Seroprevalence of human herpesvirus 6 infection in Brazilian and Japanese populations in the north-east of Brazil.

The seroprevalence of human herpesvirus 6 (HHV-6) infection was examined for 434 Brazilians and 250 Japanese immigrants living in Recife and its vicinity, in the North-East of Brazil. A total of 684 sera from the healthy individuals were screened for IgG antibodies to HHV-6 by anticomplement immunofluorescence (ACIF) test. The seropositivity rate to HHV-6 showed little difference between the two groups: namely, it was 76.5% for Brazilians and 77.2% for Japanese immigrants. The seropositivity rate was constantly higher in females than in males. The high prevalence of anti-HHV-6 antibodies among children indicates that HHV-6 infection occurs very early in life.

Serologic evidence of toxocariasis in northeast Brazil.

Sera from 54 Brazilian children with a history of tropical eosinophilia syndrome were screened for toxocariasis. The enzyme-linked immunosorbent assay detected 21 positive sera (40%). Many of these cases (15 of 21) were positive for Dilofilaria immitis as well. Of the six Toxocara-positive children with hypereosinophilia, two were asthmatic. Their sera were further examined by the Toxocara species-specific Ouchterlony's diffusion-in-gel test and one Toxocara canis and one T. cati infections were identified. This is the first report of serologically diagnosed toxocariasis in Brazil.

Prevalence of cytomegalovirus antibodies in Brazilian and Japanese populations in the north-east of Brazil.

The serological status to cytomegalovirus (CMV) was examined for 616 Brazilians and 399 Japanese immigrants living in the North-East of Brazil. The sera were screened for IgG antibodies to CMV by enzyme-linked immunosorbent assay (ELISA). The overall prevalence of CMV antibodies was higher in the Japanese population (83.7%) than in the Brazilian population (69.8%). The seropositivity was analyzed by factors of age, sex, ethnic background, and socioeconomic status. Mother-baby contact seems to be a factor of significance in the seroepidemiology of CMV infection.

Dietary marine lipids suppress murine autoimmune disease.

Dietary marine lipids reduce both mortality and the severity of glomerulonephritis in inbred murine strains which develop spontaneous autoimmune disease. The protective effects of marine lipids appear to be accounted for by the major n-3 fatty acids in these preparations, 20:5 and 22:6. The n-3 fatty acids in dietary fish oil are extensively incorporated into several lipid classes in the spleen of autoimmune mice, including phosphatidylinositol, phosphatidylethanolamine, plasmalogens and saturated ether-linked phospholipids as well as diacylphosphoglycerides. The effects of dietary marine lipids on autoimmune disease in experimental models are highly specific. Careful controlled trials will be required to establish the role of dietary marine lipids in the therapy of human autoimmune disease.

Moderately proteinuric IgA nephropathy: prognostic prediction of individual clinical courses and steroid therapy in progressive cases.

Eighteen of 60 patients with persistent moderate proteinuria between 1.0 and 2.0 g/day and a long-term follow-up of more than 4 years received steroid therapy for a mean period of 18 months. Fifteen of these 18 patients maintained their initial creatinine clearance (CCr) of 70 ml/min or more. The remaining 42 received antithrombocyte drugs and/or nonsteroidal anti-inflammatory drugs, with 31 of them keeping their initial Ccr values of 70 ml/min or more. All 14 cases with initial Ccr values of less than 70 ml/min in both the steroid and nonsteroid groups followed progressive courses, with 12 ending up in hemodialysis. Of the nonsteroid, preserved group of 31 cases, 12 followed a stable course, 10 a progressive course, and the other 9 went into end-stage renal failure necessitating hemodialysis. There were no differences in initial clinical features among these three subgroups. But histological changes were milder in the stable subgroup than in the progressive and hemodialysis subgroups. Further, the total score of eight histological parameters was 6 or less in all but 1 of the cases of the stable subgroup, but 8 or higher in the other two subgroups. Therefore, in the progressive cases alone with initial Ccr values of 70 ml/min or more and high total scores of 7 or more, the long-term clinical courses were compared between steroid and nonsteroid groups (10 and 20 cases, respectively). In the steroid group proteinuria decreased significantly and renal function was preserved well as compared with that in the nonsteroid group.(ABSTRACT TRUNCATED AT 250 WORDS)

Elevation of laminin and beta-subunit of prolyl 4-hydroxylase in the sera of human subjects with Schistosomiasis mansoni.

