An unusual case of self-induced electrolyte depletion.

A case of anorexia nervosa, presenting with unexplained hypokalaemia, is described. The patient was also secretly addicted to purgatives and diuretics. During an attempted metabolic balance study she secretly disposed of food and excreta, which were smuggled from the hospital by her sister. The patient induced her husband to bring his own stools into the ward, these then being substituted for her own. The interrelationships of the electrolyte disturbances, elevation of plasma renin, renin substrate, and hyperaldosteronism are discussed, particularly in connexion with the pathogenesis of peripheral oedema in anorexia nervosa.

Symptoms of irritable bowel in school children: prevalence and psychosocial effects.

INTRODUCTION: This study was to ascertain the prevalence of symptoms of irritable bowel in a sample of school children 11 to 17 years of age and to determine whether children with such symptoms differed from children without symptoms on various psychosocial questions. METHOD: Eight hundred fifty-one school children completed a 20-item questionnaire consisting of questions related to symptoms of irritable bowel, psychosocial questions, and filler questions. Questions referred to a 1-week time frame to minimize memory distortion. RESULTS: Of the sample, 133 (16.2%) had one or more symptoms of irritable bowel. A significant association was found between bowel symptoms and other factors. Children with symptoms were more likely to report that they felt different from other children (chi 2 analysis = 10.06, p = 0.002), were embarrassed about their health (chi 2 analysis = 16.14, p = 0.0001), and felt their health prevented them from going out with their friends (chi 2 analysis = 4.69 p = 0.03). DISCUSSION: The study suggests that a large number of children have symptoms of irritable bowel and that having such symptoms can have a significant impact on children's lives beyond the immediate physical effects. Because children with symptoms of irritable bowel are likely to come to the attention of health professionals, nurses are in an ideal position to give such children both information and reassurance about their condition.

Subarachnoid hemorrhage: epidemiology, diagnosis, management, and outcome.

A population-based study of primary subarachnoid hemorrhage in Auckland (population 829,454), New Zealand, identified 180 cases in a two-year period. This represented an age adjusted incidence rate of 10.5 and 18.3 per 100,000 for men and women respectively. Sixty-eight percent of all cases had a proven intracranial aneurysm or arteriovenous malformation, 15% had negative angiographic findings and in the remaining 17%, the presence or absence of a localized lesion was unknown since neither angiography nor autopsy were performed. Twenty-six patients (15%) died before hospitalization and a further 36 patients (20%) died within 48 hours of onset. Only 94 patients (53% of all patients registered) were fit enough to undergo angiography. A surgical operation was carried out on 60 of the 68 patients in whom an aneurysm was confirmed at angiography. The overall case fatality rate was 36% within the first 48 hours, 43% in the first week and 57% at both six months and one year. The high early case fatality rates are similar to those found in previous population-based studies, suggesting that despite the major advances to individual patients from technological advances, the potential contribution of hospital management to the reduction of subarachnoid haemorrhage mortality rates is likely to be limited.

How to become a nurse teacher.

It may take a minimum of 4 years to become a nurse teacher. Teaching is about communicating effectively with a variety of groups. The ENB and UKCC have clear criteria that must be met. If teaching is for you, it is important to be proactive, explore alternatives and plan your way forward. The role of the teacher is open to a wide interpretation. Nurse teaching is moving towards an all-graduate profession. Your college of nursing library will have information packs and details of local courses. The college principal will be pleased to offer careers advice to prospective teachers.

A fast responding intra-arterial pH electrode for use in the peripheral artery of adult humans and large mammals: a technical development for use in research.

A rapidly responding intra-arterial pH electrode has been developed to provide a continuous record of arterial pH (pHa) in the radial artery of adult humans and large mammals. The current method for measuring pHa is discontinuous and is achieved by taking blood samples and subsequently measuring pHa in a blood gas analyser. The development of an intravascular electrode is needed for studies on the chemical control of pulmonary ventilation when a continuous record of pHa is required. It will be particularly useful in rapidly changing circumstances such as at the onset and termination of exercise and at sleep onset and arousal. The sensor of the electrode system described here consists of a pH sensitive plastic membrane adherent to the tip of a catheter. This catheter is threaded down a radial artery catheter and protrudes 2-3 mm into the arterial lumen. The electrode system has been used in patients in an intensive care unit and in patients undergoing sleep studies. No adverse complications have occurred. The records obtained showed that arterial pH faithfully followed changes in pulmonary ventilation.
