Maxi K+ channel in apical membrane of vestibular dark cells.

Recordings were made on excised apical membrane patches from vestibular dark cells from the semicircular canal of gerbils to determine if ion channels could be involved in the process of K+ secretion. Both nonselective cation channels [Am. J. Physiol. 262 (Cell Physiol. 31): C1430-C1436, 1992] and K(+)-selective channels were found. The K+ channels occurred in only 0.7% of the patches. In symmetrical 145 mM KCl solutions, the current-voltage (I-V) relation of the K(+)-selective channel was linear, indicating the absence of rectification, and the conductance was 240 +/- 8 pS (n = 8). The Goldman-Hodgkin-Katz equation for current carried solely by K+ could be fitted to the I-V relation in asymmetrical K+ and Na+ solutions and yielded a K+ permeability of 5.78 x 10(-13) cm3/s (n = 12). The channel was shown to be impermeable to Li+, NH4+, N-methyl-D-glucamine, and Cl-. Channel activity increased with depolarization and with increasing free [Ca2+]; for voltages between +40 and -60 mV, the strongest regulation occurred in the range 10(-6) to 10(-5) M Ca2+. Tetraethylammonium (2 x 10(-2) M) had from the cytosolic side no effect on the open probability (Po) but completely inhibited activity from the extracellular side. Po was reduced by Ba2+ (5 x 10(-3) M), verapamil (10(-4) M), quinine (10(-4) M), and quinidine (10(-4) and 10(-3) M), while lidocaine (5 x 10(-3) M) had no measurable effect on Po but decreased the amplitude. Rb+ and Cs+ were either poorly permeable or partially blocked the channel in a voltage-dependent manner.(ABSTRACT TRUNCATED AT 250 WORDS)

Stimulation of Cl secretion by lactoferrin across canine airway epithelial cells in culture.

To investigate the effect of lactoferrin, an iron-binding glycoprotein in the respiratory tract, on ion transport function of airway epithelial cells, we measured bioelectric properties of canine cultured tracheal epithelium by Ussing's short-circuit technique. Addition of lactoferrin (60 micrograms/ml) to the mucosal side of epithelial sheet increased the short-circuit current (Isc) from 4.8 +/- 0.8 to 7.5 +/- 1.1 microA/cm2, and the submucosal addition likewise increased Isc from 3.8 +/- 0.6 to 5.6 +/- 0.7 microA/cm2 (p < 0.001, in each case). This effect was concentration dependent. The lactoferrin-induced increase in Isc was not altered by amiloride, indomethacin, or propranolol but was abolished by diphenylamine-2-carboxylate or substitution of Cl with iodide in the medium. Intracellular cyclic AMP levels were not increased by lactoferrin. These results suggest that lactoferrin may selectively stimulate Cl secretion across the airway mucosa, an effect that may not be dependent on prostaglandins, beta-adrenergic receptor or intracellular cyclic AMP.

Cerebral arteriovenous malformations associated with a straight sinus anomaly.

Nine cerebral arteriovenous malformations (AVM) associated with straight sinus anomaly were demonstrated on angiograms. All 10 AVM (1 patient had two AVM) occupied deep cerebral structures: Seven were totally or partially located in the basal ganglia or the corpus callosum, and 3 were in the medial occipital or temporal lobe. In 6 patients, almost no filling of the straight sinus was observed, although the AVM were located deeply and the main drainage was via the vein of Galen. All 6 patients showed retrograde flow in the deep cerebral veins or venous drainage through a persistent facial sinus. The other 3 patients had duplication or septal formation of the straight sinus. Straight sinus anomaly is extremely rare in the normal population but seems not uncommon in patients with deep-seated AVM.

Homeostasis as regulated by activated macrophage. IV. Analgesic effect of LPSw, a lipopolysaccharide of wheat flour.

The effect of LPSw, a lipopolysaccharide from a water extract of wheat flour, on pain response was investigated using an acetic acid-induced writhing test in mice. LPSw inhibited writhing dose-dependently in the range of 10 ng-10 micrograms/mouse i.v. This effect reached its maximum 1.5-3 h after the LPSw inoculation and was detectable even after 8 h. The analgesic effect of LPSw was inhibited by i.v. injection of naloxone and also beta-endorphin was detected in serum and brain tissue following injection of LPSw. Preliminary clinical trials were done in which LPSw was administered percutaneously to relieve the pain of patients with herpes. The results showed that pain was relieved by this application. LPSw may be the best analgesic drug so far known, since it induces the endogenous mediator of analgesia, beta-endorphin.

