BACKGROUND/AIMS: The aim of this study was to compare the clinical characteristics of bleeding peptic ulcers in the elderly with those in younger patients, retrospectively. METHODOLOGY: Between 1986 and 1994, 274 patients with bleeding peptic ulcers were treated with heater probe endoscopically. They were divided into 2 groups: 48 in the elder group (70 years of age or older) and 226 in the younger group (<70). We evaluated the rate of concomitant disease, rebleeding rate, incidence of emergency surgery, mortality and blood transfusion requirement between the 2 groups. RESULTS: The incidence of concomitant disease was significantly higher in the elderly group (83.3%) than in the younger group (33.3%) (p<0.01). The rate of rebleeding (younger group 23.5% vs. elderly group 31.3%), the incidence of emergency surgery (5.8% vs. 6.3%, respectively) and the rate of mortality due to hemorrhage (2.2% vs. 4.2%, respectively) were similar in the 2 groups. There was no significant difference in the mean volume of blood transfused. CONCLUSIONS: It was revealed that aggressive endoscopic hemostasis improved the mortality rate and the incidence of emergency surgery in elderly patients as well as in younger patients, provided that their general condition was monitored carefully.
UNLABELLED: The effect of passive movement of a leg on alpha-motoneurons of the soleus was investigated while the ankle was immobilized. Electrophysiological parameters of the H-reflex were used. Nineteen fully informed volunteers with no history of neurological or internal disease participated in the study. The mean age across all the experiments was 20.9 years. Contributions to the modulation of the soleus H-reflex by hip and knee joint movements were analyzed in two tasks. These were the most general movement methods used in physical therapy. Soleus H-reflexes were measured six times per experiment. Stimulation intensity was set at the 1.1 x M-wave threshold. RESULTS: 1. In both exercises, H/M decreased as flexion progressed with fast or slow movement. The quadriceps femoris inhibited soleus muscle alpha-motoneurons in both experiments. 2. H/M increased on initiation of extension in the two-joint exercise. In the single-joint exercise, inhibition was retained through the extension phase during both fast and slow movements. Hamstring effects were strong during extension of the leg. 3. In both exercises, the H/M for fast movement was lower than for slow movement at all angles, indicative that H/M decreases with fast movement irrespective of the effects produced by the hip joint.
OBJECTIVES: To conduct a phase I/II study of irinotecan with cisplatin to establish a recommended dose, and assess the safety, efficacy and feasibility of this regimen in unresectable advanced or recurrent gastric cancer. PATIENTS AND METHODS: In the phase I portion of the study, patients received a fixed dose of cisplatin (30 mg/m2) with escalating doses of irinotecan, ranging from 30 mg/m2 to 70 mg/m2, on days 1 and 15. In the phase II portion of the study, 40 patients were evaluated for response and safety at the recommended dose. RESULTS: Eighteen patients were enrolled in the phase I study. Dose-limiting toxicity (diarrhea and neutropenia) appeared at the irinotecan dose of 70 mg/m2. Therefore, the recommended irinotecan dose was 60 mg/m2. In the phase II study, 40 patients received cisplatin (30 mg/m2) plus irinotecan (60 mg/m2). Twenty-five out of 40 patients had received prior chemotherapy. The median number of cycles was 3.5. The response rate was 32.5% (13/40) overall, and 53.3% (8/15) in patients without prior chemotherapy. The median time to tumor progression (TTP) was 162 days. The median survival time was 288 days. Four patients (10%) developed grade 4 neutropenia and 3 patients (7.5%) developed grade 4 anemia. The only observed non-hematological toxicity at grade 3 or higher was diarrhea, seen in 2.5% (1/40) of the patients. CONCLUSION: Bi-weekly administration of irinotecan and cisplatin is safe and active for the management of unresectable advanced or recurrent gastric cancer.
