Simultaneous 99mTc-P-butyl-IDA and 131I-rose bengal scintigraphy in neonatal jaundice.

Eight neonates with jaundice were studied simultaneously with 99mTc-p-butyl-IDA and 131I-rose bengal. Due to physical decay, 99mTc-p-butyl-IDA failed to demonstrate delayed excretion through the patent extrahepatic biliary tract in 3 of 5 patients with concomitant hepatitis; 131I-rose bengal showed small-bowel activity in all 5. Neither agent demonstrated small-bowel activity in 3 neonates with extrahepatic biliary atresia. Based on this clinical trial, 131I-rose bengal remains the radiopharmaceutical of choice for distinguishing between hepatitis and biliary atresia in these patients.

Changing indications for bone scintigraphy in patients with osteosarcoma.

Fifty-six patients with osteosarcoma were studied to determine the onset of pulmonary and bone metastases. While pulmonary metastases were always detected prior to bone metastases in the era before adjuvant chemotherapy, in this study of patients on adjuvant therapy 16% of patients with metastases showed osseous metastases prior to or without pulmonary metastases.

Lung growth and airway function after lobectomy in infancy for congenital lobar emphysema.

To characterize the outcome of lobectomy in infancy and the low expiratory flows which persist after lobectomy for congenital lobar emphysema, 15 subjects with this history were studied at age 8-30 yr. Total lung capacity was normal in all, but higher values (P < 0.05) were observed in nine subjects with upper lobectomy than in five subjects with right middle lobectomy. Ratio of residual volume to total lung capacity was correlated (P < 0.05) with the amount of lung missing as estimated from normal relative weights of the respective lobes. Xe(133) radiospirometry in eight subjects showed that the operated and unoperated sides had nearly equal volumes at total lung capacity, but that the operated side was larger than the unoperated side at residual volume. Perfusion was equally distributed between the two sides. Similar findings were detected radiographically in four other subjects. Forced expiratory volume in 1 s and maximal midexpiratory flow rate averaged 72 and 45% of predicted, respectively. Low values of specific airway conductance and normal density dependence of maximal flows in 12 subjects suggested that obstruction was not limited to peripheral airways. Pathologic observations at the time of surgery and morphometry of the resected lobes were not correlated with any test of pulmonary function. These data show that lung volume can be completely recovered after lobectomy for congenital lobar emphysema in infancy. The volume increase occurs on the operated side, and probably represents tissue growth rather than simple distension. The response to resection is influenced by the particular lobe resected and may be associated with decreased lung recoil near residual volume. Low expiratory flows in these subjects could be explained by several mechanisms, among which a disproportion between airway and parenchymal growth in infancy (dysanaptic growth) is most compatible with our data.

Iridium- 191 angiocardiography for the detection and quantitation of left-to-right shunting.

An osmium- 191 leads to iridium- 191 generator that can deliver multiple doses of Ir- 191m for first-pass radionuclide angiography has been developed. Iridium- 191m has a physical half-life of 4.96 sec and decays with emission of 65-keV and 129-keV photons in 58 and 30% abundance, respectively. Using a gamma camera, Ir- 191m radionuclide angiography was carried out, in dogs and ten patients, for the detection and quantitation of left-to-right shunting. In a one-year-old patients, 25 mCi of Ir- 191m results in a whole-body radiation absorbed dose of 35 mrad. Multiple Ir- 191m angiograms can be performed, seconds to minutes apart, without interference from background. The 15.4-day half-life of Os- 191 permits transportation of the generator to centers far from the production facility. With the low radiation dose, high information density, and the ability to repeat studies with Ir- 191m, clinical use of radionuclide angiography should be expanded.

Is there still a place for bone scanning in Ewing's sarcoma? Concise communication.

In this retrospective study, 28 cases of Ewing's sarcoma are reviewed for the onset of metastasis. Bone scans demonstrated bone metastasis in three out of 28 patients at presentation. Of the 22 patients free of metastases at presentation, ten subsequently developed bone metastases. In six of these patients, the bone scan was the earliest demonstrator of metastatic disease. Bone scans are recommended at presentation and periodically during follow-up.

A new osmium-191 leads to iridium-191m generator.

A new osmium-191 leads to iridium-191m generator suitable for first-pass radionuclide angiocardiography has been developed. This generator system allows repeated elutions of 4.96-sec iridium-191m from its 15.4-day Os- 191 parent. The Os-191 is loaded on an anion-exchange column (AGMP-1) and Ir- 191m eluted with 0.9% NaCl at pH 1. Each elution (0.6 to 1 ml) of the generator yields about 7 to 10% Ir-191m and gives 0.003-0.008% of Os-191 breakthrough. Toxicity studies of the generator eluate carried out in animals support the safety of using iridium-191m in humans. The long shelf-life of the generator (approximately equal to 2 wk) will allow medical centers to use Ir- 191m for routine clinical diagnosis. Iridium-191m obtained by this method should find additional useful applications in nuclear medicine.