PTA of supra-aortic arteries with temporary balloon occlusion to avoid distal embolism.

Percutaneous transluminal angioplasty (PTA) was carried out in eight patients with cervical arterial stenosis; six in the subclavian and brachiocephalic arteries (5 with subclavian steal syndrome), one in the common carotid artery, and one in both the brachiocephalic and common carotid arteries (with subclavian steal syndrome). The PTA balloon catheters were introduced via the femoral artery in seven and brachial artery in one. To prevent distal embolization through the vertebral and internal carotid arteries, the blood flow in these vessels was temporarily occluded with a balloon catheter. The dilation of the stenotic areas was generally satisfactory. Antegrade blood flow was promptly obtained in the vertebral artery even in patients with subclavian steal syndrome. In all patients, the clinical symptoms improved. Two patients underwent repeat PTA because of restenosis.

Acute subdural hematoma in young patient with moyamoya disease--case report.

A 17-year-old boy with known moyamoya disease developed an acute subdural hematoma after a mild head trauma. He had been confined to a wheelchair with contracture in the upper and lower extremities due to juvenile rheumatoid arthritis since age 1 year. He had undergone encephalo-duro-arterio-synangiosis (EDAS) on the right and encephalo-myo-synangiosis (EMS) on the left at 13 years of age. He was admitted with headache, nausea, and vomiting after a fall from his wheelchair at age 17. Computed tomography on admission showed a large acute subdural hematoma in the right fronto-temporal region but no bleeding at the EDAS or EMS sites. Cerebral angiography 12 weeks after the head trauma revealed a remarkable reduction in the spontaneous transdural external-internal carotid anastomoses in the right frontal region. The acute subdural hematoma was probably caused by rupture of the spontaneous transdural anastomoses.

[Differentiation and maturation of excurrent duct system of rat testes].

The present study was designed to investigate relations of gamma-Glutamyl transpeptidase (gamma-GTP) activities to morphological differentiation and maturation of rat testicular excurrent duct system including rete testis, efferent ductuli and epididymal ducts. Animals used were Wistar rats aged from 3 days to 12 weeks. Histochemical demonstration of gamma-GTP activities was carried out by the method of Rutenburg et al. (1969). Morphological differentiation of the epithelium and structure of the rete testis occurred between 2 and 3 weeks and completed by the age of 6 weeks, which was accompanied by induction and deletion of gamma-GTP activity. Morphological differentiation and maturation of the epithelium of the efferent ductuli and the epididymal ducts became evident by the age of 4 weeks and gamma-GTP was active in both epithelial cells throughout the period examined. Since the maturation of rete testis epithelium was found to proceed as observed as Sertoli cells, the origin and functions of the rete testis epithelium and Sertoli cells are considered to be identical. The simultaneous development of the efferent ductuli and the epididymal ducts suggests that both tissues originate from mesonephric ducts and have similar functions, although mature epithelial cells of both tissues are completely different.

[Usefulness of magnetic resonance imaging (MRI) in the detection of the lesions of gestational trophoblastic disease--comparison with computed tomography and digital subtraction angiography].

Twenty patients with gestational trophoblastic disease (GTN) were examined by Magnetic Resonance Imaging (MRI), Computed Tomography (CT) and Digital Subtraction Angiography (DSA), to evaluate their usefulness in the diagnosis of the disease. The lesions of hydatidiform mole were mainly composed of molar vesicles, dilated vessels and hemorrhage which were depicted as small round high intensity lesions on the T2-weighted images and as tree-like low intensity lesions and high or low intensity lesions of various shapes in the T1-, T2-weighted images. These MRI findings closely corresponded to the histopathological findings. On the other hand, CT findings obtained with hydatidiform mole were characterized by filling defects or a small round low density area on contrast enhanced images. The detection ratio for intramural lesions of invasive mole and choriocarcinoma by MRI was 83% (5/6), while that by CT was 50% (3/6). The obliteration of the junctional zone and interruption of the myometrium observed in MRI were significant signs suggesting intramural invasion of the disease. In fact, these signs in MRI were observed in all of the six cases with invasive mole or choriocarcinoma examined. In conclusion, MRI is a powerful means for the determining the intramural invasive mole and choriocarcinoma. Thus more accurate diagnosis of GTN will be obtained with the combined use of MRI and DSA.

