[A case report of an aged patient with erythroleukemia coexistent with pulmonary emphysema, responding well to AAAP therapy].

A case of erythroleukemia coexistent with pulmonary emphysema is reported. A 67-year-old male was admitted to our hospital in May 1981, with a few year history of cough, sputum and fatigue. He had already been diagnosed as having pulmonary emphysema and moderate anemia. On physical examination, except for pallor, no other findings were remarkable. The initial hematological examination showed hemoglobin, 9.6 g/dl, red cell count, 251 x 10(4)/microliters, platelet count, 7.3 x 10(4)/microliters, white cell count, 2600/microliters with neither myeloblasts nor erythroblasts. A sternal marrow aspiration revealed 21% myeloblasts and 40% erythroblasts including 7.5% megaloblastoids. Periodic Acid Schiff staining was strongly positive for a part of erythroblasts. A chest X-P finding was typical for pulmonary emphysema. Pulmonary function was moderately damaged. He was started on chemotherapy with AAAP (ACNU 50 mg/d i.v. drip over 4 hr x 4d, adriamycin 20 mg/d i.v. push x 4d, Methotrexate 20 mg i.v. push x 4d). The first course of AAAP brought him a complete remission with both disappearance of myeloblasts and erythroid precursors with megaloblastoid nuclei in the marrow and the normalization of white cell count and platelet count in the blood. He was discharged in September 1981 after completion of a consolidation chemotherapy with AAAP. Since then, he received two courses of AAAP as an intensification chemotherapy and has been in complete remission for more than 13 months. His pulmonary function has not been affected and no myocardial damage has been seen throughout AAAP therapy. Thus, AAAP therapy seems to be an excellent chemotherapy even for an aged patient with erythroleukemia.

Transdetermination and transdifferentiation of neural retinal cells into lens in cell culture.

Two aspects of transdifferentiation of avian neural retina (NR) cells into lens in cell culture were discussed. First, by means of the transfer experiments of NR cells pre-cultivated in spreading cultures (SpC) longer than 10 days into aggregation cultures (AgC), it was shown that NR cells are "transdetermined" into lens direction, before the phenotypic expression of lens in cells at such earlier stages of SpC. In the second part of this article, we showed that NR-cells which have already expressed some neuronal phenotypes can transdifferentiate into lens. This statement is based upon the results of chimeric cultures consisting of neuronal cell fraction separated from 10-day SpC of quail NR and of the epithelial cell fraction of SpC of chick NR. Lens cells formed in such chimeric cultures were mainly of quail origin.

Electroencephalographic analysis of activities in the optic tectum of unrestrained carp.

Electroencephalographic activity of the optic tectum in unrestrained resting carp was classified into three dominant frequency ranges of 4-7, 8-13 and 14-25 HZ, peaking at 6, 10, and 17 and 22 HZ, respectively, under power spectral analysis. All these activities were suppressed in the dark. The suppression was most prominent in the 8-13 HZ waves, but less so in the 4-7 HZ. However, while the fish was swimming actively, the 4-7 HZ spectrum increased in power and no actual increase could be observed for the higher frequency waves. Thus, it is probable that the 4-7 HZ waves involve rich motor activity, while the 8-13 HZ waves are mainly visual. The tectal activity was enhanced in the hypoxic state, with an increase in all frequency components, and enhanced further after loading, each of which corresponds to the hypoxic and post-hypoxic activations described for the mammalian cortex. A component analysis for the photically evoked response in restrained carp supported the four peaks of fundamental tectal rhythm being obtained as the spontaneous activity. In addition, each component belonging to the lower two peaks could be decomposed into the others, suggesting that the 17 and 22 HZ waves might be elementary for the tectal activity.

Study of oxytocin receptor: II. Gestational changes in oxytocin activity in the human myometrium.

This study was designed to investigate the oxytocin (OT) specific binding receptors in 20,000 x g pellets of nonpregnant, first trimester and term human myometria. The receptor analysis was done using the lower uterine segment at term and the lower portion of the anterior uterine body in nonpregnant and first trimester subjects, and no difference was found in the myometrial receptor concentrations in the various uteri. The mean +/- S.D. values of the receptor dissociation constants were 3.33 +/- 0.50, 2.71 +/- 1.03 and 1.87 +/- 0.30 nM and the number of binding sites was 0.30 +/- 0.10, 0.50 +/- 0.10 and 1.50 +/- 0.50 pmol/mg protein at each stage studied, indicating that the gestational increase of uterine sensitivity to OT is due to the increase in myometrial OT binding sites as well as its binding affinity. Further, myometrial OT binding before and after the onset of labor was studied and a marked decrease in total myometrial OT binding was noticed; 35.6 +/- 13.0% before and 20.2 +/- 5.0% after. This decrease was thought to be due to the decrease in the number of binding sites from 1.50 +/- 0.50 to 0.74 +/- 0.21 pmol/mg protein after the onset of labor (p less than 0.01). No changes were found in the dissociation constants. Thus it seems that OT and its receptor coupling triggers labor or is involved in the early steps of labor.

Specificities and some properties of human T and B lymphocyte antisera.

