Ciliary and retinal changes in myotonic dystrophy.

We examined 15 patients with myotonic dystrophy by cycloscopy, ophthalmoscopy, and fluorescein angiography. In six cases, we found depigmentation in the relatively short ciliary processes, which may account for ocular hypotony in this disorder. Eight of the patients had retinal lesions, three with butterfly-shaped dystrophy, two with reticular dystrophy, and three with peripheral yellow flecks. These lesions were biomicroscopically at the level of the retinal pigment epithelium or in the deep retinal layer and did not substantially alter visual functions. Psychophysical and electrophysiologic test results were normal or only slightly affected. The butterfly-shaped and reticular dystrophies in myotonic dystrophy may be variant forms of patterned dystrophy of the retinal pigment epithelium. The peripheral yellow flecks in a stone-wall configuration noted in this disorder are particularly unique and, to our knowledge, have not been documented previously.

Treatment of multiple sclerosis with anti-measles cow colostrum.

Previous virological and immunological studies have suggested that multiple sclerosis (MS) is an auto-immune disease triggered by a virus infection. In order to inhibit the growth of measles virus in the patient's jejunum, we obtained an IgA-rich cow colostrum containing anti-measles lactoglobulin resistant to proteases. This colostrum was orally administered to patients with MS to investigate its effect on the course of the disease. Measles-positive antibody colostrum was orally administered every morning to 15 patients with MS at a daily dosage of 100 ml for 30 days. Similarly, measles-negative antibody (less than 8) control colostrum was orally administered to 5 patients. As a clinical assessment, disability scores developed by the International Federation of Multiple Sclerosis Societies were used. As a result, of 7 high NT titre (512-5120) anti-measles colostrum recipients 5 patients improved and 2 remained unchanged. Among 8 low NT titre (8-32) anti-measles colostrum recipients 5 patients improved and 3 remained unchanged. However, of 5 negative NT titre (less than 8) colostrum recipients 2 patients remained unchanged and 3 worsened. No side-effects were observed in colostrum recipients. These findings suggest the efficacy of orally administered anti-measles colostrum in improving the condition of MS patients (P less than 0.05).

Papilloedema in late-onset citrullinaemia: report of second case.

A young adult patient suddenly began to have episodes of unconsciousness that each lasted for several hours. Brief episodes of lethargy occurred monthly. Biochemical tests disclosed high levels of ammonia and citrulline in the serum. Brain oedema was suggested by CT scan, and optic disc swelling was observed ophthalmoscopically, indicating papilloedema. He died in coma. Deficient activity of argininosuccinate synthetase in the liver was demonstrated. Papilloedema was confirmed histologically. We believe that papilloedema is a serious sign indicating poor prognosis in late-onset citrullinaemia.

The effects of the administration routes on the biliary excretion of antibiotics.

The effects of the administration routes on the biliary excretion of gentamicin and cefazolin were investigated in male white rabbits. The administration routes studied were intravenous, intramuscular injection and injection into portal vein. In both antibiotics, total excretion into bile was the highest when drug was administered by the injection into portal vein. In case of gentamicin, the bile level above MIC (against P.aeruginosa) could be obtained only by the administration into portal vein. These results indicate that the administration into portal vein is useful clinically, for the drug delivery to biliary tract, especially in drugs like aminoglycoside antibiotics which have an extremely low rate of transfer to the bile.

Amauromine, a new vasodilator. Taxonomy, isolation and characterization.

Amauromine is a new alkaloid with vasodilating activity obtained from the culture broth of Amauroascus sp. No. 6237. Its molecular formula was determined to be C32H36N4O2 on the basis of elementary analysis and high resolution mass spectroscopic measurement. It has low toxicity in mice.

Changes of hepatic microtubules and secretory proteins in human alcoholic liver disease.

It has been shown that alcohol consumption disrupts liver microtubules, impairs protein secretion and leads to ballooning of the hepatocytes in rats. Ethanol-induced hepatomegaly was accounted for by an increase of the hepatocytes volume. To study whether these changes occur in human alcoholic liver disease, hepatic tubular protein and export protein content were measured in 29 cases of alcoholic liver disease and were compared with those of 37 cases of non-alcoholic liver disease and 5 cases of non-hepatobiliary disease. Hepatic polymerized tubulin was significantly decreased in alcoholic liver disease compared to non-alcoholic liver disease (p less than 0.01), while free tubulin was increased in alcoholic liver disease. Hepatic transferrin (one of the export proteins) content was significantly higher (p less than 0.01) and serum transferrin level was significantly lower (p less than 0.05) in alcoholic liver disease than in non-alcoholic liver disease. These findings indicated that even in humans, chronic alcohol consumption decreased hepatic microtubules by impairing polymerization of tubular protein and increased hepatic export protein content. This decrease in hepatic microtubules by chronic alcohol consumption may play an important role in the development of human alcoholic liver disease.

Serial changes of complement titers in the acute phase of myocardial infarction.

Although some studies suggest the involvement of a complement activation in the development of acute myocardial infarction, there has been little convincing evidence of the change of the complement in patients suffering from myocardial infarction. In 11 patients with acute myocardial infarction the serial changes of the total hemolytic complement titer (CH50), C3, C4, total serum protein and C3 conversion were investigated up to 120 hours after its attack. The level of CH50 and C4 increased consistently in the acute phase of myocardial infarction, but C3 did not show any change and total serum protein declined after 96 hours. Beta 1 A globulin, the split product of C3, could not be detected in the blood stream. These results suggest that the infarcted or ischemic myocardium would activate complements, and C3 might be consumed continuously by the infarcted or ischemic myocardium. Such consumption of complements should stimulate the production of complement and would give rise of an increase in C4 and CH50. It is suggested that activated complement might play a significant role in the development of myocardial infarction.

[Clinical application of latamoxef during the perinatal period].

Latamoxef (LMOX), a new semisynthetic beta-lactam antibiotic developed by Shionogi Research Laboratory, was evaluated basically and clinically in the perinatal period. In basal study, concentrations of LMOX on maternal serum, umbilical card serum and amniotic fluid were measured by intravenous injection method, and that's placental transferences were relatively favorable. The ratios of umbilical cord serum to maternal serum were 1/3--1/5 at early state after administration, 5--20 minutes, 2/3 at 1.5--1.75 hours. Umbilical serum concentrations were higher than maternal serum concentrations after about 2.5 hours of administration. In 1 case, transferences into tissues of uterus and placenta were studied, that's values were 13.4 and 13.0 micrograms/g at 1.75 hours after administration, respectively. In clinical application, LMOX was administered to 4 cases (intrauterine infection 1 case, urinary tract infection 3 cases) at dose of 2.0 g twice a day by intravenous drip infusion, intravenous injection and intramuscular injection method. Excellent and good clinical response was observed in all 4 cases, and bacteriological response to causative organisms was satisfactory in 3 cases except S. faecalis of mixed infection case. No side effect or abnormal laboratory values were noticed in the all cases.