Serum levels of several markers for liver fibrosis were measured utilizing three groups of human subjects related with schistosomiasis mansoni in northeast Brazil; (1) 20 Schistosoma mansoni egg-positives, who have never been administered with anti-schistosomal drugs, (2) 29 egg-negative inhabitants in the endemic area of schistosomiasis, and (3) 23 egg-negative Japanese immigrants in the non-endemic area. None of these sera were positive for antibody to the surface antigen of human hepatitis B (HBs) and circulating HBs antigen. There was no significant difference in the serum levels of N-terminal peptide of procollagen type-III between the egg-positive subjects and either of the egg-negative Brazilian or Japanese immigrants, whereas the mean value of serum laminin significantly increased in the egg-positive subjects. A significantly higher concentration of serum immunoreactive beta-subunit of prolyl 4-hydroxylase (IR beta PH) was also observed in the egg-positive subjects only in comparison with that of the egg-negative Brazilian. Serum laminin and IR beta PH concentrations of the egg-positive subjects did not correlate with the absorbance of enzyme-linked immunosorbent assay (ELISA) which utilized crude antigens isolated from schistosome adults or eggs. No significant difference in these two parameters was observed between two subgroups of the egg-negative Brazilian or Japanese immigrants divided according to the serological data by ELISA. These findings suggest that serum laminin and IR beta PH levels are worth further evaluation for their usefulness as the marker for liver fibrosis in schistosomiasis.

Steroid therapy in IgA nephropathy: a retrospective study in heavy proteinuric cases.

29 patients with IgA nephropathy whose proteinuria persisted at a level of 2.0 g/day or more and who received prednisolone treatment for 1-3 years were retrospectively evaluated on their clinical courses. 13 of 14 patients with renal dysfunction of less than 70 ml/min in initial creatinine clearance (Ccr) values subsequently entered a progressive course during a follow-up period of 47 months, leading to end-stage renal failure in 8 cases. On the other hand, only 1 of the other 15 patients with preserved renal function of 70 ml/min or more ended up with end-stage renal failure during a follow-up period of 74 months, although 7 underwent a progressive course. Three patients in the latter group experienced a prominent reduction in proteinuria to less than 1.0 g/day and maintained renal function. Meanwhile, the steroid group of moderate proteinuric patients with a creatinine clearance greater than 70 ml/min had a benign course, while the nonsteroid group had an unfavorable one. These results suggest that steroid therapy in IgA nephropathy may be able to stabilize a progressive course, especially in the early stage of the disease, although, because they come from an uncontrolled study, a definite conclusion cannot be drawn.

[Experimental mitral regurgitation in ischemia-induced papillary muscle dysfunction].

Mitral regurgitation (MR) reportedly develops by ischemia of the papillary muscles, which is called papillary muscle dysfunction. This report deals with the roles of papillary muscles and left ventricular walls on the pathogenesis of MR using graded injuries of these structures in 23 dogs. Implanted ultrasonic microcrystal and occluder with an electromagnetic flowmetry for the left circumflex coronary artery were the main experimental setting. Graded occlusion of the artery was done by the six-step approach regarding coronary blood flow (CBF) reduction (C1-C6). Left ventricular (LV) pressure, systolic thickening (%W: sonomicrometry) of the LV anterior (AW) and posterior walls (PW), and systolic longitudinal shortening (%S: sonomicrometry) of both the anterior and posterior papillary muscles (PPM) were measured. MR was assessed by left ventricular contrast two-dimensional echocardiography. In eight dogs, all the data were adequate for analysis. In category 3 (C3: 55-70% CBF of control), %S in PPM decreased, but %W did not change significantly, and only mild MR developed in three of the eight dogs. MR clearly developed in category 4 (C4: 40-54% CBF as compared with the control stage), where %S was replaced by holosystolic lengthening and %W reduced to 50% of the control state, and total occlusion (C6) accompanied by significant thinning of both the PW and AW. Thus, the asynergy of the LVPW was needed to induce the MR in seven of the eight dogs. It was concluded that the injury of the PPM alone is not sufficient to cause MR, and the associated ischemic changes of the LV free wall as well as LV dilatation are necessary to induce severe MR.

