[Fundamental and clinical studies of imipenem/cilastatin sodium in the field of obstetrics and gynecology].

A new carbapenem antibiotic and renal dipeptidase inhibitor, imipenem/cilastatin sodium (MK-0787/MK-0791) were studied for the distribution into various tissues and for clinical aspects in obstetrical and gynecological fields. The following results were obtained. Concentrations of the drug in arterial blood, venous blood, and internal genital organs following an intravenous drip infusion were measured. The results demonstrated good transfer of the drug into various internal genital organs. Four patients with bacterial infections were treated with MK-0787/MK-0791. Therapeutic results were good in 2 cases for, an effective rate of 50%. No side effects were noted in any cases. The above results suggested that MK-0787/MK-0791 may be a useful antibiotic for infectious diseases in the field of obstetrics and gynecology.

[Prenatal diagnosis and treatment of intrauterine growth retardation].

A diagnosis of intrauterine growth retardation (IUGR) was made in 150 pregnancies on the basis that the estimated fetal body weight was less than -1.5 SD of the intrauterine growth curve for the Japanese population in two consecutive measurements. The estimation of fetal body weight was made from the biparietal diameter and abdominal circumference using the formula of Shepard et al. Maternal plasma estriol, urinary estriol and plasma hPL were determined, and routine instructions to take daytime bedrest and a high protein diet were made. 75 optionally selected cases were given oral allylestrenol 30 mg/day until the time of delivery (the medicated group), and the other 75 cases (control group) were given no medication. Ultrasonic and biochemical measurements were repeated every two weeks thereafter. The estimated fetal weight calculated from Shepard's formula was of sufficient accuracy to make it possible to predict the fetal weight before delivery. In the control group, the estimated fetal weight averaged 1,281 grams at the initial measurements at an average of 32.7 weeks' gestation when the diagnosis of IUGR was made, and 2,498 grams at the final measurements at an average of 38.7 weeks gestation immediately before delivery. In the medicated group, the estimated fetal weight averaged 1,242 grams at the initial measurements at 32.8 weeks' gestation, and 2,826 grams at the final measurements at 39.0 weeks' gestation. Statistically significant increases in the fetal body weight were noted in the medicated group. Maternal plasma estriol, urinary estriol and plasma hPL concentrations rose significantly in the medicated group when compared with the control group.(ABSTRACT TRUNCATED AT 250 WORDS)

Enzymatic responses and adaptations to swimming training and hypobaric hypoxia in postnatal rats.

Twenty four male Sprague-Dawley rats, 35 days old, were randomly assigned to one of four groups: 2 resting control groups and 2 swimming groups. The sea level-control and the sea level-swimming groups were housed 5 weeks at 1,011 hPa (760 mmHg) while the hypoxic control and swimming groups were housed for 1 week at 678 hPa, followed by 4 weeks at 611 hPa. The swimming rats were subjected to a swimming program of 30 min, 6 days/week for 5 weeks. Both hypoxia groups developed significantly higher Hb and Hct levels than the sea level groups. The glycogen content in the extensor digitorum longus (EDL) and the deep portion of the vastus lateralis (DVL) muscles of the sea level-swimming group were significantly greater as compared to the hypoxia swimming group. The succinate dehydrogenase (SDH) activity in the sea level-control group was significantly lower in the EDL muscle than in the 3 other groups, and in the DVL muscle lower than that of the sea level-swimming group. Histochemically, hypoxia and swimming training induced significant increases in the fast-twitch-oxidative-glycolytic (FOG) fibers (6-11%) in soleus muscle, and decreases in the slow-twitch-oxidative (SO) fibers. The EDL muscles had significantly higher percentages of FOG fibers in the hypoxia and swimming groups than in the sea level-control group. On the basis of the present study it seems probable that hypoxia is a triggering factor for the conversions of muscle fiber types and the increase in oxidative capacity.

[Hyperthermia: microwave hyperthermia in combination with intra-arterial infusion chemotherapy of cancer of the pancreas].

Fourty eight patients with cancer of head of pancreas have been treated by the intra-arterial infusion of 5-FU and Mitomycin C alone or in combination with regional hyperthermia, employing microwave machine operating at a frequency of 2450 MHz. In patients treated with intra-arterial infusion chemotherapy alone the average survival of 4.2 month was attained, whereas the average survival of 9.0 month has been seen in 39 patients treated with infusion chemotherapy and hyperthermia. The hyperthermia treatment has improved the local therapeutic effects and the effects on the survival of the patients. Thanks to this combination therapy 27 of 39 patients (69%) survived more than 6 month, and 12 patients (31%) survived more than one year, longest survival being 7 years. This study confirms that heat administered by the microwave unit potentiated the effects of the intraarterial infusion chemotherapy.

