We measured pressures of the papillary sphincter zone and pancreatic main duct endoscopically in 20 control subjects and in 38 patients with chronic pancreatitis without papillary stenosis, including 19 cases believed to be in the early stages of chronic pancreatitis. There was no significant difference between control subjects and patients with chronic pancreatitis without papillary stenosis in the motility of the sphincter of Oddi. The pancreatic main ductal pressure was significantly higher in the patients in the early stage of chronic pancreatitis (35.7 +/- 9.1 mm Hg (mean +/- SD) in patients suspected of having chronic pancreatitis and 44.9 +/- 20.4 mm Hg in minimal-change chronic pancreatitis) than in the controls (16.2 +/- 8.7 mm Hg). The viscosity of pure pancreatic juice of patients with chronic pancreatitis (5.0 centi-Poise (cP] in the basal secretory phase was significantly higher than that of the controls (1.32 cP). These data suggest that endoscopic measurement of pancreatic ductal pressure is useful for diagnosis of the early stages of chronic pancreatitis.
The urinary level of pseudouridine, primarily a degradation product of transfer ribonucleic acid (tRNA), was determined in 38 patients with primary hepatocellular carcinoma, 18 with liver cirrhosis, 12 with chronic hepatitis, nine with acute hepatitis, and 28 healthy subjects. The mean urinary pseudouridine concentration was significantly higher in the patients with hepatoma [38.2 +/- 12.8 (SD) nmol/mumol creatinine] than in those with liver cirrhosis (20.3 +/- 6.8), chronic hepatitis (24.4 +/- 8.2), and acute hepatitis (21.7 +/- 8.2), and in healthy subjects (23.8 +/- 4.9). Urinary pseudouridine level was elevated above the mean value plus 2 SD for the healthy subjects (33.6 nmol/mumol creatinine) in 71% of our hepatoma cases. Serum alpha-fetoprotein levels correlated poorly with urinary pseudouridine levels, thus, the combination assay for urinary pseudouridine and serum alpha-fetoprotein could detect 79% of the patients with hepatoma. Moreover, urinary pseudouridine level was reduced after effective transcatheter arterial embolization therapy.
We treated a 62-year-old woman with adenomyoepithelioma of the breast, an extremely rare tumor, which showed a bicellular pattern of ductal and myoepithelial elements bearing some histologic resemblance to pleomorphic adenoma of the salivary gland. The tumor was made up of cells positive for actin and S-100 protein, and some positive for epithelial membrane antigen (EMA). Thus, this tumor showed biphasic differentiation towards myoepithelial cells and to duct epithelial cells. Compared with findings in related tumors reported in the literature, the myoepithelial cells in the present tumor were less frequently spindle-shaped and had abundant eosinophilic or clear cytoplasm. A simple mastectomy was done and at the time of writing 20 months later there has been no recurrence. Adjuvant chemotherapy was not prescribed.

An experiment to gauge the effect of OK-432 on UFT activation under a combined administration of UFT and OK-432 at a therapeutic dose has been pursued clinically by determining the blood and intratumoral 5-FU concentrations after administration of UFT. UFT was administered orally to a group of AH-130-bearing rats at a dose of 12 mg/kg or 8 mg/kg. In another group that was given combined treatment with OK-432, the OK-432 was administered subcutaneously at a dose of 0.1 KE/kg together with UFT. The intratumoral 5-FU concentration at 4 hours after UFT 12 mg/kg administration showed higher values in th single UFT treated group than in the combined treated group, but at 8 hours no significant difference was found between these two groups as the concentrations were 0.133 +/- 0.027 microgram/g and 0.128 +/- 0.021 microgram/g, respectively. Further, no significant differences were noticed in the blood and intratumoral 5-FU concentrations between both groups after administration of UFT at a dose of 8 mg/kg. Clinically, the blood FT-207 and 5-FU concentrations after UFT administration showed no significant difference between the single UFT treated group and the combined OK-432 treated group, nor was there a significant difference in the intratumoral concentration. In summary, it is considered that OK-432 has little effect on UFT activation when UFT and OK-432 are administered in combination.
