Patterns of intestinal motility recovery during the early stage following abdominal surgery: clinical and manometric study.

The purpose of this study was to evaluate the pattern of recovery of intestinal motility using manometric and clinical assessment in the postoperative ileus following abdominal surgery. We reviewed the charts of 18 patients who underwent partial colectomy for colon cancer (group A, without vagotomy) and compared them to those of 15 patients who underwent gastrectomy with truncal vagotomy and reconstruction by the Billroth I technique (group B, with vagotomy). A three-lumen catheter was inserted via the nose into the proximal jejunum for drainage of intestinal fluids and to record intestinal motility at laparotomy. The first appearance of a burst activity following a silent period and a migrating motor complex (MMC) was at 3.8 hours (mean) and 16.4 hours, respectively, in group A, and at 2.5 and 14.3 hours, respectively, in group B. The first audible bowel sounds and the time of feeling abdominal fullness and passage of flatus was at 34.8, 47.5, and 62.9 hours (group A), and at 43.0, 53.8, and 77.9 hours (group B), respectively. The peak volume of nasointestinal drainage was recorded at 16 to 24 hours in group A and 40 to 48 hours in group B (p < 0.05). As measured by manometric examination, an MMC reappeared within a few hours after surgery. Physiologic postoperative ileus was ended when MMCs extended throughout the gastrointestinal (GI) tract with forwarding GI fluids.

[Effect of percutaneous transluminal mitral commissurotomy on pulmonary function and on the pulmonary vasculature].

We evaluated the therapeutic effect of percutaneous transluminal mitral commissurotomy (PTMC) on pulmonary function, and the damage caused to pulmonary vasculature by transient occlusion of the mitral valve during PTMC in patients with mild mitral stenosis. Pulmonary function tests were done and serum thrombomodulin was measured in 6 patients before and after PTMC. The mean pulmonary arterial pressure and mean pulmonary arterial wedge pressure decreased significantly from the control values, and the decreases were proportional to the increase in the area of the mitral valve. VC, %VC, and DLco were in the normal range before and after PTMC but PEFR, FEV1%, FEV1, V50, V25, and V50/V25 were significantly higher after PTMC. Change in the area of the mitral valve correlated with the changes in FEV1% (r = 0.841), in V50 (r = 0.624), and in V25 (r = 0.697). The serum thrombomodulin levels before and after PTMC did not differ. We conclude that pulmonary dysfunction in patients with mild mitral stenosis MS is mainly due to an obstructive ventilatory defect, and that PTMC can correct this dysfunction without damaging the pulmonary vasculature.

Optimal configuration of pyrene excimer induced by the excimer-forming two-probe nucleic acid hybridization method.

Excimer-forming two-probe nucleic acid hybridization (ETPH) method with pyrene as a fluorophore enables homogeneous hybridization assays. We examined the effect of linker length between a pyrene residue and a terminal sugar moiety on Tm of hybrids in the presence of 20% dimethylformamide (DMF). The results including those of CD measurements indicated no interaction of pyrene residues with the duplex formed between a target 32-mer and a pyrenemethyliodoacetamide-introduced 16-mer probe (PMIA-P5)/a pyrenebutanoic acid-introduced 16-mer probe (PBuA-P3), which is the best pair of probes for intense excimer emission. This was also supported by a computer-assisted molecular modeling using Insight II and Discover software.

Tree and loop as moments for measurement.

Biologically motivated computing presents us with a measurement process in science. It triggers an epistemological shift from state-oriented physics to measurement-oriented physics, in which we can find a parallelism with Wittgenstein's shift from rule following to a language game. We argue here that an approximation or computing process can be viewed as a language game and propose an idea of proto-computing which is metaphorically formalized through disequilibration between tree- and loop-program, as a model for measurement-oriented computing.

[The relation of dental examination results at corporation offices to occupation, subjective symptoms of fatigue and daily lifestyle--an investigation based on the number of missing teeth].

Our investigation was intended to find a relationship between the dental health of labourers and, their jobs, subjective symptoms of fatigue and daily lifestyle. Dentists checked the dental health of 2,565 corporation employees (790 male and 775 female) of small to medium-sized enterprises in Hiroshima prefecture. At the same time they examined the employees' occupation, subjective symptoms of fatigue and their health maintenance behavior according to Breslow's criteria. The data were divided into age groups spanning 10 years. Among males in their 30s, those engaged in transport/driving and engineering/production had more missing teeth than others. We found a relationship between the number of missing teeth and subjective symptoms of fatigue among men in their 30s. This suggests that the greater the number of missing teeth, the greater their subjective symptoms of fatigue. The results suggest that missing teeth cause malocclusion which leads to subjective symptoms of fatigue.

Carcinoma of the cystic duct protruding into the common bile duct: report of a case.

We report herein the case of 65-year-old man in whom a diagnosis of primary carcinoma of the cystic duct was made on the basis of Farrar's criteria. The patient was admitted with upper abdominal pain, and although there was no evidence of jaundice or a palpable mass, there was tenderness in his right upper quadrant. Carcinoma of the cystic duct was suspected on the basis of computed tomography and magnetic resonance imaging findings. Cholecystectomy with resection of the bile duct and lymph node resection was performed, and percutaneous transhepatic cholangiography revealed a filling defect in the common bile duct (CBD). The tumor was found to have arisen from the cystic duct and demonstrated papillary growth into the CBD intraluminally through the orifice of the cystic duct. Microscopically, the tumor was identified as papillary adenocarcinoma with invasion limited to the subserosal layer of the cystic duct. There were no signs of nodal metastasis.

