Maximizing satisfaction and managing dissatisfaction in mental health and human services: a model for administrative practice.

The production satisfaction is central to the delivery of mental health and human service. The most critical factor in service delivery is providing quality care. However, the ways in which services are delivered impacts the perception of quality. This paper provides a model for maximizing satisfaction among client and other important public of mental health and human service organizations.

VTM--an image-processing system for measuring ocular torsion.

This paper reports a new, fast, accurate realization of an image-processing method of measuring ocular torsion (rotation of the eyeball around the visual axis) called Video Torsion Measurement (VTM). The method is to cross-correlate the two grey-level distributions of an arc of the iris from two separate images using a fast image processor card interfaced to an IBM-AT compatible computer. The card (Matrox MVP-AT) is supplied with a library of low-level functions for controlling the hardware operations of the board and the VTM system software, which is written in the C programming language, incorporates these low-level functions to interface with the MVP-AT board as well as carrying out the data-acquisition and processing algorithms. These programs: acquire an image of an iris illuminated by a single infrared (IR) light source; threshold this image in order to identify the pupil; calculate the pupil area and locate the centre of the pupil using a centre-of-gravity algorithm; record the grey-level distribution along an arc 256 pixels long at a selected radius from the pupil centre; carry out an FFT on this (interpolated) grey level distribution; store the parameters of this reference FFT and cross-correlate the comparable iral grey-level distribution from other test images of the same eye in order to determine the amount of torsional rotation of the test images relative to the reference image. This system is interactive and is designed for operation in a clinical testing situation with a minimum of operator intervention. The VTM system has a resolution of the order of 0.1 deg depending on the arc radius used and it has been validated in two ways: by using it to measure known torsional rotations of an artificial iris-like pattern and also by direct simultaneous comparison of measures on the same human iris images from VTM and those from the standard 35 mm photographic procedure of measuring torsion.

Social density, stressors, and depression: gender differences among the black elderly.

This research examined gender differences with regard to the effects of social density and stressors upon depressive symptomatology among 600 Black elderly community residents (aged 55-85 years) of Nashville, Tennessee. The sample had more females than males and fewer married individuals. Approximately half of the males and females lived alone. Regression analyses show that poor ego and chronic medical problems were the common predictors of depression among both the males and females. Gender differences were found with regard to life events in that females tended to become more depressed as the number of events increased and as level of contact with relatives and friends decreased. Further, females with lower levels of social attachment, guidance, and reliability were more depressed. None of these social support dimensions related to depression among the males. These relationships tended to be stronger for those living alone than for those living with others.

Integrating housing and long-term care services for the elderly: a social marketing approach.

Subsidized senior high-rise apartments have tended to neglect the needs of an increasingly aged and frail resident population. Research demonstrates that this population has greater unmet needs than elderly who reside in traditional community housing. This paper makes the case for a vertically integrated marketing approach to serving the elderly. Such an approach would combine housing and community based long-term care services into a single system of care. Enriched senior high-rise apartments are a viable alternative for elders who need assistance in order to maintain an independent lifestyle.

Marketing ambulatory services to the elderly: the special role of the physician.

Elderly health care consumers are different. Understanding these differences is the key to the development of successful marketing strategies. This paper examines the area of dental services, preventive health care, physician services and mental health services. It explores how elderly health care consumers respond differently than their younger counterparts. Finally the special role of the primary physician in marketing to the elderly is examined.

Arthritis disability, depression, and life satisfaction among black elderly people.

This study examined the correlates of arthritis disorder in a sample of black elderly people in Nashville, Tennessee. More specifically, both the presence of arthritis and its severity were examined with regard to level of depression and life satisfaction, particularly in the areas of family life, social support, and leisure activities. Data were collected through face-to-face interviews with 600 black elderly people residing in the community. The sample had more females (70 percent) than males (30 percent) and majorities who were not married (75 percent) and who were living alone (56 percent). The authors' analyses indicated significant differences between arthritic and nonarthritic black elderly people in that arthritic elderly people were more depressed and experienced lower levels of life satisfaction. Further, levels of depression and life dissatisfaction were higher among those experiencing disability associated with the illness and those who were currently in treatment.

