[Combination chemotherapy of UFT with adriamycin in advanced gastrointestinal cancer].

Seventeen patients with advanced gastrointestinal cancer were treated with a combined chemotherapy of UFT with Adriamycin (UFT-A). The UFT-A regimen consisted of UFT, 600 mg/body daily. As for the Adriamycin, 10 mg/body was give intravenously from day 1-4 and was repeated every two weeks. Partial responses were seen in 7 cases (41%) (5 cases of gastric cancer, 1 case of colon cancer, and 1 case of bile-duct cancer) out of 17 evaluable patients (7 cases of gastric cancer, 3 cases of colon cancer, 4 cases of biliary tract cancer, and 3 cases of pancreatic cancer). Two patients had minor responses, and in eight patients their disease had stabilized. As for side effects, nausea and vomiting occurred in seven patients (41%), and anorexia was observed in eight patients (47%). Two patients (12%) showed a leukopenia count of less than 2,000/mm3 and none of these seventeen patients had thrombocytopenia, of less than 5 x 10(4)/mm3. Considering these results, UFT-A therapy appears to be useful in cases of advanced gastrointestinal cancer, especially gastric cancer.

Effects of human recombinant interleukin 2 in patients with chronic hepatitis B: a preliminary report.

Five patients with HBe antigen-positive chronic active hepatitis were treated with recombinant interleukin 2 (r-IL2) for 3 to 4 wk. r-IL2 inhibited HBV replication during therapy as judged by decrease of serum DNA polymerase activity. Two of five patients lost HBeAg from serum. Furthermore, characteristic increases of serum aminotransferase were observed during r-IL2 therapy. This rise might have been mediated by enhanced immune responses to hepatitis B virus and increased rate of lysis of the infected hepatocytes. No consistent changes in immune markers such as OKT4 positive cells or OKT8 positive cells were demonstrated during or after the therapy. The activities of natural killer cells gradually increased during therapy, although they tended to decrease within 1 wk after the start of r-IL2 injections. In two responders who lost HBeAg from serum, the natural killer activity was high before the therapy, compared with nonresponders. Furthermore, the percentage of OKT4-positive cells in the responders was increased within 1 to 2 wk after starting r-IL2 injections. These findings suggest that the responders might be immunologically different from the nonresponders. Recombinant-IL2 therapy over short periods did not result in complete clearance of hepatitis B virus. Further studies with high doses of r-IL2 given over longer periods are warranted.

Rabbit autoantibodies to actin induced by immunization with modified homologous actins.

Actin is a major antigen involved in the reaction of smooth muscle antibody positive sera from patients with chronic active hepatitis. In the present study, actin extracted from rabbit skeletal muscle was denatured by sodium dodecyl sulfate and was immunized into the rabbit, a homologous animal for actin. The rabbits, thus immunized, produced antibodies reactive with actins of homologous and heterologous animals. In addition, the antibodies showed reactivity with autologous actin. It indicates that the denatured homologous actin is capable of terminating immunological tolerance to actin and induces formation of autoantibody to rabbit actin. This phenomenon may be implicated in the occurrence of anti-actin antibody in sera from patients with chronic liver disease and several other diseases.

[Alpha-fetoprotein producing gastric cancer responding to combined chemotherapy with UFT and adriamycin].

A case of AFP producing gastric carcinoma with liver metastasis that showed marked response to combined chemotherapy with UFT and Adriamycin (ADM) is reported. A 61-year-old man was admitted because of lassitude and abdominal fullness. An upper GI series and computed tomography revealed gastric cancer (Borrmann III) and multiple liver tumors. He had a remarkably high serum AFP level (90,000 ng/ml) and a high CEA level (270 ng/ml). The presence of AFP in the tumor cells of the biopsy specimen was proved immunohistochemically. He was treated with 600 mg of UFT orally every day and ADM (10 mg, iv, on days 1-4, repeated every 14 days), resulting in marked regression (PR, partial response) of both the primary tumor and liver metastasis on the 33 rd day after the start of treatment, with decreasing of serum levels of AFP and CEA. The patient has been asymptomatic without evidence of recurrence for a follow-up period of more than three months with continuing treatment in our outpatient clinic. UFT-ADM therapy appears to be useful for gastric cancer.

Continuous fluorometric assay of Ca2+ transport by liposomes with Quin 2 entrapped: effect of phospholipase A2 and unsaturated long-chain fatty acids.

The amount of free calcium in the cytoplasm is important in stimulation coupled with a number of cellular functions. The putative ionophoretic action of membrane lipid metabolites on Ca2+ offers convenient explanation of the stimulation-coupled mobilization of cytoplasmic Ca2+. To analyze the ionophoretic action of the lipid metabolites, we devised a sensitive method to study Ca2+ transport that uses liposome-entrapped Quin 2. A calcium ionophore, A23187, increased the fluorescence intensity of the Ca2+-Quin 2 complex as a function of Ca2+ transport into liposomes. A similar Ca2+ flux into the liposomes was induced by phospholipase A2 (PLA2) and by various long-chain fatty acids in liposomes that consist of phospholipids containing unsaturated fatty acids. The potencies of the fatty acids for Ca2+ transport is inversely correlated with their melting points. The oxidized products of the unsaturated fatty acids increased the Ca2+ and nonspecific permeability of the biological membranes. These results suggest that stimulation-coupled PLA2 activation might mediates the mobilization of cytoplasmic Ca2+.

