"Index Medicus" and 18 other publications have been consulted in an attempt to provide an easily assimilated selection of the recently published and widely dispersed material relevant to the ethical debate the editors of the "BMJ" called for on 4 March 1978. The medical profession is shown to be deeply divided on the ethics of a predictive test for Huntington's chorea. Some members are already using the prospect of a reliable test as an inducement to potential transmitters of this incurable hereditary disease to postpone procreation. Other members would prefer to see any future test withheld from every applicant until such time as radically improved means of treatment or a cure is discovered. The evolution of generally acceptable professional guidelines requires further informed debate.
Studies of deinstitutionalization often use a limited set of criteria by which to judge success. Social integration and recidivism, for example, are particularly popular criteria. Yet such criteria may not reflect the diverse settings in which community care occurs, nor the diverse views of success held by different interest groups. The present study presents a methodology for developing more comprehensive criteria for evaluating social programs, including deinstitutionalization. The methodology is illustrated by analyzing the perceptions of different interest groups about the nature of successful care for mental patients in nursing homes.
(1) The use of an extracorporeal blood circulation method has made it possible to obtain information on the time course of possible modifications in acid-base status of trout blood during hypoxia. (2) Two series of experiments are described: (a) Continuous measurement of arterial pH, PCO2 and PO2 during 24 h of moderate hypoxia (60 Torr). (b) Continuous measurement of acid-base status during 24 h of deep hypoxia (40 Torr). (3) The results of the first series show that hypoxia provokes a stimulation of ventilatory frequency and amplitude which is associated with an increased output of CO2 from the blood which induces respiratory alkalosis. This alkalosis is compensated during the following 24 h, and results in a return to the initial pH, a decrease in concentration of HCO-3 + CO2-(3) and an increase in lactate levels and Cl- ion concentration. (4) The second set of experiments show that in deep hypoxia the respiratory alkalosis is preceded by a brief stage of deep metabolic acidosis during which lactate levels suddenly increase dramatically.

Continuous recordings have been made of pH, PO2, and PCO2 of arterial blood (pHa, PaO2, PaCO2) in an extracorporeal circulation during periods of hypoxia (inspired PO2 45-10 Torr) in sea bass, Morone labrax. During moderate hypoxia hyperventilation was accompanied by an increase in pHa. Continuation of moderate hypoxia for periods up to 24 h produces an increase in lactate and a consequent decrease in pHa. During deep hypoxia there is a very brief alkalosis as ventilation increases but a marked decrease in pHa as lactate levels rise. Recovery from hypoxia is associated with an increase in lactate concentration reaching values of more than 6 meq X l-1 following deep hypoxia, and pHa falls to 7.73. PaO2 recovers rapidly, but recovery of PaCO2 is not so rapid and together with the residual hyperventilation indicates that the fish is paying off an O2 debt.

Amoxapine, a new tricyclic antidepressant, has been recently released in the United States and Europe. There has been limited experience with amoxapine overdose, and no serious reactions or sequelae were observed in the reported cases. A case of amoxapine overdose associated with rhabdomyolysis and acute renal failure is reported. Amoxapine overdose could cause convulsions or prolonged coma leading to atraumatic rhabdomyolysis and myoglobinuric renal failure. Direct muscle or renal toxicity from this drug and its metabolites should not be excluded until evidence to the contrary is available. Acute renal failure should be looked for in cases of amoxapine overdose.

A closed injury of the left index finger displacing the skeleton out of its soft tissue bed is reported. Reduction of the displaced segments in place resulted in normal function of the finger.
Several simple alkyl and aralkyl derivatives of mesoionic thiazolopyrimidines (1) and mesoionic 1,3,4-thiadiazolopyrimidines (2) were found to possess theophylline-like activity as inhibitors of cyclic-AMP phosphodiesterase (PDE). Reduction of the C2-C3 double bond of 1 or replacement of the sulfur atom of 1 or 2 with an N-methyl group nearly abolishes activity. Optimal activity appears to be associated with a hydrophobic substituent at the N8 position. The five-membered ring of 1 can be replaced by a pyridine or isoquinoline nucleus without untoward effects. Preliminary kinetic data suggest that the type of enzyme inhibition produced by the mesoionic derivatives is similar to that observed for theophylline. Thus, several novel mesoionic ring systems display activity as inhibitors of cyclic-AMP PDE and can serve as lead compounds for further investigation.
The influence of a nutrition poor in Cu on the copper status of dairy cows was investigated with and without Cu-supplementation through the mineral mixture typical of the territory and enriched with Cu. The roughage produced in the territory investigated contained less than 8.0 mg Cu/kg and did not guarantee a Cu supply of the dairy cows meeting their needs. Feeding the mineral mixture rich in Cu improved the Cu-status of the dairy cows, did, however, in one of the territories not result in a Cu-incorporation in the liver that meets the needs. Possibly the reason for this divergent behaviour is an oversufficient S- and Fe-supply. In contrast to this, the Cu-content of the cerebrum of the cows in both territories showed the effect of the Cu in the mineral mixture.
The influence of copper deficiency on the reproduction performance of cows has not often been investigated. In two habitats feeding cows with mineral mixtures rich in copper resulted in a significant reduction of the aperiodically extended sexual cycles. The beginning of the first visible symptoms of heat, the period between gravidities, the duration of gravidity, the birth weight of the calves and the Cu-content of the colostrum remained uninfluenced by Cu-supply. The liver weight increase of the calves from cows without Cu-supplement during gravidity was by 5% lower during the first 90 days of life than that of the offspring of cows with additional intrauterine Cu-supply (p greater than 0.05). The mineral mixture increased the milk yield by 0.4 resp. 0.5 kg and the milk fat content by 0.10 resp. 0.13%. This resulted in an excess production of 32 resp. 62 g milk fat per cow and day of the experiment (p greater than 0,05).
1. Short loops of Henle were artificially perfused with saline solutions containing 5 or 0 mM-glucose in the presence and absence of phlorizin. 2. Net fluid reabsorption was greater when glucose but no phlorizin was present than in all other series. 3. Glucose was reabsorbed from glucose-containing perfusate and this was abolished by phlorizin. Secretion of glucose occurred into the perfusate which initially contained no glucose and this secretion was enhanced by phlorizin. 4. Sodium reabsorption was inhibited by phlorizin, when glucose was present, but enhanced by phlorizin when glucose was absent. 5. It can be shown that there is secretion of some osmotically active solute in all series. Its secretion is enhanced by phlorizin in the absence of glucose.
A method is described for the measurement of hydrogen in expired air, using an electrochemical detector. The apparatus is simple to use and sensitive. Its application is illustrated by studies of small bowel transit time made by measuring the time between oral ingestion of the unabsorbable carbohydrate lactulose and a rise in the concentration of hydrogen in expired air. In 20 control subjects transit time was 93.0 +/- 6.6 minutes, while in 16 patients with diarrhoea due to the irritable bowel syndrome it was 54.1 +/- 6.3 minutes (P less than 0.001), suggesting an abnormality in small intestinal motility in these patients. Loperamide, a potent antidiarrhoeal agent, increased transit time in 12 of these patients from 56.3 +/- 6.7 to 100.0 +/- 10.2 minutes (P less than 0.001).