Dealing with labor shortages in long-term care: a marketing problem.

A recent analysis of data from the Bureau of Labor statistics raise serious implications for the long-term care industry. The human resource problems faced by managers in long-term care will escalate into a fullblown crisis by the end of this century. This will result from a decrease in the number of young workers available to work in unskilled and semiskilled occupations. The effect of this shortage will be exaggerated by an expansion of other sectors of the service industry. Long-term care facilities will be forced to compete with the fast food and retail industry as well as other sectors of the health industry for scarce workers. This article briefly examines the causes, consequences of this problem and suggests several strategies to mitigate the effect of the coming labor shortage.

Consumer driven strategies in the management of long term care services.

This paper discusses the concept of consumer oriented long term care management. It is argued that when long term care managers have a consumer orientation they are more responsive to the needs of their clients. This orientation puts the needs and preferences of clients at the forefront of planning and marketing decisions.

An evaluation of a therapeutic health program for the black elderly.

This paper evaluates the effectiveness of a therapeutic health program which was offered in 1988 to the black elderly living in subsidized high-rise apartments in Nashville. The pre-program data (of 1987) indicated distinct differences in that the elderly living in the high-rises had significantly more medical problems, higher levels of depression, and smaller social support networks than those living in their own single dwelling homes. The therapeutic program included various modules such as cognitive and grief therapy, reminiscence therapy, social skills and remotivation therapy. Analyses of pre-post measures of selected variables showed that the program was effective in improving the health status of the participants. As compared to a non-treatment control group, the elderly in the experimental group showed significant improvements in depression, social network, and a sense of control over their lives. A discussion of these findings is provided.

A social work practice model of case management: the case management grid.

Case management is a major component of the current mainstream of social work practice. Nevertheless, the field has failed to adequately conceptualize case management. The first step in defining case management practice is to separate it from the administrative structure that forms the context of practice. The next step is to delineate the two basic dimensions of case management--enabling and facilitating. The role of the social work case manager is to integrate formal systems of care with the activities of families and primary groups. Case management practice focuses on enabling individuals and primary groups to reach their full potential and on facilitating more effective interaction with the larger social environment. The case management grid illustrates how social work practitioners integrate enabling and facilitating into their practice approaches.

Marketing home care services.

With the decline in inpatients, hospital administrators and marketers are seeking revenue opportunities in meeting the needs of a new market, home care patients. The challenge for managers in the home care arena is to target their marketing efforts towards the important decision makers. Managers who can best meet the goal of patients, families and professionals will be most successful.

Delegation of functions by dietitians to dietetic technicians.

Questionnaires were sent to 197 randomly selected hospitals in the continental United States. Replies were used as a basis for determinign the degree to which dietitians are willing to delegate tasks to dietetic technicians. No significant difference was found between the percentage of task functions clinical dietitians were willing to delegate to clinical dietetic techncians and the number that administrative dietitians were willing to delegate to administrative dietetic technicians. Dietitians who serve in both capacities are willing to delegate a significantly higher percentage of ther clinical task functions than their administrative functions. Comparing dietitians who graduated before 1968 with those graduating in 1968 or later, no significant difference was found between the two groups of administrative dietitians; however, the clinical dietitians who graduated in 1968 or later were willing to delegate significantly more duties. No significant difference was found in the average number of task functions dietitians working in different size hospitals were willing to delegate. For task functions which 33 per cent or more of dietitians were unwilling to delegate, it appears that those who do not feel this would be ideal also feel it is impractical. Dietitians who have performed a given function do not appear more or less willing to delegate it than those who have not performed that task. Thus, while a substantial number of dietitians were willing to delegate to the dietetic technician, there was also reservation as to the type of task functions, some of which the dietitians apparently feel should remain a function of the dietitian's role. This is especially true of the administrative dietitian.