Skeletal scintigraphy in the diagnosis of acute osteomyelitis.

Bone scans detect osteomyelitis earlier, sometimes several days earlier, than x-rays. Prompt institution of therapy, before significant bone damage has developed, thus becomes possible. A positive scintigram is, however, nonspecific, and firm diagnosis requires radiologic and other confirmation. Moreover, because the technique is so new, many open questions remain both about its optimal utilization and about its potential.

Thallium-201: quantitation of right ventricular hypertrophy in chronically hypoxic rats.

Sprague Dawley rats were divided into two groups. Ten were kept in room air and 10 in hypobaric hypoxia (air at 380 mm Hg). After two weeks all were injected intravenously with 50 muCi of 201Tl and sacrificed. The right and left ventricles were separated, weighed, and measured for radioactivity in a gamma well counter. Left and right ventricular mass ratios (MR) correlated with 201Tl radioactivity ratios (TAR) in both control and hypoxic rats: r = 0.962 where MR = 0.863 TAR + 0.27. Myocardial 201Tl uptake reflects and quantitates normal and abnormal ventricular mass, the abnormal mass in this model consisting of right ventricular hypertrophy associated with hypoxic pulmonary hypertension.

Krypton-81m: a better radiopharmaceutical for assessment of regional lung function in children.

Krypton-81m, an inert, radioactive gas with a physical half-life of 13 seconds was used to evaluate regional lung ventilation in 25 pediatric patients. The results suggest that Kr-81m has several advantages over previous methods employing xenon-133. These include: more favorable physical characteristics for imaging with present scintillation cameras; ease of performance of the examinations; improved resolution and multiple views with better detection and localization of abnormalities; and a much lower dose to the patient.

An improved radionuclide method for the diagnosis of gastroesophageal reflux and aspiration in children (milk scan).

A radionuclide study using technetium-99m-labelled milk feeding is described for the detection of gastroesophageal reflux and aspiration in children. A comparison of findings in 39 patients referred for both radiographic and radionuclide studies showed that barium studies were positive in 25.6% and radionuclide studies in 59% of cases, reflecting the sensitivity of the radionuclide technique. This technique is also physiological and allows prolonged patient monitoring. It is simple to perform and should prove useful in the evaluation of patients suspected of having gastroesophageal reflux and aspiration.

Cardiac imaging with tantalum-178.

Tantalum-178 is a potential tracer for the first transit studies of the heart. It has a short physical half-life (9.3 minutes) and is obtained as a generator product from its long-lived (21.3 days) parent, tungsten-178. First transit studies were performed in the monkey and dog following the intravenous injection of 20 mCi (740 MBq) of tantalum-178. Imaging was performed with a large field of view single crystal camera and a multicrystal camera. Pulmonary dilution curves were derived for the analysis of left-to-right intracardiac shunting and high temporal resolution studies were obtained to assess left ventricular function and to measure left ventricular ejection fraction. The short physical half-life of tantalum-178 permits fraction. The short physical half-life of tantalum-178 permits multiple sequential studies with reduced patient radiation dose compared to the current radionuclide of choice, technetium-99m.

Computed tomography of the heart using thallium-201 in children.

Thallium-201 emission computed tomography (ECT) was performed in 3 pediatric patients in whom conventional scintigraphy was normal but there was a strong clinical suspicion of myocardial disease. Abnormalities in the distribution of myocardial perfusion appeared sharply. Single photon ECT provides a three dimensional reconstruction which results in greater enhancement since activity in overlying structures does not interfere. Its widespread use is limited only by the cost of the imaging device.

Adrenal hemorrhage in the newborn: scintigraphic diagnosis.

Four neonates with abdominal masses following adrenal hemorrhages were investigated by renal scintigraphy. Characteristic photon-deficient lesions were noted in the suprarenal areas during the phase of whole-body radionuclide distribution.

Wandering spleen in three children.

Three children with wandering spleen are presented. Liver-spleen technetium99m sulphur colloid scintigraphy is a helpful diagnostic tool that will demonstrate the abnormally located spleen or absent splenic activity when the blood supply is interrupted by a twisted pedicle. The diagnosis can also be made by angiography, but this relatively elaborate procedure is usually not necessary. Absence of the normal ligamentous attachments of the spleen associated with a long splenic pedicle may well be the cause of the wandering spleen and its torsion.

Bone scintigraphy of osteoid osteoma: a clinical review.

Osteoid osteoma is a painful osteoblastic process. The pre-operative diagnosis is usually made by roentgenogram. Four surgically confirmed cases are examined where the preoperative diagnosis was clarified by bone scintigraphy. The clinical indications, the diseases process, the literature are reviewed.
