The inhibitory factors of hematopoiesis in chronic hemodialysis patients treated with recombinant human erythropoietin.

In order to clarify possible factors responsible for varying responses in uremic patients treated with recombinant human erythropoietin (rHuEPO), we determined the inhibitory effects of ten uremic sera on the erythroid progenitors (CFU-E) and erythroid bursts (BFU-E). We also measured plasma EPO titers, Fe, UIBC, ferritin, PTH-C, beta 2-microglobulin, and aluminum in all ten patients. The inhibitor of CFU-E but not BFU-E, was present in the serum of the single anemic patient whose recovery took longer after the administration of rHuEPO. He did not have such conditions as iron deficiency, excess of aluminum, or chronic inflammation. The remaining patients, who had no CFU-E or BFU-E inhibitors, were good responders to rHuEPO. In none of ten patients were there inhibitors of granulocyte-macrophage progenitors (CFU-GM) or any differences in the other parameters. Although the inhibitory factor of CFU-E can be overcome with a larger dose, its prior determination may be useful to set out minimal effective dose of EPO treatment.

[A double-blind trial on the effect of bunazosin hydrochloride for the symptoms of benign prostatic hypertrophy].

A multicenter clinical trial was carried out on 372 patients in double-blind conditions in order to determine the clinical effects of Ea-0643 (bunazosin hydrochloride) on voiding disorders in benign prostatic hypertrophy, compared with paraprost and placebo. Of the 372 patients, 129 were assigned to bunazosin hydrochloride, 118 to paraprost and 125 to placebo. The improvement rating for all five subjective symptoms improved with passage of time in all the bunazosin hydrochloride, paraprost and placebo groups. A higher improvement rating was obtained in the bunazosin hydrochloride group for retarded urination, urinary stream condition and abdominal pressure at voiding, while the improvement rating was higher for prolonged urination in the placebo group and for residual urine in the paraprost group, but there was no significant difference in improvement ratings between the groups. The daily frequency of voiding decreased to a significant extent in the bunazosin hydrochloride and placebo group at week 1, and there was a significant difference between the bunazosin hydrochloride and the paraprost groups and between the placebo and the paraprost groups. The improvement rating for conditions of voiding was higher with the bunazosin hydrochloride group, when "slightly or better improved" cases were taken into account, but there was no difference between the groups. As for objective symptoms, maximum and average flow rate, useful measures for clinical evaluation of drug effects on voiding disorders, were significantly increased, with a decrease to match in residual urine ratio in the bunazosin hydrochloride group. In terms of maximum and average flow rate bunazosin hydrochloride was significantly superior to paraprost at weeks 1 and 2 and superior to placebo at weeks 2 and 4 and at the final evaluation as well. In terms of residual urine ratio bunazosin hydrochloride was superior to both paraprost and placebo. The global improvement rating, as assessed by the U- and chi 2-tests, was significantly higher in the bunazosin hydrochloride group than in the paraprost group, and there was a significant difference in global improvement ratings, as assessed by the chi 2-test, between the placebo and the paraprost groups, when "moderately or better improved" cases were taken into account. The stratified analysis of the prostate glands, subjective symptoms, maximum flow rate and residual urine ratio revealed that in patients with more advanced conditions the bunazosin hydrochloride group showed significantly superior improvement rates than the paraprost and placebo groups.(ABSTRACT TRUNCATED AT 400 WORDS)

[A comparative study of the effects of propiverine, terodiline, oxybutynin on the lower urinary tract function in the decerebrate dog].

The effects of propiverine, terodiline, oxybutynin on the lower urinary tract function was studied by cystometry in 44 decerebrate dogs. Micturition was induced by bladder filling before and after the intravenous administration of each drug. The statistical analysis was carried out on the urodynamic parameters. Propiverine at a dose of 5, 10 mg/kg significantly increased the bladder volume. And, there was a substantial increase in the functional bladder volume at a dose of 10 mg/kg. Terodiline at a dose of 5 mg/kg also significantly increased the bladder volume and residual volume. Oxybutynin at a dose of 0.5 or 1 mg/kg produced a substantial increase in the bladder volume and residual volume, and produced a significant decrease in contraction pressure. Furthermore, there was a significant decrease in the functional bladder volume at a dose of 1 mg/kg. This study compared the three drugs under the same test conditions. Only propiverine increased the functional bladder volume, thus suggesting promise for the relief of pollakisuria in the clinical applications.

[Effects of trospium chloride on the lower urinary tract function].

