[Emergency surgery for thrombosed Björk-Shiley mitral valve prosthesis].

A case of a thrombosed Björk-Shiley mitral valve with severe pulmonary edema which developed six years following an initial operation was reported. Doppler echocardiography was one of the most useful tools to evaluate prosthetic mitral valve dysfunction. Peak blood flow velocity was high (2.7 m/sec) compared to the data (1.7 m/sec) obtained six months before the reoperation. Emergency surgery disclosed the thrombosed valve prosthesis which was replaced with a St. Jude Medical valve. The patient recovered successfully with no problems.

Heritable pulmonary lobation anomaly in the rat.

A new mutant gene which caused fusion of lung lobes was found in the Wistar rat. The genetic analysis revealed an autosomal recessive inheritance and the mutant gene was named fused pulmonary lobes (gene symbol: fpl). The right lung of the fpl/fpl homozygotes had fused lobes of varying degrees. The fpl/fpl homozygotes were semilethal at the neonatal stage and had externally visible associated malformations such as malocclusion of incisors, eyelid anomalies, and digit abnormalities in the fore- and hindlimbs with different incidences. These traits were considered to be caused by the pleiotropic effects of the fpl gene.

Core-cooling, heart-perfusion, lung-immersion technique provides successful cardiopulmonary preservation for heart-lung transplantation.

Mongrel dogs underwent heterotopic heart-orthotopic left lung transplantation. In Group I (N = 6), donor organs procured following core cooling to 15 degrees C on cardiopulmonary bypass (CPB) with cardioplegic arrest were immediately transplanted. In Group II (N = 6), following cardioplegic arrest without CPB core-cooling, the pulmonary artery was flushed with modified Collins' solution. Heart-lung blocks were immersed in extracellular solution for 6 hours and then transplanted. In Groups III and IV (N = 6 each), following CPB core-cooling to 15 degrees C and cardioplegic arrest, the organ blocks were immersed in extracellular solution (Group III) and the heart was perfused with oxygenated extracellular solution (Group IV). Evaluation of lung function using differences in arterial oxygen tension between the left and right atria demonstrated no differences between groups. However, extravascular lung water and pulmonary vascular resistance were significantly elevated in Group II. Cardiac function assessed by the ratio of end-systolic pressure to end-systolic dimension was significantly better in Group IV than in Groups II and III. Thus, adequate 6-hour hypothermic cardiopulmonary preservation with core cooling plus heart perfusion can be achieved for heart-lung transplantation.

An experimental study of a new sutureless intraluminal graft with an elastic ring that can attach itself to the vessel wall. A preliminary report.

A new sutureless intraluminal graft was developed with an elastic ring made of a flat spring. The diameter of the ring could be reversibly reduced by compression. The sutureless intraluminal graft with an elastic ring can attach itself to the vessel wall by elastic expansion of the ring. The elastic-ring graft was implanted in the descending thoracic aortas of nine dogs and was evaluated histologically and angiographically at different intervals from 18 to 150 days. No complication such as detachment of the ring, aortic rupture, stenosis, or aneurysmal dilatation was observed. With the new graft neither ligation nor posterior aortic wall dissection is necessary, and no anastomotic stenosis occurs. This graft is applicable even if the diameter of the aorta is small. Therefore the elastic-ring sutureless intraluminal graft promises theoretic advantages over sutureless methods that use tape ligation.

New extension of the Fontan principle: inferior vena cava-pulmonary artery bridge operation.

A new modification of the Fontan operation for complex cardiac anomalies is described. The technique comprises superior vena cava-pulmonary artery anastomosis and inferior vena cava-pulmonary artery bridging via a composite conduit connected to the inferior vena cava via the right atrium, totally excluding the right heart. The procedure was applied to a patient with complex cardiac abnormalities associated with asplenia. After operation the patient had good haemodynamic function initially though he died from multiorgan failure after airway bleeding on the 24th postoperative day. The procedure is a simple alternative to the complicated atrial partition required for the conventional Fontan operation.

Efficacy of intra-aortic balloon pumping for failing Fontan circulation.