Aiming at establishing an appropriate lymph node dissection for carcinoma of the remnant stomach and of the lower esophageal carcinoma in the status post-gastrectomy, lymphatic flow was investigated clinically as well as experimentally. Nineteen cases of carcinoma of the remnant stomach and 8 cases of esophageal carcinoma after partial gastrectomy were studied. Lymph node metastasis of the remnant stomach carcinoma were more frequently seen at perigastrium, splenic hilum, and along splenic artery. Those further extended to para-aortic and diaphragmatic nodes. Three cases of lower esophageal carcinoma after gastrectomy had massive nodal involvement at perigastrium, as well as intra-thoracic lymph nodes. Experimentally 5-Fu emulsion was injected submucosally under endoscope in 25 dogs and subserosally in 6 rabbits. 5-Fu contents in lymph nodes were measured 30 minutes after injection. The most prominent difference in lymphatic flow from the remnant stomach was increase in ascending flow into intrathoracic lymph nodes through para-aorta. This increment was seen irrespective of Billroth I or II anastomosis. On the contrary, descending lymphatic flow from the lower esophagus into the intra-abdominal lymph nodes was not disturbed by gastrectomy. Cardiac lymph node dissection in rabbits accelerated ascending flow. Those results would indicate the necessity of complete block of ascending flow in cases of the remnant stomach carcinoma and of intra-abdominal lymph node dissection in those of the lower esophageal carcinoma after gastrectomy.

A 33-year-old man with known annuloaortic ectasia was admitted to the hospital because of chest pain. A dissecting aortic aneurysm was strongly suspected, but angiography was not performed because of his history of allergic reaction to iodine contrast material. Two-dimensional echocardiography demonstrated a markedly dilated basal aorta. The aortic wall immediately above the right sinus of Valsalva appeared to divide the aortic lumen into anterior and posterior channels. A defect in the intimal flap with a diameter of two cm was also detected six cm above the aortic ring. Color Doppler flow mapping showed blood flow passing through a probable entrance tear from the true lumen into the false lumen. Reentry could not be detected by Doppler imaging. All major aortic tributaries were presumed patent and supplied via the true lumen. The patient underwent successful emergency Bentall surgical procedure and recovered. It is emphasized that in some cases of DeBakey type I or II aortic dissection, color Doppler flow imaging is helpful for determining indications for emergency surgery.
We endoscopically measured pressures of the pancreatic duct (PP) and the sphincter of Oddi (SO) in patients with alcoholic (ALCP, n = 10), gallstone-associated (GSCP, n = 7), and idiopathic chronic pancreatitis (ICP, n = 21), and in 20 controls. The PP was significantly higher in the patients with ALCP (55.7 +/- 28.9 mm Hg), GSCP (33.6 +/- 16.2 mm Hg), or ICP (44.5 +/- 25.8 mm Hg) than in the controls (16.2 +/- 8.7 mm Hg), but there was no significant difference between ALCP, GSCP, and ICP. There was no significant difference between control subjects and ICP in the motility of SO. In ICP, there was no correlation between the PP and the motility of SO. In ALCP and GSCP, the frequencies of the papillary sphincter waves were significantly higher than in normal subjects, and there were correlations between the PP and the motility of SO. These data suggest that increased pancreatic ductal pressure in GSCP with papillitis or ALCP may be due in part to papillary dysfunction, but not in ICP.
This paper reports on an autopsied case manifesting an esophageal cancer that had developed 13 years after radiotherapy for lung cancer. The patient was a 61-year-old man. He was found to have a squamous cell carcinoma of the right lower bronchus with a swelling of the mediastinal and left supraclavicular lymph nodes in July of 1973. He received 60 Gy of irradiation in the right lung, the mediastinum, and the left supraclavicular region. Later, after doing well until August of 1986, a squamous cell carcinoma of the esophagus was found at the upper intrathoracic site. Thus, he also received additional radiotherapy but died of pneumonia after this local recurrence 7 months later. At autopsy, no local recurrence of the primary lung cancer was found. The site of esophageal cancer was far from that of the primary lung cancer though it was included in the previous treatment ports. This suggests the possibility that the primary esophageal cancer had been induced by therapeutic irradiation. So far as we know, this is the first report of esophageal cancer that may have developed after irradiation for lung cancer.
Four instances (8%) of inflammatory fibroid polyp (IFP) with concomitant adenocarcinoma or adenoma, in the same area, were noted among 50 cases of IFP of the stomach. Adding two cases from other sources, four cases of gastric IFP concomitant with an adenocarcinoma and two of gastric IFP concomitant with an adenoma were studied histopathologically and immunohistochemically. All lesions were located in the gastric antrum and they were restricted to the mucosa in three, and they involved both mucosa and submucosa in the other three. Neither S100 protein nor factor VIII-associated antigen was recognized in the principal component cells, using an immunoperoxidase technique. This finding suggests the conventional view that the proliferating cells were neither neurogenic nor angioblastic, but rather, were fibroblastic in origin. The four concomitant carcinomas were early adenocarcinomas restricted to within the mucosa, and the two concomitant adenomas were tubular adenomas with a moderate epithelial atypia. All these neoplasms were present in or adjacent to the IFP. We emphasize that the IFP, albeit benign, may carry an adenocarcinoma or an adenoma.
