Erythrocyte 2,3-diphosphoglycerate in liver diseases.

The erythrocyte 2,3-diphosphoglycerate (2,3-DPG) was studied in patients with liver diseases, chronic obstructive pulmonary disease, and in normal subjects. The level of 2,3-DPG in liver diseases occurred in the following increasing order: chronic persistent hepatitis, chronic active hepatitis, liver cirrhosis, and cirrhosis with hepatocellular carcinoma. A significant negative correlation between the 2,3-DPG concentration and serum albumin concentration was found in the liver diseases. The 2,3-DPG level was correlated to the serum concentration of total bile acids and to the arterial blood pH. A negative correlation was found between the arterial blood pH and the serum albumin concentration. The level of 2,3-DPG in hepatocellular carcinoma and/or liver cirrhosis was higher than that in more hypoxic chronic obstructive pulmonary disease. And an increased level of 2,3-DPG was also shown in nonhypoxic patients with liver diseases. These results suggest that the level of erythrocyte 2,3-DPG increases according to the severity of the liver disease, and compared to the level in hypoxic chronic obstructive pulmonary disease, the level of erythrocyte 2,3-DPG is higher in both hepatocellular carcinoma and liver cirrhosis.

[A case of hepatocellular carcinoma with bone metastasis which responded to oral administration of UFT].

A 61-year-old male was referred to our hospital for evaluation of right upper arm pain. He was diagnosed as having primary hepatocellular carcinoma with bone metastasis by his high titer (111,683 ng/ml) of serum alpha-fetoprotein, computed tomography and abdominal angiography, and so UFT therapy, 400 mg daily, was instituted. After 2 months of this therapy, the titer of serum alpha-fetoprotein gradually decreased. Seven months later, the titer was 3,997 ng/ml and reduction of the hepatic tumor size was shown by computed tomography and ultrasonography. Furthermore, the right upper arm pain diminished and X-ray examination revealed remarkable improvement. During chemotherapy, there was no severe side effect apart from mild anemia and the patient is presently still alive. This case suggests the efficacy of UFT for the treatment of primary hepatocellular carcinoma.

[Liver phosphotyrosine protein phosphatase and tyrosine protein kinase in chemical hepatocarcinogenesis].

We found and characterized three forms of phosphotyrosine protein phosphatase, subsequently designated PTPP-1, -2 and -3, respectively, in rat liver. Chemical hepatocarcinogenesis according to Solt and Farber was accompanied by a slight increase in liver phosphotyrosine protein phosphatase activity and a remarkable increase in liver tyrosine protein kinase activity. A maximum 8-fold increase in tyrosine kinase activity was observed in hepatomas induced with 3'-methyl-4-dimethylaminoazobenzene (MeDAB). Tyrosine protein kinase that increased with the progress of chemical hepatocarcinogenesis was solubilized from the particulate fraction of MeDAB-induced hepatoma. The enzyme was shown to require Mg2+ for its activity, to immunoprecipitate with anti-pp-60src-IgG and to phosphorylate the IgG. Rat liver also contains another tyrosine protein kinase which requires Mn2+ and does not immunoprecipitate with anti-pp60src; the level of this enzyme appears to diminish with the progress of chemical hepatocarcinogenesis.

[The effect of gastrectomy on serum 5-FU concentrations of patients administered UFT per os].

Ten post-total gastrectomy patients and five postsubtotal patients were administered with UFT 3 capsules per os. Then the concentrations of the serums FT-207, 5-FU and Uracil, and that of 5-FU in the lymph-nodes were measured chronologically. The concentration of serum FT-207 reached to its peak one hour after administration of the regimen for both the total and subtotal gastrectomy patients and gradually lowered. The 5-FU serum concentration, on the other hand, showed a rapid drop after reaching a peak one hour after administration of the UFT for both the post-total and subtotal gastrectomy patients. The concentration of 5-FU in the lymph nodes remained five times as high as the serum concentration after four hours for both the post-total and subtotal gastrectomy patients.

Thymosin-alpha 1 increases the capability to produce interleukin-3 but not interleukin-2 in nu/nu mice.