Enhancement of antigen-specific antibody production by extracellular slime products from slime-forming Lactococcus lactis subspecies cremoris SBT 0495 in mice.

The effect of extracellular slime products (ESP) produced by Lactococcus lactis subspecies cremoris SBT 0495 on antigen specific antibody production was studied in mice. ESP contained 48.5% protein, 15.4% neutral sugar, and 1.1% of phosphorus. The optimum dose of ESP was between 100 to 500 micrograms per mouse. ESP administered intraperitoneally (200 micrograms per mouse) enhanced the production of specific antibody in mice. These results indicate that ESP may act as an adjuvant.

In vitro selection of RNA aptamers that can bind specifically to Tat protein of HIV-1.

The Tat protein of human immunodeficiency virus interacts specifically with its target RNA sequence, TAR, and activates viral gene expression at the early stage of infection. Here, we have used in vitro genetic selection strategy to determine what sequence or structural motifs might exist between RNA's that interact specifically with the Tat protein. Starting from a RNA pool that has a 120 base random sequence core, aptamers that bind specifically to the Tat protein were selected by repeating 11 rounds of selection and amplification. A comparative analysis of 64 aptamers that were isolated from the 11th generation revealed two main sequence classes. Interestingly, one of these two classes of aptamers had minimum of one U residues in bulge loop and 2 specific adjacent base pairs. This region is very much homologous to the core sequence of TAR RNA that is essential for the specific Tat-TAR interactions. Further analyses of the sequences from the 11th generation should reveal what kind of RNA structures are required in order to show a high affinity for the Tat protein.

A case of cytomegalovirus mononucleosis associated with pleural effusion.

A 5 year old boy had a spiky fever accompanied by a mild pharyngitis, cervical lymphadenopathy and hepatosplenomegaly. Laboratory findings revealed leukocytosis with 26% atypical lymphocytes, and liver dysfunction. A chest X-ray showed pneumonia and a considerable amount of pleural effusion. Serum antibody titers for cytomegalovirus (CMV) were elevated significantly and CMV-DNA (polymerase chain reaction) was detected in the pleural effusion. Only 13 cases of pleural effusion associated with infectious mononucleosis have been reported previously in the literature, but there was no documentation that proved CMV infection. The case reported here suggests that the pleural effusion was caused by the infiltration of mononuclear cells to the pleura as a result of systemic inflammation, and the possible alternative of host immune response against CMV was related to recent Varicella zoster virus infection.

[A case of lung cancer avoidable right pneumonectomy due to external and intraluminal irradiation therapy].

A 54-year-old man with squamous cell carcinoma of the lung resected after external and high dose rate intraluminal irradiation (HDRII) therapy is reported. Cancer existed in right lower bronchus, extended to bifurcation of upper broncus and truncus intermedius. The patient received external irradiation of 40 Gy and HDRII of 18 Gy (Iridium192 was used) simultaneously. Effectiveness of irradiation was remarkable, tumor disappeared in chest CT and bronchoscopy. Middle and lower lobectomy and bronchial wedge resection was performed. Histologically, cancer was not existed in second carinal portion, so curative resection was done. The result suggests that HDRII is effective therapy as preoperative adjuvant irradiation.

Effect of dental metals on magnetic resonance imaging (MRI).

The aim of this research was to examine the effects of metals used in dentistry upon magnetic resonance imaging (MRI). Ten disk-shaped samples of metals have been used in this study. Each sample has been set into a volunteer who had produced a normal MR image when tested. The subject was imaged in the sagittal, frontal and coronal plane with a 0.1 T MRI device and results examined for MR defects. Au, Ag, Au-Ag-Pd and amalgam produced no defect. Ni-Cr, Co-Cr and SUS304 expressed small amounts of MR defects, but SUS405, Pd-Co-Ni and Sm-Co expressed large defects. Those metals which create MR image degradation must not be used for fixed prosthesis or orthodontic devices both of which could easily be made from other metals. If the MR image degrading metals are to be used in dentistry, their application should be restricted to removable prosthesis.

[A study on the detection of Helicobacter pylori in children with gastric and duodenal diseases].

Infection of Hellcobacter pylori (H. pylori) in the gastric mucosa is said to participate in the pathogenesis of gastric and duodenal ulcers as well as gastritis. The detection rate of H. pylori in tissue specimens endoscopically taken from adult patients with these diseases was 75.2% (408/542). In affected children (2-15 yrs), on the other hand, 27 biopsies were performed in 22 cases but the initial positive rats was as low as 45.5%. According to the diagnosis, it was 75% for acute gastric mucosal lesion (AGML), 50% for duodenal ulcer, 33% for duodenitis, and 38% in seemingly normal children. When changes in the detection rate of H. pylori was examined after administration of H2 blocking antiulcer agents, it was found to very case by case, e.g. changed in individual disease stages or completely or reoccurrence disappeared.

Removable magnetic dental attachment that permits magnetic resonance imaging.

Magnetic dental attachments cause magnetic resonance image (MRI) degradation. A new magnetic dental attachment that allows removal of magnetic parts to exchange them for nonmagnetic parts was developed and tested. It allows high quality MRI with no change in occlusal vertical dimension. A volunteer test subject with combinations of attachments in place was examined by MRI. The images produced showed no degradation or distortion. Magnetic attachments for overdentures or maxillofacial prostheses should be removable to permit use of MRI.