The Fontan circulation occasionally cannot be sustained by currently available medical treatment. In these circumstances, mechanical assistance to the Fontan circulation should be considered. We have experienced three cases where Fontan circulation was assisted by intra-aortic balloon pumping (IABP). The hemodynamic improvement and weaning from IABP were successfully achieved in all cases. One patient survived the serious postoperative course with the aid of IAPB, and two patients died following multiorgan failure, though the circulation had eventually been re-established. By studying the postoperative hemodynamics assisted by IABP, it is concluded that IABP is a reasonable, efficacious strategy to support and to re-establish the failing Fontan circulation, though the changes in hemodynamic variables will not immediately coincide with IABP application.

Morphological and genetic characteristics of the open-eyelid mutant spontaneously occurring in NC-strain mice.

A heritable open-eyelid mutant which arose spontaneously in the NC strain of mouse in 1982 has been maintained as the NC-eob line through brother-sister matings. When the line exceeded the 12th generation, its reproductive capabilities were initially compared with those of the NC strain. Then eye abnormalities were investigated morphologically and genetically. Although most of the reproductive parameters in the NC-eob line were comparable to those in the NC strain, a significant decrease was observed in the mean number of pups delivered, and the viability of pups at 7 days of age was reduced to 55%. The eyelids of pups were widely open at birth. The exposed cornea dried within a few hours and a bloody exudate appeared around the margin of the eyelids. At 1 day of age, the eye opening was smaller in size and covered with a scab. Histological examination demonstrated apparent inflammatory changes in the cornea and conjunctiva. At 21 days of age, the eyes revealed opacity, a rough surface, and microphthalmia. Histological findings in the cornea, such as keratosis and vascularization, coincided with the eye opacity. The weight and size of the eyeballs were almost the same as those in normal NC pups. However, a significant decrease in the length of the palpebral rima was evident and this was considered to be the cause of the microphthalmia. The results of genetic analysis indicated that these eye abnormalities were caused by an autosomal recessive gene, eob. These results suggested that the eob gene might be a new mutation manifesting semi-lethality as a pleiotropic effect.

Results of surgery for aortic regurgitation due to aortic valve prolapse.

The clinical, hemodynamic and pathological findings of 13 patients with aortic regurgitation due to aortic valve prolapse caused by advanced myxomatous degeneration were evaluated. Eleven patients showed a favorable outcome with no complications resulting from surgery. One patient died from aortic dissection, and another died suddenly from an unknown cause. Five patients had mitral valve prolapse as a complication. Ten patients (77%) had a long-standing history of hypertension. Twelve patients (92%) were male. None of the patients had the stigmata of Marfan's syndrome. All patients had marked myxomatous degeneration of the aortic valves without any inflammatory changes. Two patients showed microcalcification; 7 demonstrated moderate fibrosis. Five patients showed severe fragility of the cusps which appeared redundant, gelatinous and softened by degenerative changes. Myxomatous degeneration of the aortic valve is not rare, and, in fact, it may be one of the most common pathologic and clinical entities associated with pure aortic insufficiency.

A case of complete atrioventricular block due to malignant lymphoma.

A case of malignant lymphoma associated with complete heart block in a 30-year-old woman is reported. The patient progressively deteriorated despite temporary pacing and died 24 days after being admitted. Microscopic examination of the heart revealed marked infiltration by lymphoma cells in the atrioventricular node and the bundle of His. A diffuse lymphoma (large cell type, B cell) was diagnosed. This case is considered to be rare, since complete heart block was the first and only manifestation of the malignant lymphoma.

Improvement of left ventricular function after surgical correction of chronic aortic regurgitation.