Improvement of the anoxia-induced mitochondrial dysfunction by membrane modulation.

The mitochondrial dysfunction induced by anoxia in vitro was improved with chlorpromazine, cepharanthine, bromophenacyl bromide, and mepacrine without affecting phospholipid or adenine nucleotide metabolisms. The drugs inhibited lipid peroxidation by Fe2+, mitochondrial disruption by Ca2+, and membrane perturbation by lysolecithin, and retained the activity to control H+ permeability across mitochondrial membranes. The drugs appeared to preserve the functions by acting to suppress the development of membrane deterioration which may have resided in the deenergization of mitochondria in the absence of oxygen.

Endoscopic sonography of the liver--diagnostic application of the echolaparoscope to localize intrahepatic lesions.

To overcome the limitations of ultrasonography and laparoscopy for abdominal diseases, the use of echolaparoscopy was investigated. The instruments used were the prototype echolaparoscope type 1-3 manufactured by Olympus Optical Co., Tokyo, Japan. The first prototype has the external diameter of 12 mm. It consists of an internal shaft connecting to a fixed transducer and a rotating mirror connected to the motor unit and a regular laparoscope to assess the position of the scanning head. The second and third prototypes were versions of the former and had an outer diameter of 10 mm to allow interchangeable use with a regular laparoscope. Transducers used were either of 10 or 7.5 MHz, 7 mm in diameter, and ultrasonic scanning was made by the mirror reflection method. A total of 67 cases with various abdominal diseases were examined on 73 occasions. The merit of the method is its ability to visualize occult lesions in the liver such as cysts and tumors. Differentiation between hepatic hemangioma and hepatocellular carcinoma could be mad without difficulty due to its increased resolution. Furthermore, this method was used as a guide to liver biopsy of occult tumors and decision making in cases of surgical resection of the hepatic lesions.

Endoscopic ultrasonography in the diagnosis of pancreatic carcinoma. The use of a liquid-filled stomach method.

To overcome limitation of ultrasound diagnosis of pancreatic diseases, especially of malignancy, the use of an echoendoscope was investigated. The instruments were the prototype echoendoscopes type 1-3 manufactured by Olympus Co., Tokyo, Japan. The first prototype was a mirror-type radial sector scanner equipped to a gastrofiberscope . The 2nd and 3rd types were of similar construction except the use of a small transducer with an increased frequency (7 mm in diameter, 7.5 MHz) and of direct rotation of the transducer connected to the flexible shaft. To visualize the pancreas effectively, a liquid-filled stomach method to use deaerated water in sufficient quantity was employed. As a result, complete visualization of the whole pancreas could be accomplished mainly from the gastric lumen via the gastric wall. Though the number of cases examined were still limited, ultrasonic differential diagnosis between benign and malignant lesions could be made much more clearly than those by currently used ultrasonic scanning.

Postnatal developments of AChE activity in the hippocampus of the reeler mutant mouse.

The acetylcholinesterase (AChE) activity of the hippocampus and the dentate gyrus in the reeler mutant mouse was studied histochemically by the method of Karnovsky and Roots. Developmental studies were done on days 0,3,6,9,12,15,18 and 21 postnatally, and in adults. The adult reeler mouse differs from the normal mouse in that there is no accumulation of the activity at the junction between the stratum lacunosum-moleculare and the stratum radiatum, weaker and divided activity in CA1, and the translocation of the activity to the granule cell zone of the dentate gyrus. The results obtained are considered to be due to the cells' ectopia. But also, other factors such as genesis of the cells in regard to constructing the cytoarchitecture, may influence them. According to the postnatal observations, the developmental pattern of AChE activity in the reeler mouse is about the same as in the normal mouse. AChE-rich cells in the hippocampus showed maximum activity and number from days 12 to 15. The neuropile reactions increased after day 9 in both kinds of mice. The relationship between the movement of the AChE positive neuropiles and cells and their functions are also discussed.

Biological activity of synthetic analogs of serum thymic factor.

A series of analogs of serum thymic factor (Facteur Thymique Sérique; FTS) devoid of C- and N-terminal few residues were synthesized and their ability to induce Thy-l (theta) antigen in vitro on mouse T lymphocyte precursors was examined. The pentapeptide moiety (Lys-Ser-Gln-Gly-Gly) seems to be important for the expression of agonistic activity. Some of the short chain peptides showed antagonistic properties. Synergistic activation by two inactive analogs was observed.
