Follow-up ten years after corticosteroid therapy for chronic active hepatitis type B.

Follow-up studies were conducted on chronic liver disease patients treated 10 years previously with corticosteroid (CS), Hbs antigen (HbsAg) was measured in previously collected paraffin embedded liver sections by enzyme-labelled antibody technique. Of 57 cases examined, 38 cases were treated with CS or immunosuppressive agents and 19 cases were not treated with CS (control group). -- Two deaths occurred in the CS-treated group and 4 in the non-CS treated group. These patients were diagnosed as either the 2B type (severe activity) of chronic aggressive hepatitis (CAH) or as having liver cirrhosis. One death was found in the 10th year due to hepatocellular carcinoma in a chronic persistent hepatitis (CPH) patient with HBsAg diffusely distributed in liver tissue. No significant difference was found in the rehabilitation rate in HBsAg negative cases of the CS group versus the control group. In positive cases, the rehabilitation rate was found to be 66.7% in the CS group compared with 0% in the non-CS group.

Diagnosis of drug induced liver injury using a lymphocyte blast transformation test based on whole blood culture.

Twenty cases of suspected drug induced liver injury (16 cases of which satisfied the criteria for at least a query positive diagnosis as based on the Fourth Congress of "Drugs and the Liver" in Japan) were studied by the whole blood culture technique of lumphocyte blast transformation. The positive rate with this technique was 10%, and no more than 15% even with the addition of one query positive. One reason for the low positive rate was that there was not only an allergic mechanism at work in the study group but that liver injury due to direct cytotoxicity of the drug was involved also. For a drug such as chlorpromazine with strong cytotoxicity for lymphocytes, it was difficult to demonstrate a relationship between allergic mechanisms and the drug with this method.

Detection of liver HBc antigen and its antibody in sera from viral hepatitis by the immunofluorescent complement technique.

Hepatitis B core antigen (HBc Ag) and hepatitis B surface antigen (HBs Ag) were detected in the liver tissue of a patient with chronic aggressive hepatitis by the immunofluorescent complement technique. The presence of anti-HBc was examined by the same method in 67 human sera previously tested for HBs Ag, anti-HBs and s-GPT levels. HBc Ag was localized mainly in the nucleus and sometimes in the cytoplasm of the hepatic cells. HBs Ag was found only in the cytoplasm. The focal area of HBc Ag positive hepatic cells seemed to correspond to the HBs Ag positive cells. Double staining demonstrated the simultaneous presence of HBs Ag and HBc Ag in individual cells. Anti-HBc positive serum was found in 46 (68.7%) cases. Forty-eight (71.6%) indicated a combination of HBs Ag and anti-HBc.

An epidemiological study of viral hepatitis type B in Taichung, Taiwan based on detection of HBc antibody.

The city of Taichung (Taiwan) can be considered as one of the areas contaminated by HB virus. Using, as a control group, the population of three districts in Japan, i.e. 2,820 residents of the cities of Kumayama (Okayama prefecture), Bizen (Okayama prefecture), and Aki (Kochi prefecture), the extent of this problem was investigated by reference to HB antigens detected in 350 patients who presented at the Taichung hospital. The Taichung detection rates for HBs antigen, HBs antibody and HBc-antibody were 20.3%, 45.3%, and 96.9% respectively. In contrast to this, the results for the Japanese control areas were, in order, 1.2-2.9%, 14.3-22.2%, and 28.6-94.3%. The HBs antigen subtype in Taichung was predominantly adw (87.3%) rather than adr. It became clear that Taichung is, indeed, an area heavily contaminated with HB virus and, at the same time, that the number of patients with viral hepatitis type B is large.

Rapid determination of thyroxine binding proteins of human serum.

A simple method is described for determing thyroxine binding proteins in human serum by electrophoresis at pH 8.6, using cellulose acetate membrane as the supporting medium. The procedure had high reliability in sera of normal subjects, pregnant women and patients with decreased thyroxine binding capacity of thyroxine binding globulin.

Once-daily administration of fluticasone propionate does not worsen controlled airway hyperresponsiveness in patients with asthma.

BACKGROUND: Inhaled steroids are currently the most important drugs for asthma patients, but compliance tends to be low. Compliance could be improved by reducing the number of daily administrations. OBJECTIVES: In the present study, we compared once- and twice-daily administration of fluticasone propionate (FP) to determine the differences in efficacy. METHODS: Subjects were 40 patients diagnosed with bronchial asthma with stable symptoms and pulmonary functions who were on twice-daily FP administration of 100 microg. There were 14 men and 26 women ranging from 29 to 72 years of age. After a 4-week observation period, subjects were randomized into two administration groups by the envelope method and followed for 8 weeks: group A, once-daily administration (200 microg of FP at night), and group B, twice-daily administration (100 microg of FP in the morning and at night). Clinical symptoms, pulmonary functions and airway responsiveness were compared between these two groups. RESULTS: No significant deterioration in clinical symptoms, pulmonary functions and airway responsiveness were observed in group A compared with group B. CONCLUSIONS: These results demonstrate that once-daily FP administration is as effective as twice-daily administration, and that it may improve the compliance for inhaled steroids.

Follow-up clinical electrophysiological studies in a patient with myotonic dystrophy.

A 38-year-old male myotonic dystrophy patient with left hemiblock, bradycardia and ventricular arrhythmias underwent electrophysiologic studies 19 months after he developed dizziness. Rapid and significant progression of binodal disease required implantation of a permanent pacemaker, which responded favorably. We suggest that careful monitoring or electrophysiologic studies are indicated in patients with these findings.

[Detection of inborn errors of metabolism in San Pablo, Brazil].

The authors report their experience in populational screenings, first regarding hyperphenylalaninemias at the city of S Paulo, by own programmation. In the same aminoacidopathies detection program, a similar methodology is being used in urine, in the children's health services of the State of S. Paulo. The same urine procedure is being used to select IEM of carbohydrates and mucopolysaccharides A programme similar to the one used for hyperphenylalaninemia is being performed for the detection of congenital hypothyroidism; T4 tests were made by radioimmunoassay (RIA) microtechnique in newborns and children in the first months of life. A similar experience in the detection of heterozygotes for GM2-ganglyosidosis type I (Tay-Sach disease) by performing the hexosaminidase A test is also reported. The importance of the performing of such populational screening tests, even in underdeveloped countries is stressed. Taking into account the high cost of the maintenance of patients with mental retardation (MR0, the cost of such programmes may become a saving whenever early diagnosis and therapy can avoid the MR.
