Dental extraction for patients on oral anticoagulant therapy.

Dental extraction in patients receiving long-term oral anticoagulant therapy is a controversial issue. Continuation of anticoagulation exposes the patient to serious hemorrhage, whereas cessation of therapy increases the risk of thromboembolism. Forty patients treated by coumarin underwent 63 tooth extractions, without a change in the therapeutic protocol of anticoagulation. The biologic adhesive Beriplast was used successfully to achieve local hemostasis at the site of the surgical wound. Apart from one patient who had mild oozing, there were no incidences of postsurgical hemorrhage.

Prosthetic rehabilitation of mandibulectomy and glossectomy patients.

The prosthodontic prognosis in patients with lateral discontinuity defects of the mandible is dependent on a number of complex factors. In some patients only esthetics can be improved, whereas in others mastication and speech improvement can be achieved. Two patients who had undergone the same surgical interventions but presented with different problems are described. The first patient had difficulty with mastication and mandibular movements, which was resolved by building a ramp on a maxillary prosthesis. The second patient had severe speech and swallowing problems, which were corrected with an acrylic, hollow speech prosthesis. In both patients the end result was satisfactory.

Polydimethylsiloxane button obturator for hard palate clefts.

A polydimethylsiloxane obturator for hard palate clefts is described. Because it is soft, the obturator uses anatomic undercuts of the nasal cavity for maximum retention and stability. The obturator covers only a small part of the palate. The material is nontoxic and noncarcinogenic.

Prosthetic repair of meatal stenosis.

A technique for fabrication of an acrylic resin stent that can enhance the success of surgery for meatal stenosis has been described. A temporary stent can be prepared in the operating room and used until the definitive stent is prepared in the laboratory. Both stents are easy to construct and manage.

Clinical uses of prosthetic devices in brachiotherapy for oral cavity tumors.

Three different acrylic resin intraoral stents used for radiotherapy are described. They serve multiple functions in the treatment of tumors of the oral cavity. They are prepared individually and are necessary for accurate treatment planning and in order to minimize the exposure of hospital personnel to radiation.

Acrylic resin stents for marsupialization.

Marsupialization is a surgical technique that may be preferred to enucleation in treatment of benign cysts. The acrylic resin obturator maintains the surgical opening during healing and assures success of the surgery. Two basic techniques to construct a stable and retentive obturator prosthesis were described.

Modified stock-eye ocular prosthesis.

A technique for the use of modified prefabricated eyes was described. When properly used, many of the limitations attributed to prefabricated eyes can be overcome. This technique allows the proper positioning of the iris while simultaneously recording an impression of the ocular defect tissue bed. This technique permits the finished ocular prosthesis to generate an equal distribution of pressure throughout the defect, while increasing its movement due to excellent adaptation to the walls of the socket. Although a custom-eye technique may still be necessary for some, most patients can be treated with the modified prefabricated-eye technique with excellent results.

Prosthetic treatment of earlobe keloids.

An ear clip prosthesis has been developed for maintaining pressure on earlobe keloids before and after surgical removal. The prosthesis includes an ear clip to which heat-polymerized acrylic resin is attached, which covers the keloid area.

Intraoral radiation carrier for edentulous patients.

The principles of fabricating an intraoral radioactive carrier have been described to treat malignant diseases of the oral cavity. The prosthesis provides consistent direction and fixation of the radioactive source into the same location.

A technique for fabrication of polydimethylsiloxane-acrylic resin obturators.

A flexible silicone obturator is an alternative to the acrylic resin obturator. It can be useful for patients with severe trismus who cannot insert a large prosthesis, as the flexibility of the hollow silicone section allows easy placement and removal (Fig. 9). Other patients cannot tolerate an unyielding substance against sensitive nasal tissues. Silicone can provide the necessary comfort without compromising retention. Silicone obturator sections can often be modified when further surgery is needed and result in a savings of time and expense (Fig. 10). For patients who can be reevaluated only twice a year, several silicone obturator sections can be made initially. One can be used while another is being cleaned or duplicated.