The effects of trospium chloride on the lower urinary tract function were studied by combined recording of cystometry and sphincter electromyogram in 13 decerebrated dogs. Micturition was induced by bladder filling before and after drug administration. A statistical analysis was carried out on the urodynamic parameters. Trospium chloride at the dose of 0.1 mg/kg showed a small but significant decrease in the threshold pressure during the collecting phase. In the urodynamic parameters of the emptying phase which is considered to be influenced by cholinergic activity, there was a significant decrease in maximum bladder pressure at 0.03, 0.1, and 0.3 mg/kg and a significant increase in residual urine at 0.3 mg/kg. Trospium chloride is probably acting as a antimuscarinic agent in this study. Trospium chloride appears to be useful for the relief of symptoms associated with hyperactive bladder contraction.

[Pulmonary annuloplasty in functional pulmonary regurgitation associated with mitral stenosis: a case report].

In order to prevent disadvantage of pulmonary regurgitation (PR) on cardiac function, pulmonary annuloplasty (PAP) was performed in a 58-year-old female. She suffered from severe mitral stenosis with right heart failure, and was in a state of cardiac cachexia. She was associated with PR due to pulmonary annular dilatation secondary to severe pulmonary hypertension. She underwent mitral valve replacement, tricuspid annuloplasty and PAP around the pulmonary annulus using a Teflon tape. No PR was observed by postoperative angiograms, and the operative method was proved to be effective on the treatment for PR. The PAP is recommended for patients with functional PR secondary to severe pulmonary hypertension.

Behavior of genetic markers in recipients after bone marrow transplantation and problems in forensic medicine.

The authors report studies on four pairs of donors and recipients in bone marrow transplantation (BMT). A broad range of gene markers at 41 gene loci, including 11 red blood cell markers, 5 human lymphocyte antigen (HLA) types, 12 serum protein markers, 5 red cell enzyme markers, and 8 salivary markers were evaluated before and after BMT over 2 months. As a result, 9 out of 41 gene loci of genetic markers in recipients were transformed into the donor type. BMT between family members may lead to transformation of gene markers, but within a pattern compatible with family inheritance patterns, and no genetic paradox will be found in later surveys of familial genetic relationships. However, in a personal identification system in forensic medicine using genetic markers as an index, the appearance of a phenotype incompatible with a blood relationship is possible after BMT with a non-blood-relative donor. This result is similar to the inheritance pattern observed after artificial insemination by a donor's semen (AID), a more complete out-of-family cross.

Elevation of the placental glutathione S-transferase form (GST-pi) in tumor tissues and the levels in sera of patients with cancer.

The content of glutathione S-transferase placental form (GST-pi) in colon and esophageal cancer tissues was determined by single radial immunodiffusion or activity inhibition tests. Average levels in colon adenomas and carcinomas were 163 +/- 38 and 143 +/- 30 micrograms/g, respectively, about 6-fold higher than the value of 25 +/- 4 micrograms/g observed for normal colonic mucosa. The content in esophageal cancer was similarly increased at 240 +/- 160 micrograms/g, about 6-fold higher than the level found in normal mucosa, 42 +/- 21 micrograms/g. The content was significantly higher in highly differentiated carcinoma (331 +/- 138 micrograms/g) than in moderately (205 +/- 123) or poorly (125 +/- 61) differentiated carcinomas, suggesting that GST-pi content seems to be related to the degree of differentiation of esophageal cancer. GST-pi was also expressed in cell lines derived from various cancers, including IMR 32, TE-9, and Ca Ski cells. The results thus indicate that GST-pi may be a useful marker for a wide range of cancers. An enzyme-linked immunosorbent assay developed to determine serum GST-pi content is described. Using this method GST-pi could be accurately measured in the range between 0.7 and 150 ng/ml, without interference of other isoenzymes. The serum GST-pi content was 1.3 +/- 1.2 ng/ml in 35 healthy controls and values of over 3.7 ng/ml (control mean + 2 SD) were found in patients with cancers of the stomach (10 of 23 cases), esophagus (26/43), bile duct (3/9), and colon (3/9), and in some leukemic cases. Although elevation of the serum content was not so often as that of tissue content, the fact that higher serum values of patients with esophageal cancer often reverted to the normal range after surgical removal of the cancer suggested a direct derivation of serum GST-pi from tumor tissues. Thus, follow-up of elevated serum GST-pi levels may be useful for monitoring cancer patients during the course of treatment.

