter for patients with sarcoidosis.

[Visualization of enlarged parathyroids by digital subtraction angiography in 22 patients with primary or secondary hyperparathyroidism].

Digital Subtraction Angiography (DSA) is a suitable method for visualising hypercrinal parathyroid glands on account of its high power of resolution. Hyperperfused areas stand out in contrast when using largely standardised imaging techniques with peripheral venous injection of contrast medium. Whereas 15 of 17 hyperplastic parathyroids were seen via DSA, it was not possible to identify on the subtraction image 2 out of 6 adenomas and one carcinoma. In our group of patients, surgical confirmation was obtained in 21 of 32 hyperperfused areas found via DSA (66%) and 9 out of 17 structures with poor echo (53%) in ultrasound.

[Digital subtraction angiography of aortocoronary bypass grafts in the early postoperative phase with peripheral vein x-ray contrast media injection].

Using a fully digitalised subtraction unit, aortocoronary bypass grafts can be visualised in the early postoperative phase by means of peripheral venous contrast medium injection (57 out of a total of 60 cases). In more than one-half of the cases (31 out of a total of 60), it is possible to outline sharply the bypass grafts from the beginning to their entry into the bridged-over coronary artery, in such a manner that any loops, kinks, stenoses or reduced flow rates can be recognised. This examination is not considered to be an invasive one by the patient, and the cardiosurgeon can welcome it as a valuable additional means of controlling and assessing the results of surgery.

[Anatomically shaped, unsharp, lead-containing acrylic glass filter for optimization of thoracic roentgenography].

Acrylic glass filters fitted near to the tube and shaped according to the patient's anatomy with a lead equivalent of 120 mu enhance the diagnostic value of a plain radiograph of the chest with 95% of the patients. The pulmonary vessels and mediastinal structures can be assessed on film. Compared with the X-ray film without filter, the pulmonary vessels are visualised with equal clarity. Accurate adjustment of the patient is facilitated by the fact that the filter limits show up in the light viewfinder. Repeat X-ray films are necessary in less than 2% of the case. Artifacts are caused by anatomical chest variations as well as by large patients, and hence the use of the filter should be omitted in about 5% of the patients.

[1st results of functional RES scintigraphy with 99mTc-labeled human serum albumin millimicrospheres in progressive scleroderma: possibility of early diagnosis of lung involvement?].

Functional RES scintigraphy with 99mTc-labeled human serum albumin millimicrospheres (99mTc-HSA-MM) was conducted in 11 female patients suffering from progressive scleroderma. The RES scan revealed abnormalities in the bone marrow in eight patients as well as pathological changes of the liver in 2 cases. 7 patients showed diffusely enhanced concentrations of 99mTc-HSA-MM in the lung, whereas X-ray picture and pulmonary function test revealed pathological findings in only 4 patients, respectively. Humoral inflammatory and immunologic parameters, too, indicated abnormalities less frequently than the lung scan. Thus functional RES scintigraphy seems to be a very sensitive approach to the assessment and possibly to the early diagnosis of lung involvement in progressive scleroderma.

[Determination of global and regional ventricular ejection fraction by a fully digitalized technic. Results of digital subtraction angiocardiography following peripheral venous contrast injection compared to cineventriculography, 2-dimensional echocardiography and radionuclide ventriculography].

Subtraction angiocardiography (DSAK) with a fully digitalised system (DR 960) provides a well defined demonstration of the left ventricle after peripheral venous contrast injection. Cardiac volume and ejection fractions were calculated by a dedicated software programme and the findings correlated with cine ventriculography (CA) (r = 0.91), biplane echo cardiography (2 DE) (r = 0.77) and radionucleid ventriculography (RNV) (r = 0.85); the method can be used even with reduced cardiac output (EF less than or equal to 15%). Densitometric measurements of the EF on phantoms correspond with morphometric findings, but its clinical application is at present not possible because of the frequent appearance of artefacts. Analysis of regional disturbances of ventricular wall movements shows very good correlation between DSAK and CA and these appear more accurate than 2 DE. DSAK is a valuable, rapid and reproducible method with few side effects for cardiological diagnosis.

[Induction of complete remission with adriamycin and cis-platinum in invasive thymoma].

Complete clinical remission was achieved in a very large lymphocytic thymoma with infiltration of both lungs and almost complete atelectasis of the left lung as well as superior vena cava syndrome using adriamycin (80 mg/m2, day 1) and cis-platinum (120 mg/m2, day 1). A dramatic reduction of tumour size was noticed already after the first cycle of chemotherapy. Remission still persists nine months after commencement of treatment.

[Dyssegmental dwarfism--report on two cases (author's transl)].

Report on 2 cases of dyssegmental dwarfism, a rare lethal form of dwarfism. Both male newborns died from asphyxia immediately after delivery. Its features are malformations of the first visceral arch (Pierre-Robin-Syndrome) malformations of all extremities (Camptomicromelia) and severe malformations of the spine (Anisospondyly), with the vertebral changes serving as the main differential diagnostic feature. Among other things, these malformations are due to defective enchondral ossification; and the disease is presumably autosomal recessive.