Antisera were prepared in rabbits by immunization with either human peripheral blood T lymphocytes (PTL) or tonsil B lymphocytes (TBL). Specificities of sequentially absorbed antisera were examined by cytotoxicity testing, immunoperoxidase staining and absorption study. Anti-PTL serum (ATS) reacted with E rosetting lymphocytes in the normal population. ATS was also reactive with some of lymphoid neoplasms, whether neoplastic cells carried E receptors or not. This serum did not lyse any of myelogenous leukemia cells. When antiserum was used in the immunoperoxidase staining, ATS strongly stained paracortical cells in the lymph node and medullary thymocytes in the thymus. On the other hand, anti-TBL serum (ABS) was reactive with lymphocytes corresponding to the percentage of EAC binding lymphocytes in the normal population. ABS reacted not only with the neoplastic cells carrying C3 receptors and/or surface immunoglobulin, but also with non T . non B-acute lymphoblastic leukemia cells and a small number of acute myelogenous leukemia cells. In the immunoperoxidase staining, lymph follicular cells showed positive immunoreactivity with ABS, but paracortical cells did not.

[Study of oxytocin receptor activities in human myometrial preparation (author's transl)].

This study was designed to investigate the oxytocin (OT) receptor activities in the human myometrial specimen throughout the course of pregnancy and in labor. The 3H-labeled OT with adequate specific activity (24 Ci/mM) and biologic activity (350 IU/mg) was employed for the OT radioreceptor assay and the apparent dissociation constant (Kds) and the number of binding sites (NBS) were analyzed by Scatchard plot. These activities were predominantly found in 20,000 xg pellets of the myometrial specimens and its binding activities were 2.2 +/- 0.1 nmol/mg protein that were assessed to be composed of two binding fractions: High-affinity--low capacity receptor fraction and low affinity--high capacity fraction. The gestational increase of the binding affinity was significant and so was the increase of the capacity, and the Kd was 1.87 +/- 0.30 nM and the NBS was 1.50 +/- 0.50 pmol/mg protein in the full term specimens and both were significantly higher than those of the nonpregnant specimens (p less than 0.01). When active labor was initiated, a significant decrease of the binding capacity (0.74 +/- 0.21 pmol/mg protein) was found, regardless of the mode of onset of labor; i.e. spontaneous or induced labor, while the binding affinity remained unchanged. These findings suggest that the gestational increase of the uterine sensitivity to OT is due to the increase of both binding affinity and capacity of the OT receptors. And there are OT and OT receptor interaction actively involved during the active stage of labor and this interaction seems to be coupled with the contractile process but is apparently modified when active labor had been already initiated.

Precursor T cell leukemia--immunological, cytochemical and morphological studies.

A case of acute lymphoblastic leukemia with a giant splenomegaly in an 8-year-old boy was investigated by immunological, cytochemical and electronmicroscopical techniques. Bone marrow and peripheral blood were largely replaced by large blast cells with a nonconvoluted nucleus. Cytochemically, most of the blast cells showed strong focal acid phosphatase activity. In the surface marker studies, these blast cells formed EAC rosettes but not E rosettes, while they showed positive reactivity with anti-T lymphocyte serum but not with anti-B lymphocyte serum. This membrane phenotype E-, C3R+, T+, B- suggested that leukemic blast cells in this patient presumably originated from precursor T cells.

Effect of intravascular platelet aggregation on blood recirculation following prolonged ischemia of the cat brain.

Reversible complete cerebro-circulatory arrest was produced in normothermic, anesthetized cats for 1 h by clamping the innominate and subclavian arteries, followed by blood recirculation of the brain for 30 min to 4 h. Blood volume of tissue samples was measured by intravenous injection of 125I-albumin and the platelet number by labelling of autologous platelets with 51Cr. The number of platelets trapped in the tissue was determined by subtracting from the total platelet count of the tissue sample those contained in the streaming blood, calculated from the blood volume by 125I-albumin. In sham-operated animals, after recirculation of autologous 51Cr labelled platelets for 60 min, no platelets were entrapped in the brain vasculature and total platelet number and fibrinogen concentration in the venous blood did not change. In animals submitted to complete cerebral ischemia for 1 h the number of trapped platelets increased significantly in the whole brain after 30 min of recirculation, i.e. the period of maximal postischemic vasodilatation and hyperemia, and highest concentrations were found in the border zone with 270 +/- 60 x 10(4) platelets/g tissue and in the brainstem with 240 +/- 70 x 10(4) platelets/g tissue. Blood platelets and fibrinogen concentration in the venous blood decreased significantly from 288 +/- 37 x 10(6)/ml and 299 +/- 50 mg% to 186 +/- 25 x 10(6)/ml platelets and 112 +/- 22 mg+ of fibrinogen, respectively. Platelets were nearly completely washed out of the brain after 4 h of recirculation. The highest increase of platelets was found in the kidney, lung and liver after 30 min of recirculation, indicating that intravascular coagulation following prolonged cerebral ischemia affects the extracerebral organs more than the brain itself.

HLA antigens as immunogenetic markers of alcoholism and alcoholic liver disease.

HLA antigens were studied among 94 chronic alcoholics. Concerning A and B-loci, there was no significant change of phenotype frequency (PF) in the HLA typing between the patients and controls (80 healthy subjects). However, there was a significant difference in the PF of CW3 between chronic alcoholics and controls (58.5% in alcoholics vs 30.0% in controls). The corrected p value was less than 0.05 with relative risk value being 3.29 HLA-DR loci were also detected in 26 patients, but there was no significant difference between the patients and controls. All alcoholics were subdivided according to the hepatic morphology, and the PF of HLA was examined. A significant high frequency of HLA CW3 was found in patients with hepatitis (64%) compared to controls (30%). There was also an increased PF of CW3 in the liver cirrhosis group (59% in cirrhosis group vs 30% in controls). In conclusion, chronic alcoholics have a significantly higher PF of HLA-CW3 as compared to controls. This characteristic feature becomes even more distinct in alcoholics with severe hepatic lesions.