[Airway epithelial beta 3-adrenergic receptor--effect on bioelectric properties and its mechanism of action].

To characterize the "atypical" beta-adrenergic receptor (beta 3-adrenergic receptor) and its action on ion transport across airway mucosa, we measured the bioelectric properties of canine cultured tracheal epithelium under short-circuited conditions in vitro. Submucosal but mucosal addition of BRL37344, a selective beta 3-adrenergic agonist, increased short-circuit current (Isc) in a dose-dependent fashion, the EC50 value being 30 fold higher than that of isoproterenol. This effect on Isc was accompanied by the accumulation of intracellular cyclic AMP, and it was abolished by diphenylamine-2-carboxylate, bumetanide, and Cl-free medium, but not by amiloride. Pretreatment of cell with beta 1- and beta 2-adrenergic antagonists greatly reduced the Isc response to isoproterenol, whereas it had little effect on the BRL37344-induced response. In addition, the increase in Isc produced by BRL37344 was competitively antagonized by cyanopindolol, but pA2 was significantly different from the case of isoproterenol. These results suggest that beta 3-adrenergic receptors exist on airway epithelium, and may stimulate Cl secretion across the airway mucosa via accumulation of intracellular cyclic AMP.

Papillary cystoadenocarcinoma of the prostate.

We report a papillary cystadenocarcinoma of the prostate found in a 66-year-old man presenting with gross hematuria. The tumor was identified as prostatic in origin because of positive immunohistochemical staining of prostate specific antigen.

[Clinical assessment of urinary free L-fucose levels].

We measured urinary levels of free L-fucose in healthy subjects, patients with benign diseases, and patients with cancer using an automated analyzer and a newly isolated L-fucose dehydrogenase, and evaluated the clinical usefulness of the results. The values obtained were corrected for urinary creatinine as micromoles per gram of creatinine. The cutoff value, set at the mean + 2SD for the healthy subjects, was 250 mumol/g.Cr. Patients with gallbladder cancer, bile-duct cancer, liver cancer, pancreatic cancer, or cirrhosis of the liver had significantly higher levels of L-fucose than the healthy subjects. The diagnostic sensitivity for these five diseases, taken together, was 68% (144/213). Specificity for the detection of cancer was calculated by use of false positives for patients with cholelithiasis, hepatitis, and pancreatitis: it was 73% (76/104). Diagnostic accuracy for these seven diseases taken together was therefore 69% (220/317). We compared the positive ratio of the L-fucose level with that of the tumor markers AFD and CA19-9. The positive ratio of an L-fucose value above the cutoff was higher than the positive ratio of either marker in bile-duct cancer, gallbladder cancer, liver cancer, and pancreatic cancer. The results suggested that the urinary levels of free L-fucose reflected the metabolism of sugar chains of glycoconjugates, and may be usefully clinically as a tumor marker.

[A report of two cases of multilocular cystic renal cell carcinoma: review of 51 cases reported and the results of a prognostic survey].

We report 2 cases of multilocular cystic renal cell carcinoma. One was in a 33-year-old male, presenting with ultrasonic abnormality of the left kidney at an annual employee health care examination. Computerized tomography (CT) demonstrated a 5 cm of multilocular cystic mass adjacent to the lower pole of the left kidney. Another was in a 44-year-old male, presenting with microscopic hematuria at an annual employee health care examination. CT of the abdomen revealed a 6.5 cm of multilocular cystic mass on the upper pole of the right kidney. Both were diagnosed as renal cell carcinoma by the angiography and underwent radical nephrectomy. Gross specimens showed typical multilocular cystic appearance and histopathology showed clear cell carcinoma infiltrating septa and replacing epithelium of the cyst walls. Both patients are alive without evidence of disease at, 21 months and 14 months after operation, respectively. Including our cases, 51 multilocular cystic renal cell carcinoma and multilocular cystic nephroma associated with renal cell carcinoma have been reported. From the review of the literatures and the answer of the questionnaires inquiring about the outcome of the patient to Japanese reporters, the outcome of 38 patients was ascertained. The 10-year survival rates and non-recurrence rate after operation calculated by the Kaphan-Meier formula were 97.3% and 90.3%, respectively. Because of the good prognosis of reported cases, we concluded that we should choose kidney-sparing surgery for the operation of multilocular cystic renal cell carcinoma.