Quantitative analysis of mesangial areas in serial biopsied patients with IgA nephropathy.

Clinical courses of 22 patients with IgA nephropathy, who received serial renal biopsies at a mean interval of 36 +/- 11 months, were observed until 2 years after the second biopsy. Of 22 patients, 12 showed no detectable changes in semiquantitatively analyzed mesangial sclerosis between the serial biopsies. By quantitative analysis, on the other hand, mesangial areas decreased between the serial biopsies in 12 patients (group I), and increased in 10 (group II). At the time of the first biopsy, there were no differences between the two groups in sex, age, creatinine clearance (Ccr), urine protein, hematuria, hypertension, treatment and histological parameters. Although Ccr in group I was stable during the 5-year follow-up, group II showed progressive deterioration of Ccr (p less than 0.005). Histologically, mesangial hypercellularity and crescent formation improved in group I (p less than 0.005 and (p less than 0.05, respectively), and tubular atrophy and interstitial fibrosis progressed in group II (p less than 0.05, each). These results demonstrate that changes in mesangial areas evaluated by quantitative analysis between serial renal biopsies are a valuable prognostic indicator in IgA nephropathy.

Primary perianal actinomycosis over a thirty year period.

A 50-year-old Japanese man had had abscesses and draining fistulas in the perianal region. These lesions recurred, despite surgical treatment such as incision and drainage over a 30 year period. "Sulfur granules" were found in the pus from the abscess and Actinomyces israelii was cultured. Ampicillin-cloxacillin treatment lead to healing. The patient died 4 months later with a hepatoma and multiple metastases.

Steroid therapy in IgA nephropathy: a prospective pilot study in moderate proteinuric cases.

Forty-three patients with IgA nephropathy whose proteinuria persisted between 1.0 and 2.0 g/day were assessed in an uncontrolled pilot study of steroid treatment. Fourteen patients receive an initial dose of 40 mg/day of prednisolone, followed by gradual reduction of the dose over an average period of about 19 months. After discontinuation of corticosteroids, they were treated with non-steroid anti-inflammatory drugs and/or anti-thrombocyte drugs to the end of the study. Another 29 patients received non-steroid anti-inflammatory drugs and/or anti-thrombocyte drugs throughout the study. Fourteen patients treated with steroids experienced considerable reduction in proteinuria and maintained renal function over 81 months. In 29 patients treated with non-steroid anti-inflammatory drugs and/or anti-thrombocyte drugs alone, proteinuria did not decrease and renal function deteriorated significantly during 60 months. At the end of the study, differences in degree of proteinuria and in levels of renal function between the steroid and non-steroid groups were statistically significant. In addition, these differences became more distinct in patients with initial creatinine clearance values 70 ml/min or more in both groups. These results suggest that treatment with steroids in IgA nephropathy may be beneficial, especially in the early stage of the disease.

Renal retinal dysplasia with diffuse glomerular cysts.

A 32-year-old male with renal retinal dysplasia is presented. He also showed hearing loss and growth retardation. Laboratory data showed mild proteinuria, renal dysfunction and type-1 renal tubular acidosis. Computed tomography showed multiple cysts at the corticomedullary junction of both kidneys. Ocular examinations disclosed retinitis pigmentosa. On light microscopy of renal biopsy specimens, diffuse cystic dilatation of Bowman's space as well as dilated tubules with interstitial fibrosis and cellular infiltration were noted. Electron microscopy revealed a peculiar chromatin condensation of epithelial cell nuclei in Bowman's capsules, glomeruli and tubules. The association of renal retinal dysplasia with diffuse glomerular cysts has not, to our knowledge, been previously reported.
