An unusual case of bilateral Krukenberg tumors having a predominant multicystic mucinous component which on gross and microscopic examination resembled a mucinous cystadenocarcinoma is presented. It is important to distinguish between these two tumors as Krukenberg tumours have a significantly worse prognosis.
A 26-year-old man presented with pain in the right upper abdomen. Ultrasonography revealed a large solitary right lobe liver abscess, which was treated by aspiration and antiamebic treatment. The patient continued to be symptomatic with the abscess increasing in size. Repeat aspiration revealed a pure growth of coccidioidomycosis, which was confirmed on culture. He was treated with amphotericin-B; the abscess resolved completely and the patient has remained asymptomatic at one-year follow up.
An aged female donkey developed a severe, localised, suppurative panniculitis secondary to a skin wound. Bacterial culture of swabs taken from the wound gave a profuse growth of multi-drug-resistant Pseudomonas aeruginosa, a profuse growth of Escherichia coli and a moderate growth of beta-haemolytic Streptococcus species. The lesion did not respond to conventional medical and surgical treatment and continued to progress. Six applications of sterile larvae (maggots) of the common greenbottle, Lucilia sericata, were used to debride the wound successfully.
Interventional radiology has had a significant impact on patient care over the past 20 years, with the development of low-risk procedures. Nurses specialising in this field must maintain their knowledge of innovations to ensure that the nursing care of such patients remains appropriate. This article describes the indications for and procedural technique of transjugular intrahepatic portosystemic shunting (TIPS), a new initiative in interventional radiology. Although there are risks and side-effects, initial evidence of the method's efficacy suggests it has great potential in reducing morbidity and length of hospital stay.