Serial left ventricular (LV) echocardiographic studies were performed in 21 patients before and after aortic valve replacement for chronic aortic regurgitation. The effect of valve replacement on LV dimensions, cross-sectional area of the LV muscle and LV function was determined from the echocardiographic data. The relation between degeneration of the myocardium and surgical outcome was also investigated. The average LV end-diastolic dimension decreased from 66.0 +/- 8.3 mm to 46.3 +/- 5.7 mm twelve months postoperatively. The average LV end-systolic dimension also fell from 43.4 +/- 8.1 mm to 31.1 +/- 5.0 mm. The muscle cross-sectional area decreased from 33.1 +/- 5.1 cm2 to 24.5 +/- 4.0 cm2, indicating a decrease in LV mass. The indices of contractility (fractional shortening, ejection fraction and mean velocity of circumferential fibre shortening) had a tendency to decrease one month after surgery, but they subsequently increased to the normal level 12 months after surgery. Nineteen out of 21 patients showed a favorable outcome as to the functional status. The remaining two patients had a large LV dimension and subnormal contractility, and they failed to show a significant reduction in the follow-up period. The muscle score in the two patients was greater than 8 points, which indicated irreversible impairment of the myocardium. Patients with persistent postoperative LV enlargement have a poor prognosis and should be identified so that aggressive medical treatment can be instituted.

Activity and stability of recombinant human superoxide dismutase in buffer solutions and hypothermic perfusates.

The stability of recombinant human superoxide dismutase (r-hSOD) in buffer solutions was studied in solutions at various pH and temperatures. Additionally, we studied the effects of incubation with proteases, serum and two types of hypothermic perfusates. R-hSOD was stable in the pH range of 6-11 and at temperatures up to 80 degrees C for 30 min. R-hSOD activity was not affected by incubation with trypsin, aminopeptidase M or serum for 2 h. R-hSOD activity determined at various temperatures (4-37 degrees C) did not vary remarkably. R-hSOD in hypothermic perfusates was stable at 4-37 degrees C for 24 h.

[A case of hepatocellular carcinoma with survival of more than 7 years after extended right lobectomy].

A 48-year-old male was referred to our hospital because of a space occupying lesion that had been revealed by hepatic scintigraphy, which had been seen because of an evaluation of his serum alpha-fetoprotein level while under therapy for chronic hepatitis. Laboratory data revealed an impaired liver function and a hepatic angiography showed a hypervascular tumor of the right hepatic branch. An extended right lobectomy was performed. Postoperatively, this case was histologically diagnosed as hepatocellular carcinoma. Despite having had an intraportal tumor thrombus, the patient survived for 7 years and 3 months and died of a cancer recurrence with hepatic failure. In this case surgical resection proved very useful for survival over a long period.

[Surgical treatment and result of high aortic occlusion].

Twenty-one patients with high aortic occlusion treated at our institution from 1967 to 1986 were reviewed. There were seventeen men aged from 39 to 78 (mean age: 61.0) underwent surgical intervention. All patients presented clinical manifestations of vascular insufficiency of the lower limbs; rest pain in eleven patients, intermittent claudication in nine and the others. Sexual impotence was present in eight patients. Renal artery involvement was seen in one case, and renovascular hypertension was observed in this patient. Hypertension and ischemic heart disease were present in twelve cases, cerebrovascular insufficiency in one case, diabetes mellitus in three cases. The following surgical treatments were performed; end-to-end Y-shaped Dacron graft implantations from the infrarenal abdominal aorta to the common femoral arteries in six patients, onlay V-shaped Dacron graft implantations in three patients, axillofemoral extra-anatomic bypass in four patients, and amputation only in one. The hospital mortality was 18% (3/17). Twelve patients discharged from the hospital are followed up (average period was 118 months), but the follow up was lost in two patients. There were two late deaths, which course was not related to operations. The prognosis of high aortic occlusion after anatomic bypass is good, thus it was concluded that anatomic bypass with Y-shaped or V-shaped Dacron graft was recommended and extra-anatomic bypass might be performed only in a high risk patient.

[Healing of an external velour warp-knit Dacron vascular prosthesis].

An external velour warp-knit Dacron vascular prosthesis (8mm in internal diameter, 6cm in length), whose porosity is 1200cc/cm2/min at 120mmHg, was implanted in the descending thoracic aorta of 30 mongrel dogs. The dogs were sacrificed at different intervals between 2 weeks and 6 months. All grafts were patent at sacrifice and no serious complication was observed. Thrombus free surface score was about 90%. Cell coverage on the flow surface was first observed near anastomoses in 1-month specimens and was almost completed in 2 months. Transinterstices healing was completed in 2 months. This excellent healing in a canine model would allow clinical availability of this graft.
