A case of macrocystic serous cystadenoma of the pancreas.

A case of macrocystic serous cystadenoma of the pancreas is presented, and literature is reviewed. A 35-yr-old woman presented with mild upper abdominal pain. Abdominal ultrasonography and an abdominal computed tomography revealed a multiloculated and calcified cyst in the body of the pancreas. A T1-weighted image, using magnetic resonance imaging, revealed a low-intensity multiloculated, pancreatic mass. In contrast, T2-imaging of the tumor showed a high-intensity mass. Endoscopic retrograde cholangiopancreatography showed no contact between the main pancreatic duct and the tumor. The preoperative diagnosis was a mucinous cystic neoplasm. Tumor enucleation was performed. Subsequent microscopic examination of this tumor suggested the diagnosis of a macrocystic serous cystadenoma of the pancreas.

[A case of myelodysplastic syndrome who died of septic pulmonary embolism].

A case of myelodysplastic syndrome (MDS) complicated by septic pulmonary embolism is reported. A 61-year-old female who had been followed for refractory anemia with excess of blasts suddenly died of acute respiratory failure. An autopsy revealed massive pulmonary emboli with gram-positive cocci gathered in the emboli and alveolar spaces. Staphylococcus aureus was also detected through a blood culture from the right atrium. We speculate that pulmonary embolism was the result of septicemia induced by the immunosuppressive condition associated with MDS.

Biosynthesis of acetylenic compounds in cultured cells of Asparagus officinalis from [1-13C]- and [U-13C] Glucose.

The biosynthetic pathway of phenolics with an acetylenic group, 4-[5-(4-methoxyphenoxy)-3-penten-1-ynyl]phenol (1) and its related compounds, isolated from suspension-cultured cells of Asparagus officinalis L. (Liliaceae), was studied in feeding experiments using [1-13C]- and [U-13C] glucose. Analyses of the 13C-NMR spectra of the methyl ether and acetate of the 13C-labelled species of 1 indicated that the aromatic rings at both ends of the molecule are formed from shikimic acid. It was also assumed that the acetylenic carbons in the C5 chain could be incorporated together with its adjacent aromatic ring through any phenethyl class compound derived from phenylpropanoids, and the propenylene carbon through glycolysis metabolites with C3 carbon units.

Augmentation of tumor immunity by 6-mercaptopurine (6-MP) and its analogs in the double grafted tumor system in mice.

We investigated the antitumor effect of 6-mercaptopurine (6-MP) and its analogs using the double grafted tumor technique. BALB/c mice were inoculated intradermally with MethA fibrosarcoma cells at the right inguinal region on day 0 (the primary tumor) and at the left on day 10 (the secondary tumor). Intraperitoneal or intra-lesional administration of 6-MP, 6-mercaptopurine riboside (6-MP-r) and 6-mercaptopurine riboside triacetate (6-MPRTA) from day 3 to 7 dose-dependently inhibited growth of the secondary tumor. Without the primary inoculation, 6-MP showed no effect on growth of the tumor inoculated on day 10, indicating that the antitumor effect of 6-MP could not be attributable to its direct antimetabolic or tumoricidal action only, and that the primary tumor inoculation is necessary for these compounds to inhibits growth of the challenging tumor. 6-MP did not inhibit the secondary MethA growth in the BALB/c (nu/nu) mouse. Both CD4+ and CD8+ T cells increased in the spleen of mice treated with 6-MP. Meanwhile, delayed-type hypersensitivity (DTH) reaction to the methylated bovine serum albumin (MBSA) antigen at the footpad was not augmented but inhibited by 6-MP-r and 6-MPRTA in both normal and tumor-bearing mice. Thus, the immunomodulatory activity of 6-MP could be observed in two opposite directions, augmentation of tumor immunity and inhibition of DTH to MBSA. This indicates that the immune mechanism and/or the type of effector cells induced in these two cell-mediated immune systems are different from each other.

Acetylenic compounds isolated from cultured cells of Asparagus officinalis.

Three new acetylenic compounds, compounds I, II and III were isolated from the cultured cells of Asparagus officinalis L. (Liliaceae) and their structures identified as 1-methoxy-4-[5-(4-methoxyphenoxy)-3-penten-1-ynyl]-benzene, 4-[5-(4-methoxyphenoxy)-3-penten-1-ynyl]phenol and 4-[5-(4-hydroxyphenoxy)-3-penten-1-ynyl]phenol, respectively, from chemical and spectral analysis.

The hypocholesterolemic action of TA-7552 and its effects on cholesterol metabolism in the rat.

The hypocholesterolemic property of 1-(3,4-dimethoxyphenyl)-2,3- bis(methoxycarbonyl)-4-hydroxy-6,7,8-trimethoxynaphthalene (TA-7552) and its effects on cholesterol metabolism were investigated in the rat. TA-7552 incorporated into a hypercholesterolemic diet at a concentration of 0.2% and administered for 7 days reduced serum cholesterol by 72% and liver cholesterol by 90%, and its minimal effective dose was 0.01% in the diet. Its hypocholesterolemic effect was associated with an elevation of serum HDL-cholesterol. Inclusion of 0.1% TA-7552 in the normal laboratory chow accelerated fecal excretion of 14C derived from orally administered 4-[14C]cholesterol or carbonyl-[14C]taurocholate. The net amounts of fecal neutral sterols and bile acids were markedly increased by the same treatment. Hepatic bile acid production and hepatic and intestinal cholesterol biosynthesis as measured by cholesterol 7 alpha-hydroxylase activity and 1-[14C]acetate incorporation into tissue cholesterol, respectively, were both stimulated by the drug treatment. All these data indicate that this hypocholesterolemic agent inhibits intestinal absorption of both cholesterol and bile acids and compensatorily stimulates hepatic production of bile acids and cholesterol.

Primary adenocarcinoma of the vermiform appendix associated with transitional cell carcinoma of the bladder.

A case of primary adenocarcinoma of the vermiform appendix coexisting with transitional cell carcinoma (TCC) of the bladder is presented. The patient was a 74-year-old male who underwent radical cystectomy with continent urinary diversion using the Florida pouch. Two different cancers were found, TCC of the bladder and mucinous adenocarcinoma of the appendix. To our knowledge, this is the only such case reported in the medical literature.

[An adult case of congenital esophagobronchial fistula complicated with pyothorax].

A 64-year-old woman with congenital esophagobronchial fistula complicated with pyothorax is reported. She was admitted to our hospital because of high fever and cough. Chest X-ray showed a cavity with niveau in the right lower lobe. The next day she noticed right chest pain. Chest X-ray showed right pleural effusion. The diagnosis of left esophagobronchial fistula with pyothorax was confirmed by esophagography, esophagoscopy, chest X-ray and chest CT. After treatment of the pyothorax with antibiotics and drainage, fistulectomy and right lower lobectomy were performed. The postoperative course was uneventful. Histologically, the fistula had esophageal epithelium with a submucosal muscle layer and demonstrated a transitional zone between squamous epithelium and bronchial epithelium.
