[Intra-aortic prostaglandin E1 infusion in the treatment of advanced neuroblastoma].

Neuroblastoma, one of the commonest malignant childhood tumors, arises from the adrenal glands and/or sympathetic ganglia, and metastases into lymph nodes, bone marrow, bones, etc. Patients with advanced stages of neuroblastoma are poorly responsive to current combined chemo-radiotherapy. However, neuroblastoma is known to maturate into benign ganglioneuroma spontaneously or induced by certain chemicals. In our previous studies, adenosine 3', 5'-monophosphate (cAMP) was found to play an important role in this maturational process. It was also demonstrated that long-term continuous infusion of dibutyryl cAMP and papaverine caused significant growth-inhibition and tumor differentiation of C 1300 neuroblastoma in mice. These results prompted us to apply intra-aortic PGE1 infusion to two cases of abdominal neuroblastomas with stages III and IV.PGE1 infusion (0.25-3.0 ng/kg/min, over the period of 3 to 32 weeks), combined with oral papaverine and conventional chemotherapy, was effective in decreasing tumor size and promoting tumor maturation in the infused area. However, distant osseous metastases occurred in both cases, during and after the PGE1 administration. Similar modalities should be further evaluated in order to potentiate anti-metastatic effect on neuroblastoma cells.

Mass screening of neuroblastoma in infancy.

A vanillylmandelic acid spot test for screening infants for neuroblastoma was developed in cooperation with 11 health centers in Kyoto, Japan. In this system, the parent sends a filter paper spotted with the infant's urine at 6 or 7 months of age. From 78,331 infants screened during the six years from July 1973 to September 1979, four asymptomatic infants with neuroblastoma were discovered. Three of them have already completed their treatments and have recovered. The cost of screening 13,000 infants per year was $7,500. One false-negative test was encountered, as were a number of false-positive tests.

Isolation and primary culture of adult human hepatocytes. Ultrastructural and functional studies.

Biopsy tissue of adult human liver was gently dissociated with collagenase followed by Dispase. By repeated low g centrifugation, a large number of almost pure, viable hepatocytes was obtained. This is the first report of a successful procedure for obtaining adult human hepatocytes for study in tissue culture. The isolated cells have the typical morphology of liver parenchyma, and these characteristics persist throughout the period of culturing. Evidence of their function is indicated by albumin synthesis. This procedure is now being used to study human hepatocyte functions in vitro and the effects of a variety of agents including carcinogens and viruses.

Simple method for the collection of pancreatic islets by the use of Ficoll-Conray gradient.

Simple method for the collection of large numbers of viable pancreatic islets from normal rats is described. This method involves collagenase digestion of the pancreas, followed by the use of Ficoll-Conray discontinuous gradient for the separation of isolated islets from unwanted acinal debris. Ficoll-Conray A solution was prepared by mixing undialyzed 12.5% Ficoll and 33.4% Conray in the ratio 2 : 1, to make a specific gravity of 1.095 and osmolarity of 396 mOsm/l. The solution B, C, and D, with specific gravities 1.084, 1.072, and 1.048, respectively were obtained by diluting A solution with distilled water. Using Ficoll-Conray gradient, about 200 viable islets which maintained excellently their morphology and function, were collected consistently from a young adult rat pancreas.

Morphological and functional changes in the tight junctions of the bile canaliculi induced by bile duct ligation.

Thin sections after bile duct ligation showed that the depth of tight junctions appeared to increase and that the distance between individual punctate contacts appeared to become irregular and wider than in controls. The freeze fracture replicas clearly demonstrated these changes in the tight junction morphology. Changes were noted most conspicuously in the tight junction three weeks after ligation. Measurements of the junctional morphology in control and ligated specimens showed that the junctional depth had increased two fold in the latter, whereas the number of strands had scarcely changed. Lanthanum tracer experiments showed that the tight junctions did not permit the passage of the tracer in normal nor ligated rats. It was concluded that the mechanism of obstructive jaundice could not be related to changes in junctional morphology causing increased junctional permeability.

Electron microscopic observations on the cholesterol distributed in the epithelial cells of the gallbladder.

Epithelial cells of eight human gallbladders with cholesterosis were examined. In the supranuclear portion of the epithelial cells of one case, many spicular, circular and plate-like crystalline structures are present. Spicular and circular structures have not a limiting membrane, but plate-like structures are apparently bounded by a limiting membrane that clearly shows trilamellar structure. After digitonin treatment, the spicular and circular crystalline structures become denser. On the other hand, the plate-like structures do not become denser by digitonization. In the epithelial cells that contain no crystalline structure, there also occur many reaction precipitates after digitonization. These findings may suggest that free cholesterol is highly present in the epithelial cells of gallbladder with cholesterosis and that it, in some case, precipitates in the form of spicular or circular structure for the rapid fixation process.