[Changes in Y-protein and ligandin in human liver tumor tissues].

Since the relationship between tissue ligandin and liver tumors has not been studied yet, we investigated the changes of Y protein and ligandin in human hepatoma and cholangioma by gel filtration, BSP-affinity chromatography, and SDS-gel electrophoresis. The concentration of Y protein was markedly increased in both hepatoma and cholangioma, 2.8 and 4.8 times that of control, respectively. The content of ligandin was also increased in both conditions. SDS polyacrylamide gel electrophoresis of the increased ligandin confirmed the increment of 2.3 K dalton protein, which coincided with the MW of the ligandin subunit. Although the mechanism of the ligandin increase in hepatoma tissue is not clear, one possible reason might be due to the degree of differentiation of the tumor cells. In our case, the pathological examination revealed that the tumor cell was classified as Edmondoson Type II.

[Acidosis tolerance of fetal metabolism--comparative studies on isolated perfused adult, fetal and neonatal livers].

In order to compare adult, fetal and neonatal metabolism in an acidotic environment, the intracellular NAD reduction rate, tissue oxygen uptake and glucose production were measured fluorometrically, polarographically and enzymatically in isolated perfused adult, fetal and neonatal guinea pig livers at perfusion pHs of 7.40, 6.50 and 6.00, following the administration of norepinephrine (NE), lactate/pyruvate solution (L/P:10/1), octanoate (Oct), ethanol (Eth) and succinate (Succ). The following results were obtained. In the comparison of adult and fetal liver metabolic reactions at a perfusion pH of 7.40, the fetal liver demonstrated approximately half the amount of the metabolic reaction in the intracellular NAD reduction rate, tissue oxygen uptake and glucose productin following the administration of both NE and L/P, indicating the prematurity of fetal metabolism. At a perfusion pH of either 6.50 or 6.00, although the adult liver demonstrated a remarkable decrease in NAD reduction, oxygen uptake and glucose productin following the administration of both NE and L/P, the fetal livers showed a rather enhanced metabolic response acidosis often increasing to 221% increase, at a perfusion pH of 6.50 and 6.00, indicating acidosis tolerance of the fetal cellular metabolism. The perfused neonatal livers demonstrated less glucose production following the administration of NE, and increased glucose production following the administration of NE, and increased glucose production and intracellular NAD oxidation following the administration of L/P. Both fetal and neonatal livers demonstrated prematurity in the metabolic response following the administrations of octanoate and ethanol. However, enhanced metabolic response was observed at perfusion pHs of 6.50 and 6.00.

[Effect of local sustained stress on general oxygen consumption in belt conveyer workers].

The modern mechanization and rationalization of production procedures have recently reduced the intensity of work in most factories. However, it has been pointed out that monotonous inactive work and/or co-operative work can in some sense impose stronger mental and physical stresses which may induce ill health in workers. In this study, assuming the assembly and adjustment of videotape-recorders on a belt conveyor to be representative of monotonous inactive and co-operative work, changes in oxygen consumption, heart rate, blood pressure, body temperature, critical flicker fusion and subjective symptoms of fatigue throughout a workday have been followed for six female workers engaged in this type of work. The main results obtained were as follows: The average oxygen consumption of six workers was 230 ml/min, almost unchanged during the work. The belt conveyor work indicated the metabolic rate of 0.3 in terms of relative metabolic rate. The net daily energy expenditure due to the work was only about 100 kcal. The average heart rate during assembly and adjustment were respectively, about 1.4 and 1.2 times the average heart rate at rest. The blood pressure and the rate of oxygen removal increased slightly as the work progressed in assembly work, but did not increase at all in adjustment work. The critical flicker fusion and body temperature showed little change during the work.

Prenatal diagnosis and treatment of intrauterine growth retardation.

Prenatal treatment consisting of daytime bedrest, high protein diet and oral administration of allylestrenol was assessed in a prospective study of 30 patients with IUGR infants whose ultrasonically estimated body weight was less than the 10th percentile. In these pregnancies, the gestational age was confirmed in the first trimester, and the fetal weight was estimated from the BPD and AC measurements in the third trimester (Fig. 1). Following treatment, ultrasonic and biochemical determinations were performed. As results: The estimated fetal weight of 1,362 g at 32.9 g gestational weeks increased to 2,678 g at 39.2 weeks on average. The average weekly weight gain was significantly higher than the standard, and 16 cases (53.3%) were more than 10th percentile at birth (Tab. I, Fig. 2). A significant correlation (r = 0.94) between the estimated fetal weight and the birth weight was found. Following prenatal treatments, maternal plasma and urinary estriol, plasma HPL and progesterone increased significantly (Tab. II).