PURPOSE: The soleus H-reflex during passive hip movement was measured to clarify the modulation of excitability of the soleus monosynaptic reflex during locomotion-like movement in spastic stroke patients. METHOD: The experiment was performed in five patients with spastic hemiparesis. The hip joint was moved passively ranging from 0 to 40 degrees. The knee joint was fixed at full extension and the ankle joint was fixed at the mid-position. During the movement, the soleus M-wave and soleus H-reflex were measured. RESULTS: Flexion caused a decrease in the soleus H-reflex, whereas extension caused an increase symmetrically for both the static and dynamic conditions. In addition, the lowest value was observed at the end of the flexion phase during fast movement. CONCLUSION: These findings indicate that the phase-related modulation of soleus H-reflex during hip movement is partially disordered in stroke patients.

Three congeners of coplanar PCBs (non-o-o'-chlorine-substituted polychlorinated biphenyls) were determined in representative commercial PCB preparations. The 3,3',4,4'-tetrachlorobiphenyl was highest in Kanechlor 400 (8500 micrograms/g) and Aroclor 1248 (6060 micrograms/g) followed by Kanechlors 300, 500, and 600 and Aroclors 1242, 1254, and 1260 in that order. The toxic 3,3',4,4',5-pentachlorobiphenyl and 3,3',4,4',5,5'-hexachlorobiphenyl were also detected in those mixtures. The highest concentrations of 3,3',4,4',5-pentachlorobiphenyl were found in Kanechlor 400 (89.3 micrograms/g) and Aroclor 1248 (62.3 micrograms/g) followed by Kanechlors 500, 300, and 600 and Aroclors 1254, 1242, and 1260 in that order. The 3,3',4,4',5,5'-hexachlorobiphenyl was detected in all Kanechlor mixtures, with the highest concentration (1.16 micrograms/g) in Kanechlor 500. However, among Aroclor mixtures, it was detected only in Aroclor 1254 at a concentration of 0.66 micrograms/g. The importance of these toxic constituents in PCB mixtures is worth considering in view of widespread pollution of the environment by PCBs.
Chronic polyarthritis was produced in inbred C3H/He mice after intraperitoneal (ip) administration of allogeneic thymocytes of inbred line BALB/c mice. The polyarthritis persisted more than 18 months without additional ip treatment. Histologically, synovial cell hyperplasia, granulation tissue formation, and inflammatory cell infiltration were seen. More than 6 months after the initial administration, range of motion of the affected joints was significantly restricted and the erosive destructions of joint cartilage were found roentgenographically. Thus, this experimentally induced arthritis proved to have many pathologic features in common with human rheumatoid arthritis.
A rapid and simple procedure for the determination of thiabendazole (TBZ) residue in citrus and apple juices is described. A juice sample is made basic with 2M NaOH and applied to a disposable Extrelut prepacked column. TBZ is eluted with hexane-ethyl acetate (3 + 1) from the column. The eluate is evaporated to dryness under reduced pressure and then dissolved in an internal standard solution. TBZ is monitored without derivatization by capillary gas chromatography with nitrogen-phosphorus detection. The recoveries of TBZ added to fruit juices at 0.05-1.0 micrograms/g were 90-96%. The limit of detection of the method for TBZ was 0.01 micrograms/g. The proposed method is rapid, simple, and sensitive and is applicable to the determination of TBZ in commercial fruit juices.
A sensitive and specific method is described for determination of 5 fasciolicides in milk. The drugs are used to control liver flukes in cattle. The milk sample was homogenized with acetone and acetonitrile, sonicated, and centrifuged. The supernatant was extracted with dichloromethane. The extract was evaporated to dryness, dissolved in 1% sodium hydrogen carbonate, and purified on a C18 cartridge. The 5 drugs were separated from the matrix by reversed-phase liquid chromatography (LC) and determined by dual-electrode coulometric detection on a Kaseisorb LC ODS-300-5 column. The mobile phase was acetonitrile-0.05M potassium dihydrogen phosphate (55 + 45) at pH 3.0. The flow rate was 1 mL/min at 40 degrees C. The applied potentials of detectors 1 and 2 were set at 0.20 and 0.55 V, respectively. The average recovery of the drugs added to milk at 0.01 and 0.1 micrograms/mL was 89.6%, and the coefficient of variation was 4.7%. The detection limits of the drugs in milk were 4-20 ng/mL. The method is used to monitor commercial milk samples and to determine the residual levels of these drugs in milk from cows treated with a fasciolicide.
