Necrobiotic xanthogranuloma in a human T-lymphotropic virus type 1 carrier.

A 67-year-old man who had necrobiotic xanthogranuloma associated with paraproteinemia is described. He was a human T-lymphotropic virus type 1 (HTLV-1) carrier who had a high titer of circulating anti-HTLV-1 antibodies and neurologic abnormalities that suggested HTLV-1-associated myelopathy. The patient's necrobiotic xanthogranuloma and neurologic symptoms improved after he received four 5-day courses of melphalan, 2 mg/day, and prednisolone, 20 mg/day, at 4-week intervals.

[Dermatological findings in the annual examination of the patients with Yusho in 1989-1990].

We reported the skin symptoms of the patients with Yusho in the 1989 and 1990 annual examinations. We also studied the alteration of the skin symptoms in 33 patients followed up for 14 years (1977-1990). Most of these patients showed obvious improvement in their skin symptoms during 12 years (1977-1988). However, a remarkable improvement was not observed in recent 3 years and approximately 40% of the patients still had comedones and/or acneiform eruptions.

A new apparatus for hair regrowth in male-pattern baldness.

A newly devised apparatus (Scalp-Tension-Relaxer, STR) can efficiently promote hair regrowth in patients with male-pattern baldness. When this apparatus is applied, the scalp is pushed up to relieve tension on the vertex. The efficacy rate of hair regrowth in alopecia patients was 40%. An investigation into the basis for the hair regrowth caused by this apparatus was directed toward the changes in hemodynamics and skin temperature of the scalp. During and after use of this apparatus, subjects exhibited an increase both in the cutaneous blood flow rate (as determined by laser Doppler flowmetry) and in the cutaneous temperature (as determined by thermography).

Malignant hidroacanthoma simplex.

A case of malignant hidroacanthoma simplex is described. Histological examination revealed that the malignant cells had arisen from a tumor cell nest of benign hidroacanthoma simplex and invaded the dermis. Two years and 7 months after the excision of the original thigh lesion, a groin lymph node metastasis was discovered. The metastatic tumor cells in the lymph node were histologically similar to the carcinoma cells in the original thigh lesion and had enzymatic and ultrastructural features resembling eccrine sweat duct.

[Usefulness of bone imaging in diagnosis of sternocostoclavicular hyperostosis].

Sternocostoclavicular hyperostosis (SCCH) is a rare disease that is characterized by hyperostosis and soft tissue ossification between the clavicles and the anterior part of the upper ribs. There have been few published reports of 99mTc-MDP bone imaging findings in this disease. Thirty-three patients who were suspected to have SCCH were studied with 99mTc-MDP bone imaging. Bone images were abnormal in 31 (94%) of 33 patients. Chest radiographs were available in 30 patients, but negative in 14 (46%) of them. Bone imaging revealed also radionuclide accumulations in other bones such as the vertebrae, femur or sacroiliac joints in ten (14 sites) of 33 cases. Bone scintigraphy was useful in the diagnosis of sternoclavicular hyperostosis.

[Dermatological findings in the annual examination of the patients with yusho in 1987-1988--with special reference to the alteration of the skin symptoms in the patients followed up for 20 years].

We reported on the skin symptoms of the patients with yusho in the 1987 and 1988 annual examinations. We also studied the alteration of the skin symptoms in the patients followed up for 20 years. These patients showed obvious improvement in their skin symptoms during these 20 years. However, about a half of the patients still had comedones and/or acneiform eruptions, although the number of eruptions had decreased. The cases with blood PCB pattern A (typical yusho pattern) demonstrated higher skin severity grading points than those of the other groups (patterns B and C), but even in these cases the skin eruptions had improved.

[A case of neurilemmomatosis].

A 25-year-old man had multiple subcutaneous nodules on the neck, trunk and extremities. Those nodules first appeared in the right popliteal fossa about 15 years ago and thereafter gradually increased in the number with hearing loss, tinnitus, and vertigo. The tumors had smooth surfaces and were elastic hard without any adhesion to the overlying skin or to subcutaneous tissue. Histological examination revealed, in most parts of the lesion, Antoni type A neurilemmoma and the Antoni type B picture at the periphery. As extracutaneous lesion, there were bilateral cerebello-pontine angle neurilemmomas, and spinal and adrenal tumors. Similar subcutaneous tumors have been recognized in 6 of 17 family members of the previous four generations of his family. According to a statistical analysis of 119 cases, including this one, in the Japanese dermatological field, of 75 cases of solitary neurilemmoma and 44 cases of multiple ones, the latter showed male predominance and lower age of the onset. Many of them had tumors not only in the skin but also in extracutaneous sites.
