The strong association of HLA-DR4 with spherical mesangial dense deposits in IgA nephropathy.

41 Japanese patients with primary IgA nephropathy (IgA GN) were determined for HLA-A, -B, -C and -DR antigens. The frequency of HLA-DR4 increased slightly in total patients compared with 63 normal control persons, but this increase was not statistically significant. However, 24 patients with spherical mesangial dense deposits (SMDD) in electron micrograph out of 41 patients with IgA GN were found to have markedly increased incidence of HLA-DR4 (chi 2 = 11.52, Relative risk = 6.45, Corrected p less than 0.03). In contrast to this finding, 17 patients without SMDD had no association with any particular HLA antigens. These results suggest that IgA GN might be subdivided by the association of HLA and electron microscopic findings into at least 2 types.

[Objective tinnitus caused by myoclonus of the musculus tensor veli palitini].

Objective tinnitus is often caused by palatal myoclonus. We report a 15 years old boy with objective tinnitus in both ears and palatal myoclonus. He had myorhythmic movements of both tensor veli palatini muscles asynchronous with the objective tinnitus. The frequency of the clonus was 120 contractions a minute. The myoclonus and the objective tinnitus disappeared after division of the bilateral tensor veli palatini muscles.

[Application of radiofrequency hyperthermia for treating advanced urinary bladder cancer--a case report].

A patient with advanced urinary bladder cancer treated with a combination of radiation and hyperthermia, using short-wave radiofrequency (RF) and intravesical irrigation with warm water is reported. The tumor temperature was successfully maintained at 43 degrees to 44.6 degrees by external application of 13.56 MHZ RF when the bladder was irrigated with heated saline (intravesical temperature: 43 degrees) Hyperthermia was performed for 30 min immediately after each course of external bladder irradiation (Linac: 400 rad, twice a week; total 5 weeks exposure of 4000 rad). The tumor disappeared completely (CR) after the completion of the combination therapy.

Proteinuria in amyloidosis correlates with epithelial detachment and distortion of amyloid fibrils.

To obtain a better understanding of the mechanism of proteinuria in amyloidosis we investigated the ultrastructural distribution and pattern of arrangement of glomerular amyloid fibrils in 15 patients. Clinical data were correlated with the ultrastructural features and with the morphometric indices obtained from composite electron micrographs of whole glomeruli. The epithelial side of amyloid deposits showed spicules (discrete pointed bundles of fibrils) and apparent fraying which we termed tufts (ill-defined and diffuse expansion of fibrils). The extent of mesangial amyloid deposition and total GBM deposition did not correlate with proteinuria. Current data suggest the importance of partial detachment of epithelial cells in pathogenesis of proteinuria, which in turn correlates with distortion of amyloid fibrils, spicules and tufts.

Sequential ultrastructural changes of arteries in rats induced by restrained stress.

Single and repeated restrained stresses in rats were conducted as a model for evaluating sequential changes of gastric submucosal arteries evoked by stressful stimuli with an emphasis on medial alterations. By single restrained stress, three distinct acute changes of the media were found after a short time of the stress-withdrawal; (1) focal cytoplasmic necrosis, (2) fibrin insudation, and (3) leukocyte migration. These acute lesions were readily healed within 20 days without cellular increase or fibrous thickening of the intima. When the stress was repeated over a long period of time, medial smooth muscle cells showed marked degenerative changes ("atrophy with moth-eaten structure") with an increase of granular cell debris and basement membrane-like materials. The results suggest that angiopathic effects of stresses may be attributed to the unsuited responsiveness of small artery to vasospasm and its sequential hemodynamic derangements.

Vitamin D sclerosis in rats.

The early fine structural changes in the arteries of rats induced by excess vitamin D3 perorally or parenterally were essentially similar, except the latter had a more prominent toxic effect to the vascular wall. The ultrastructural features, incidental to calcification, included the appearance of increased ground substance with a separation of collagenous and elastic fibrils, and degenerative changes in smooth muscle cells. Atherosclerosis was greatly accelerated at the sites of vascular injury when cholesterol, cholic acid and thiouracil were added to the basal diet. Calcification was initially observed in relation to elastic fibrils or degenerated cells in the upper and middle layers of the arteries, although there were few such deposits in the thickened intima of the coronary arteries. Calcium deposition could not be a direct effect of hypercalcemia, but the functional activity of smooth muscle cells did seem to promote the mineralization of calcium and phosphate. Furthermore, vitamin D-induced sclerosis did not prevent intimal thickening of the arteries when vitamin D3 was withdrawn.
