[Modifications in alpha 2 globulins, gamma globulins and in rheumatoid factor during gold salt therapy in rheumatoid arthritis].

A total dose of g 1.071, given as hydrosoluble salts for a 12 month period, showed a significant decrease in serum gamma globulins along with clinical improvement in 17 patients affected with rheumatoid arthritis. A decrease in alpha 2 globulins and in rheumatoid factor titre was observed too, but it was not significant. The data suggest that in rheumatoid arthritis the gold therapy might also be effective on the immunological disease mechanism.

Fructose-induced hyperuricemia in essential hypertension.

A rapid intravenous fructose load was given to nine normouricemic essential hypertensive and eight control subjects. The following increase in plasma uric acid concentration was significantly higher in hypertensives than in controls. There was no significant difference in urinary excretion of urate between the two groups. Since the increase in uric acid concentration brought about by fructose is most probably due to an increased metabolism of preformed purine nucleotides, it is suggested that essential hypertensive patients have a higher than normal "pool" of purine nucleotides.

Electrocardiogram, arterial and central venous pressure during laparoscopy under local anaesthesia.

Cardiovascular hazards of laparoscopy performed under local anaesthesia and with room air pneumoperitoneum are not well known. Therefore we have recorded electrocardiogram, arterial blood pressure and central venous pressure in 63 consecutive liver patients undergoing this procedure. Electrocardiographic changes were found in 34 cases, and consisted in transistory tachycardia and bradycardia, ectopic supraventricular and ventricular beats, ST segment depression and flattening of T wave. Blood pressure did not change significantly, but five patients had transitory hypotension during the procedure. Central venous pressure did not vary immediately after inflation, but a significant increase was found during the performance of laparoscopy and it was still observed after deflation. Our findings show that cardiovascular changes during laparoscopy under local anaesthesia are minimal, and that they are probably due to neurogenic factors.

[Clinical effects of a clonidine-chlorthalidone combination in the treatment of arterial hypertension].

Clonidine (0.075-0.225 mg/day) and chlorthalidone (0.15-0.45 mg/day) were associated in the management of 23 cases of arterial hypertension of varying nature and gravity. A significant and satisfactory decrease in pressure was accompanied by reduced cardiac frequency. Diuresis, body weight and the main biohumoral indices were not significantly affected. The mechanism of action of the two drugs is explained and the reason why success was obtained with their combination in less-than-normal doses is discussed.

A possible role of the kidney in activating a renin preinhibitor.

In order to verify the possibility than human plasma and kidney can activate a renin preinhibitor (Phospholipid) into inhibitor (lysophospholipid), constant quantities of preinhibitor were added to plasma and kidney homogenate. Addition of preinhibitor to plasma did not modify the quantity of Angiotensin I that developed. On the other hand, addition of preinhibitor to crude kidney homogenate, followed by incubation with human angiotensinogen, caused a significant fall in the quantity of Angiotensin I generated. While plasma is deficient in the specific enzyme delegated to the transformation of preinhibitor into inhibitor, it appears that this enzyme is present in the kidney.

Effect of uninephrectomy on tissue kallicrein concentration of the remaining kidney.

Compensatory renal hypertrophy has been induced in rats by uninephrectomy. Tissue kallicrein concentration of the remaining kidney was evaluated 5, 10, 15 and 20 days after operation. The previously removed kidneys were used as controls. In the hypertrophic kidney the kallicrein increases progressively up to 20 days after uninephrectomy. Since kallicrein generates kinins, potent vasodilators, it is suggested that the increase in renal blood flow observed after uninephrectomy might be promoted by an overproduction of kinins.

Renal kallikrein content of spontaneously hypertensive rats.

1. The kallikrein content of kidneys from spontaneously hypertensive and normal rats at birth and at age 37 days was determined. 2. Kallikrein values were significantly lower in the hypertensive rats. 3. It is suggested that the lowered kallikrein may be related to the development of hypertension.

Primary aldosteronism due to a malignant ovarian tumor.

A case of a young woman with the syndrome of primary aldosteronism and malignant ovarian tumor is reported. Hormone studies revealed extremely high urinary aldosterone, undetectable plasma renin activity, elevated plasma 17beta estradiol and testosterone, and low plasma FSH and LH. Plasma cortisol and urinary 17-hydroxycorticoids were at the upper normal limits. Autopsy disclosed an ovarian tumor, histologically an arrhenoblastoma, with polymorphic aspects. The adrenal glands grossly were normal. Aldosterone was found by the double radioisotopic technique in the neoplastic tissue.