[Two cases of diffuse panbronchiolitis receiving long-term erythromycin (EM) therapy with acute exacerbation due to EM-resistant pneumococcus].

The first case was a 73-year-old woman with chief complaints of fever, cough, purulent sputum and dyspnea. EM therapy was begun in December 1983 due to a diagnosis of diffuse panbronchiolitis (DPB). Subsequently, P. aeruginosa was persistently detected, while in February 1991 at the time of an acute exacerbation of the DPB P. aeruginosa and S. pneumoniae were detected by TTA. The second case was a 65-year-old man with chief complaints of fever, cough and purulent sputum. DPB was diagnosed and EM therapy was begun in December 1985. In January 1991, pneumonia developed, at the time when S. pneumoniae was detected by TTA. In both cases, rapid disappearance of S. pneumoniae from the sputum and alleviation of symptoms were obtained with carbapenem antibiotic administration. Both strains were resistant to EM, Tetracycline (TC), Minocycline (MINO) and Clindamycin (CLDM). Particularly, S. pneumoniae of case 2 showed low sensitivity to Ampicillin (ABPC), Cefotiam (CTM) and Cefoxitin (CFX) as well. These cases showed acute exacerbations due to EM-resistant pneumococcus during long-term therapy with EM, and are of interest in that they may shed light on the relation between long-term EM therapy and the emergence of resistant pneumococcus.

[Attempt for modified radical mastectomy in the treatment of early breast cancer].

Until five years ago the Aichi Cancer Center had adopted the standard radical mastectomy where breast, major and minor pectoral muscles, and axillary adipose tissues were resected en bloc. In case tumor was found at the inner half or the center of the breast, parasternal lymph node dissection was concomitantly carried out. Recently, however, we begin to perform Auchincloss by which major and minor pectoral muscles can be conserved, as an attempt for modified radical mastectomy. In some cases, furthermore, breast conserving operation is made. The reasons why we changed the operation methods are: 1) Due to the advancement of diagnostic imaging and widespread self examination, percentage of early breast cancer has considerably increased. 2) Postoperative adjuvant therapy, especially adjuvant endocrine therapy, has remarkably progressed.

[Efficacy of sodium imipenem/cilastatin on patients of complicated urinary tract infections--following the failure of prior antimicrobial agents].

Clinical efficacy of sodium imipenem/cilastatin was studied on 45 patients with complicated urinary tract infections, on whom prior antimicrobial agents were ineffective. A 0.5g dose of IPM/CS was administered by drop infusion twice a day for 5 days. The clinical efficacy was evaluated according to the criteria of the Japanese UTI committee. Of a total of 65 strains, 29 strains of gram positive bacteria and 36 strains of gram negative bacteria, were detected as causative microbials. Bacteriologically, 23 strains of gram positive bacteria (79%) and 34 strains of gram negative bacteria (94%) were eradicated following the treatment. S. aureus, E. faecalis and Flavobacterium sp. were less sensitive to IPM/CS. Overall clinical effectiveness rate of IPM/CS in the present study was 87%. Adverse drug side effects were observed in six patients, namely they were eruption, headache and slight elevation of serum transaminase. These findings suggest that IPM/CS is an effective agent for the treatment of complicated UTI even when the prior medicine was ineffective.

[Changes in serum cytokine levels in patients with malignant bone and soft tissue tumors in the course of chemotherapy].

We studied seven patients with malignant bone and soft tissue tumors to examine the influence of chemotherapy on production of serum cytokines. Four types of cytokines (IL-1 beta, IL-2, IL-6, TNF alpha) were measured by ELISA method. A significant increase of IL-6 and TNF alpha production was observed after intravenous infusion of antitumor drugs. After intraarterial infusion of drugs there was a slight increase in the production of these cytokines. On the other hand, there were no changes in the IL-1 beta and IL-2 levels after infusion of the drugs. IL-6 and TNF alpha increased almost at the same time that the numbers of WBC (neutrophils) decreased after infusion. And this phenomenon was clear in the patients who had a good performance status.