Effect of adrenergic agents on urethral pressure and urethral compliance measurements in dog proximal urethra.

The effects of adrenergic drugs on urethral compliance and urethral closure pressure at the proximal urethra of dogs were investigated using a new probe. The probe, with a balloon at the tip, was equipped with two generating and two detecting electrodes. The probe was inserted into the proximal urethra, and saline was then infused at a constant rate into the balloon with an infusion pump. The pressure and cross-sectional area of the urethra were recorded and a correlation curve of these parameters was drawn. The regression (of the form y = ax + b) was determined from the curve. Urethral compliance and closure pressures were obtained from the values of "a" and "-b/a" respectively, and the maximum cross-sectional area was measured when no further distension of the urethra was obtained. Urethral compliance decreased after the administration of phenylephrine, but increased after the administration of prazosin and labetalol. The urethral closure pressure decreased after the administration of dopamine and haloperidol. There was no change in either urethral compliance or closure pressure after the administration of clonidine, yohimbine, isoproterenol, or propranolol. The results indicate that adrenergic drugs work on urethral compliance, and that the alpha-adrenergic receptor of the proximal urethra is of the alpha-1 type. The changes in the urethral pressure after dopamine administration seems to be a response mediated by dopamine receptors.

Further studies of salivary polymorphisms in the Japanese population.

The distributions of the phenotype and gene frequencies of nine polymorphic systems in human parotid and whole saliva were studied in the Japanese population of the eastern part of Japan and compared with those of populations in other areas of Japan and other ethnic groups. The gene frequencies were: Pa+ = 0.214; Pb1 = 1.000; Pr1 = 0.753; Db+ = 0.035; PmF+ = 0.394; Ph+ = 0.028; PIF+ = 0.733; Amy1V = 0.011, and s-AcpA = 0.217.

Effect of cerebellectomy on reflex micturition in the decerebrate dog as determined by urodynamic evaluation.

The effect of cerebellectomy on reflex micturition in the decerebrate dog was investigated by cystometric and striated urethral sphincter EMG studies. Before and after cerebellectomy, reflex micturition consisting of bladder contraction and spasmodic contraction of the striated urethral sphincter occurred when a critical degree of filling was reached. Cerebellectomy showed no influence on the striated urethral sphincter EMG activity. However, cerebellectomy produced a significant decrease in threshold volume and threshold pressure during the collecting phase, and also in the contraction pressure and voided volume of the emptying phase. The present study suggests that the cerebellum plays an inhibitory role in the collecting phase and a facilitatory role in the emptying phase during the entire reflex micturition cycle of the decerebrate dog. Further study will have to be done concerning the neurotransmission mechanism that causes these different effects in the collecting and emptying phases.

[Preventive effect of KZ-1026 on acute liver injury induced by CCl4 in rats: histochemical, enzyme-histochemical and ultrastructural studies].

The preventive effect of KZ-1026 on acute liver injury induced by CCl4 in rats was evaluated histochemically, enzyme-histochemically and ultrastructurally. Rats that received 50% CCl4 (2 ml/kg) intraperitoneally were sacrificed at 3, 7 and 24 hr. Remarkable reductions in hepatic glycogen, RNA and G-6-Pase activity were observed in the centrilobular area at 3 hr, and ballooned cells appeared in the mid-zone at 7 hr. At 24 hr, the above histochemical parameters in the hepatocytes of the centrilobular area and mid-zone were extensively reduced, while the number of ballooned cells in the mid-zone was increased. KZ-1026 (200 mg/kg) was given orally at 24 and 4 hr before, simultaneously with or 3 hr after CCl4 treatment, and each rat was sacrificed at 24 hr after CCl4 administration. Pretreatment with KZ-1026 24 hr before CCl4 administration prevented reduction of RNA, glycogen and G-6-Phase activity, as well as disruption of rER and proliferation of sER due to CCL4 toxicity. This preventive effect of KZ-1026 was reduced by posttreatment; however, only the decrease in cytoplasmic RNA was well prevented. These results suggested that KZ-1026 is protective against CCl4-induced acute liver injury.

[Effects of NB-818 on a transient bilateral common carotid arteries occlusion model in Mongolian gerbils].