Thymosin-alpha 1 increased the capability of nu/nu mice to produce interleukin (IL)-3 but not IL-2 when administered intraperitoneally at doses of 0.5 and 5.0 micrograms/kg twice a week for 3-6 weeks. Thy-1-positive cells were responsible for the production of IL-3, which was determined by proliferation of the IL-3-dependent cell line (FDC-P2) as well as by augmentation of the in vitro growth of hematopoietic stem cells (colony-forming unit S [CFU-S]). The IL-3 activity released by nu/nu splenocytes emerged in fractions coincidentally with IL-3 released by WEHI-3 cells on Mono Q anion-exchange chromatography. However, IL-2 activity determined by proliferation of the IL-2-dependent cell line was not detected in any fractions of the same chromatography. These findings indicate that IL-2 is not always coproduced with IL-3 by Thy-1-positive cells. This suggests, taken together with results from previous studies about the cells producing IL-2 and IL-3 by other investigators, that thymosin-alpha 1 exerts its effect at an early stage of T cell differentiation to induce a T cell subpopulation capable of producing IL-3 (but not yet IL-2). Based on the present findings, we discuss the mechanism of action by which thymosin-alpha 1 exerts its diverse effects in immunosuppressed hosts but not a potentiating effect in healthy normal animals or in in vitro assays of T cell function.

[Thallium-201 scintigraphy of the myocardium during exertion in the indication for aortocoronary bypass of an area in the myocardium, the site of an infarct].

The purpose of this study is to precisely evaluate the indications and the results of direct revascularization surgery in a myocardial area previously infarcted. 35 cases were analyzed using thallium scintigraphy during stress. Before surgery, the patients were divided in two groups. Group I (18 cases) includes patients who presented initially a regional hypoperfusion and a reperfusion at the site of the infarction, on late scintigrams; group II (17 cases) also presented a regional hypoperfusion initially, but showed no reperfusion on late scintigrams. Concerning the results during stress, the thallium 201 scintigrams during exercise show alterations of the myocardial perfusion in the infarcted area, before and after surgery; in group I, a significant improvement (p less than 0.01) is noted in the entire group, i.e. 16 patients (88.9%), except in 2 cases where the graft was occluded, and group II shows an improvement in only 4 cases (NS) after surgery. Concerning the modifications of the product pressure X cardiac rhythm, there is a significant improvement (p less than 0.01) in group I (14 out of 18 patients (77.8%)); 2 out of 4 patients who were aggravated, presented occluded grafts after surgery. Group II shows an improvement in 10 out of 17 patients (58.8%) after surgery (NS). The left ventriculography demonstrates alterations in the global ejection fraction. Group I shows a significant improvement in 15 out of 18 patients (88.3%) (p 0.01); in group II there is improvement in 11 out of 17 patients (64.7%) (NS) after surgery.(ABSTRACT TRUNCATED AT 250 WORDS)

[Effects of danazol on serum lipid and lipoprotein levels in patients with endometriosis].

Danazol was given to 16 patients with endometriosis in daily doses of 200 to 400mg for 4 to 6 months, and the change in serum lipids and lipoproteins was analysed. Danazol decreased total cholesterol by an average of 22%, triglycerides by 56%, phospholipids by 29%, free fatty acids by 17% and lipid peroxides by 42%, among which reductions in total cholesterol (p less than 0.05) and lipid peroxides (p less than 0.01) were statistically significant when compared with the pretreatment levels. Levels of lipids in HDL and VLDL also fell: HDL-cholesterol by 43%, HDL-triglycerides by 68%, HDL-phospholipids by 40%, VLDL-cholesterol by 73%, VLDL-triglycerides by 71% and VLDL-phospholipids by 48%. The decrease in HDL-cholesterol (p less than 0.05), HDL-triglycerides (p less than 0.05) and HDL-phospholipids (p less than 0.001) was significant. Concurrently apolipoproteins and LCAT activity decreased: A-I by 28% (p less than 0.001), A-II by 7% (p less than 0.05) and LCAT by 35%. In contrast, LDL-cholesterol increased, though slightly. Therefore, the atherogenic index defined as the ratio of LDL-c/HDL-c rose as much as 98% of the initial value. These changes seem to have arisen from impairment in the VLDL-HDL cycle. The decrease in VLDL and increase in LDL indicated that VLDL was catabolised and converted to LDL at a normal rate while the formation of HDL was impaired probably due to the low activity of LCAT and low level of apo A-I and A-II.(ABSTRACT TRUNCATED AT 250 WORDS)