Albumin therapy and intrauterine growth retardation in gestosis.

Intravenous administrations of albumin, 8-16 g for 5-46 days, to the total doses of 56-736 g (averaged 198 g) were given for 10 patients suffered from both EPH gestosis and hypoalbuminemia averaged 2.4 g/dl. Plasma albumin level, total proteins and A/G ratio were elevated from 2.4 to 3.3 g/dl, from 4.9 to 5.8 g/dl, and from 0.97 to 1.28, respectively in average, following the albumin therapies. Slight decreases in hematocrit, urinary protein, SGOT and SGPT were observed, while BUN increased slightly. Pregnancies were maintained until 36.7 weeks in average. Fourteen newborn infants were delivered in good conditions, mean birth weight being 2,475 g. Four infants (28.6%) were SFDs, while ten (71.4%) were AFDs. Intravenous albumin therapies appear to be safe and effective for those who have both EPH gestosis and hypoalbuminemia.

Production and molecular size heterogeneity of immunoreactive gastrin-releasing peptide in fetal and adult lungs and primary lung tumors.

Gastrin-releasing peptide (GRP) is known to be a bombesin-like peptide present in mammalian tissues. Using GRP radioimmunoassay specific for carboxyl-terminal portion, the immunoreactive GRP (IR-GRP) content of 5 fetal lungs, 38 adult lungs, and 131 primary lung tumors was determined. All fetal lung extracts contained IR-GRP ranging from 31 to 140 ng/g, wet weight. IR-GRP was present in 7 to 21% of normal adult lungs and lung carcinomas other than small-cell carcinoma; the amount was not very large except in two cases of adenocarcinoma, in which 110 and 140 ng/g of IR-GRP were detected. In the case of small-cell carcinoma, IR-GRP was found in 23 of the 31 cases examined (74%), and nine (29%) of these contained large amounts of IR-GRP (100 to 14,000 ng/g). As for carcinoid tumors, IR-GRP was found in five of the 12 cases examined (42%), and large amounts of IR-GRP were detected in two cases (5,100 to 130,000 ng/g). Immunohistochemically, IR-GRP was found in the neuroendocrine cells of fetal lungs and in the tumor cells of primary lung tumors. When these tissue extracts were examined by bombesin radioimmunoassay that recognizes bombesin but not GRP, they did not contain immunoreactive bombesin, suggesting that IR-GRP in these tissues is more similar to GRP than to bombesin. Sephadex G-50 gel filtration always revealed two peaks of IR-GRP in both fetal lungs and IR-GRP-producing tumors. One was eluted at the position corresponding to that of porcine GRP (Peak 1) and the other, at the position just behind that of porcine GRP (14-27) (Peak 2). In the five fetal lungs, Peak 2 comprised more than 83% of the total IR-GRP. In the 12 IR-GRP-producing tumors examined, the ratio of these two peaks differed from case to case. Our data indicate that IR-GRP, which is present in fetal lung, is often produced by primary lung tumors, especially by small-cell carcinoma and carcinoid tumor, with molecular size heterogeneity.

Characterization of cathepsin D in porcine adrenocortical lysosomes.

More than 95% of the apparent cathepsin D activity in the lysosomes of porcine adrenal cortex was due to a genuine cathepsin D that has a molecular weight of 42,800 +/- 800 (mean +/- standard deviation of the mean in 5 runs) as determined by Sephadex G-100 column chromatography. On CM-Sephadex C-50 column chromatography at pH 6.9, the enzyme was resolved into five peaks which were termed Fractions D1 through D5 in order of their elution from the column. Fraction D4 and Fraction D5 together constituted more than 70% of the total cathepsin D, and were purified through chromatographic procedures to constant specific activities. The content of lysosomal cathepsin D was estimated to be about 1% of the total cellular protein. On isoelectric focusing, Fraction D4 was resolved into one major band with pI of 7.34 (termed Form D4-1) and one minor band of 7.20 (Form D4-2), while Fraction D5 gave one major band with pI of 7.50 (Form D5-1) and two minor ones of 7.37 (Form D5-2) and of 7.20 (Form D5-3). All of these 5 bands were enzymatically active. On SDS-polyacrylamide gel electrophoresis, both Form D4-1 and Form D5-1 dissociated into three subunits with molecular weights of 27,400 +/- 500 (termed Subunit A), 24,600 +/- 400 (Subunit B) and 13,800 +/- 600 (Subunit C) (mean +/- standard deviation of the mean in 16 determinations). The three subunits all contained carbohydrates.