Effects of NB-818, a new Ca2+ entry blocker, on a transient cerebral ischemic model in Mongolian gerbils were examined. Following bilateral common carotid arteries occlusion of 20 min duration, all the control animals died within 24 hr after reperfusion. The administration of NB-818 at 0.1 mg/kg, i.p., 30 min after reperfusion significantly reduced the mortality rate to 65.0% (P less than 0.01); and 0.01 mg/kg, i.p., had a tendency to reduce the mortality rate. In the nimodipine-treated groups, a similar effects in the ischemic model. Furthermore, the administration of NB-818 at 0.1 mg/kg, i.p., 15 min before occlusion also had a tendency to reduce the mortality rate. On the incidences of seizures induced by transient cerebral ischemia, there were no significant differences between each of the drug-treated groups and the control group. In NB-818 and nimodipine-treated groups, the morbidity scores NB-818 and nimodipine in this severe cerebral ischemic model have advantages over those of nicardipine and flunarizine. Thus, these results suggest that NB-818 may be useful in the treatment of ischemic cerebral damage.

Stimulation of rat liver growth by a 1,3-dithiole derivative, KZ-1026.

The oral administration of 2-(1,3-dithiol-2-ylidene)-1-phenyl-1,3-butanedione (KZ-1026) to normal rats at doses of 50, 100 and 200 mg/kg/day for 3 days accelerated liver enlargement in association with a dose-dependent increase in the total amounts of protein, RNA and DNA in the liver. The liver weight at 24 hr after the third dose of 200 mg/kg reached 174% of the control. With respect to the effect on liver enlargement, KZ-1026 differed from phenobarbital, since KZ-1026, unlike phenobarbital, increased hepatic DNA content without significant effects on P-450 and aminopyrine-N-demethylase. Incorporation of [3H]thymidine into liver DNA was stimulated by a single dose of KZ-1026 (200 mg/kg), and it peaked at 24 hr post dose (18 times the control), followed by an increase in the number of liver nuclei. Liver growth was also accompanied by an increasing hepatic reserve cell mass, expressed by the capacity of eliminating exogenous galactose from the blood stream. Pretreatment with KZ-1026 (200 mg/kg/day) for 3 days significantly improved the survival rate of subtotally hepatectomized rats from 39% to 78%. These findings indicate that KZ-1026 accelerates hepatocyte proliferation, resulting in an enhancement of liver functional mass in normal rats.

Application of Doppler color flow imaging method on the detection of vesicoureteral reflux.

The doppler color flow imaging method was employed to detect the vesicoureteral reflux. The ultrasound examination was performed by using the doppler color flow imaging system. Ultrasonic sector scan was carried out by the use of the 3.75 MHz phased array and/or conbex array transducer with a transabdominal approach. The flow in the bladder from the ureteral orifice to the bladder was detected with a red color indicating the direction towards the probe and the flow from the bladder towards the ureteral orifice during the vesicoureteral reflux was detected with a blue color indicating the direction away from the probe. The doppler color flow imaging method appears to be a useful diagnostic procedure for the detection of the vesicoureteral reflux.

[Nervous control of micturition].

The normal micturition depends on a highly elaborate control system that comprises central and peripheral nerves. The recent research on the nervous control of micturition has been markedly progressing. This paper reviews the latest understanding of 1) the functional anatomy of lower urinary tract, 2) the peripheral nervous system and nuclei in the spinal cord, 3) the neurotransmitters and receptors, 4) nervous reflex mechanism of micturition, 5) pontine micturition center, 6) pontine urine storage center and 7) neural connections between pons, higher centers and lower centers.

[Study of the clinical background and therapeutic method in patients with vulvar cancer].

Forty-two cases of vulvar cancer were reviewed with regard to clinical condition and therapeutic method, especially the efficacy of local administration of BRM. By histological examination, koilocytosis was found in 45%. Furthermore, there was 90% positive koilocytosis rate in immunohistological study using anti-human papilloma virus antibody and in situ hybridization method by HPV DNA probe (type 6, 11, 16, 18). The HPA was mostly type 16. Therefore, vulvar cancer was considered to occur in close relation with HPV type 16. Some 12% of vulvar cancer was complicated with other gynecologic cancer. In these cases, NK activity was low. Thus, cancer may be considered to occur in association with low immunological status. With regard to the therapeutic method, surgery and radiotherapy were usually used after chemotherapy in our institute. At stage 3-4, combination therapy using surgery and radiotherapy were found to be most effective. In regard to local immunotherapy using OK-432 and LAK cell and IL-2, local administration of OK-432 was effective. Therefore, surgery and radiotherapy with local immunotherapy after chemotherapy were considered to be the most effective therapeutic method for vulvar cancer.