To study the modulation of the reductive metabolism of halothane (2-bromo-2-chloro-1,1,1-trifluoroethane) by microsomal cytochrome b5, formation of 2-chloro-1,1,1-trifluoroethane (CTE) and 2-chloro-1,1-difluoroethylene (CDE), major reduced metabolites of halothane, was analyzed in vivo and in vitro. Rats were pretreated with both malotilate (diisopropyl-1,3-dithiol-2-ylidenemalonate) and sodium phenobarbital (malotilate-treated rats) or only with sodium phenobarbital (control rats). The microsomes of malotilate-treated rats had significantly more cytochrome b5 than the controls, whereas the cytochrome P-450 content was not different between the two groups. At the end of 2-h exposure to 1% halothane in 14% oxygen, the ratio of CDE to CTE in arterial blood was significantly higher in malotilate-treated rats than in the controls. Under anaerobic conditions, the formation of CDE and the ratio of CDE to CTE were significantly greater in microsomal preparations of malotilate-treated rats than those of the controls. In a reconstituted system containing cytochrome P-450PB purified from rabbit liver, addition of cytochrome b5 to the system enhanced the formation of CDE and increased the ratio of CDE to CTE. These results suggested that cytochrome b5 enhances the formation ratio of CDE to CTE by stimulating the supply of a second electron to cytochrome P-450, which might reduce radical reactions in the reductive metabolism of halothane.
This article documents a patient with lung carcinoma that produced three oncofetal antigens including alpha-fetoprotein (AFP), carcinoembryonic antigen (CEA), and human chorionic gonadotropin (hCG). Serum AFP, CEA, and hCG-beta-subunit were extremely high--118,000 ng/ml, 133 ng/ml and 0.9 ng/ml, respectively. Immunohistochemical staining of these tumor markers revealed that these proteins were present in different cells. The pattern of lectin affinity electrophoresis of AFP resembled that of hepatocellular carcinoma. Also investigated was the reactivity of serum CEA to monoclonal antibodies against peptide or sugar moieties. Serum CEA values measured by antipeptide monoclonal antibodies were higher than those measured by antisugar monoclonal antibodies. The demonstration of AFP, CEA, and hCG in different tumor cells suggests that three genomes were not reactivated together in a cell, and the lung carcinoma probably consisted of at least three clones of cancer cells with different phenotypes.
The serum level of pseudouridine, primarily a degradation product of tRNA, was determined by high-performance liquid chromatography in 24 patients with small cell lung cancer (SCLC), 13 patients with non-SCLC with advanced stages, 15 patients with pulmonary infectious diseases, and 18 healthy controls. The mean serum pseudouridine concentration was significantly higher in the patients with SCLC [4.75 +/- 1.76 (SD) nmol/ml] than that in the patients with pulmonary infectious diseases (3.39 +/- 1.38 nmol/ml) or in healthy controls (2.21 +/- 0.78 nmol/ml). The mean serum pseudouridine concentration in the patients with non-SCLC (4.07 +/- 0.95 nmol/ml) was significantly higher than that in healthy controls but not statistically different from that in the patients with pulmonary infectious diseases. The serum pseudouridine level was elevated above the mean value plus 2 SD for the healthy subjects (3.77 nmol/ml) in 66.7% of all patients with SCLC including 3 of 8 (37.5%) with limited disease and 13 of 16 (81.3%) with extensive disease, and 53.8% of the patients with non-SCLC. Serum carcinoembryonic antigen was elevated (greater than 5 ng/ml) in 29.2% and serum neuron-specific enolase (greater than 10 ng/ml) in 58.3% of the cases with SCLC. In the patients with SCLC followed up during chemotherapy, serum pseudouridine levels changed considerably parallel with the changes in the clinical response. These findings indicate that serum pseudouridine may be a useful biochemical marker in the patients with SCLC.
Possible presynaptic mechanisms involved in the naloxone-precipitated withdrawal jumping in morphine-dependent mice were studied. The withdrawal jumping precipitated by intrathecally (i.t.) administered naloxone (10 nmol) was abolished by the i.t. pretreatment with a substance P antagonist, [D-Pro4,D-Trp7,9,10]-substance P4-11 (10 nmol). The high KCl-evoked 45Ca2+ entry into synaptosomes and the release of immunoreactive substance P from slices were significantly enhanced by 10(-4) M naloxone in the morphine pelleted spinal cord, but not in the vehicle pelleted ones. These findings suggest that naloxone activates the depolarization-evoked Ca2+ entry into the nerve terminals, enhances the release of substance P from the spinal cord and the induction of withdrawal jumping in morphine-dependent mice then follows.
