Sensory and autonomic polyneuropathy associated with hypergammaglobulinemia.

Sensory and autonomic polyneuropathy is a rare disease characterized by a sensory nerve disorder and postganglionic autonomic dysfunction. The etiology of this disease is unknown. We described a 51-year-old woman who had a chronic sensory dominant polyneuropathy and dysautonomia associated with hypergammaglobulinemia. In the previous reports of sensory and autonomic polyneuropathy, not much attention was given to coexisting hypergammaglobulinemia. By reviewing the literatures, hypergammaglobulinemia was frequently present in these case reports. This fact leads us to consider that an immunological mechanism may be playing a role in the pathogenesis of this disorder.

[Effects of S-adenosyl-L-methionine on experimental cerebral ischemia].

Effects of S-adenosyl-L-methionine (SAMe) on experimental cerebral ischemia were investigated using two different ischemic models. Cerebral energy metabolites (ATP, lactate, c-AMP) and brain water content were measured. It is reported that SAMe accelerates synthesis of phosphatidyl choline and increases erythrocyte membrane fluidity. Complete ischemia was produced by heart excision using wistar kyoto rats. SAMe (100 mg/kg, I.P.) was administered twice at one hour and immediately before inducing ischemia. The brain of rats were irradiated by microwave to stop the enzyme activity exactly 60 seconds after inducing ischemia and brain energy metabolites were measured. Recirculation model was produced by one hour recirculation following two hours ischemia induced by clipping of bilateral common carotid arteries using stroke-prone spontaneously hypertensive rats. SAMe (100 mg/kg, I.V.) was administered twice one hour after clipping and ten minutes after recirculation. The brain metabolites and water content were measured one hour after recirculation. In the complete ischemia, ATP and c-AMP levels were statistically high in the SAMe treated group compared to the untreated group (vehicle). But there was no statistical difference in lactate between the treated group and the untreated group. In the recirculation model, lactate elevation was suppressed in the SAMe treated group compared to the vehicle group with statistical difference, but there was no difference in ATP and c-AMP. Also, there was no difference in water content between the treated and the untreated group. SAMe protected energy failure in ischemia and accelerated recovery from ischemia. It is indicated that this agent is beneficial for treatment of cerebral ischemia in the acute stage.

[External immobilization of cervical spine with a halo vest--experience with 31 cases].

Post-traumatic or post-operative immobilization of cervical spine often requires long term bed-rest. But, bed-rest indeed gave discomfort to patients, nonetheless, sometimes complete external fixation cannot be gained. Since 1982, we have applied Halo vest to the patients who required external immobilization of cervical spine. In this paper, we reported our experiences with 31 cases who were treated by Halo vest. Thirty-one cases comprised 19 cases of cervical spinal trauma, 5 cases of atlanto-axial dislocation, 1 case of foramen magnum meningioma, and 6 cases of ossification of posterior longitudinal ligament (OPLL). In traumatic cases, Halo vest was used as external fixation of injured spine in 8 cases, and as an adjunct to surgical fusion in other cases. In case of atlanto-axial dislocation, 3 cases underwent posterior fusion, and 2 cases underwent anterior fusion. Anterior fusion of C1-2 was also done in case of foramen magnum meningioma after total excision of tumor. Six cases of OPLL underwent anterior decompression and anterior fusion. Mean immobilization time by Halo vest was as follows: 58 days (28-113) in case of cervical spinal trauma, 85 days (56-144) in case of atlanto-axial dislocation who underwent posterior fusion, 92 days (89-93) in case of atlanto-axial dislocation and foramen magnum meningioma who underwent anterior fusion, 59 days (54-70) in case of OPLL. Early ambulation was achieved in many cases. Good external fixation and result was obtained 27 cases out of 31 cases. Re-dislocation of cervical spine occurred in 1 case of cervical spinal trauma while he was in Halo vest.(ABSTRACT TRUNCATED AT 250 WORDS)

[Surgical results and clinical analysis of 19 cases of acromegaly].

The authors have been analyzed the results from 19 patients who underwent transsphenoidal microsurgery for their pituitary adenomas. All of the patients showed some degree of clinical improvement after surgery. Surgical complications seemed to be negligible except one patient who had suffered from persistent diabetes insipidus postoperatively. The results in terms of postoperative GH levels were almost good in 17 patients in whom preoperative GH levels had been below 100 ng/ml. There was no relationship between postoperative GH values and tumor grades or types (in Hardy's classification) in this group of patients. In 12 of them, postoperative GH values fell and remained below 5 ng/ml, although abnormal GH responses to dynamic testing were still noted. On the other hand, the results were particularly poor in the remaining 2 patients in whom preoperative GH levels had been higher than 100 ng/ml. Postsurgical radiation therapy was applied to 5 cases. Postirradiation GH values decreased in all 5 cases, but did not reach to the normal level (below 5 ng/ml) in 3 of them.

Sonographic patterns of renal cell carcinoma with emphasis on relation to tumor size.

The sonographic appearance of 37 renal cell carcinomas was reviewed with particular emphasis on the relationship between the echo pattern of tumor and tumor size. Small carcinomas tended to be solid and hypoechoic; seven lesions less than 5 cm in the largest dimension were either hypo- or isoechoic . The echogenicity level of the solid parts within the lesion, whether it was solid or mixed, tended to increase as the tumor grew. It is important to recognize that the sonographic appearance of renal cell carcinomas varies with the size of the tumor.

[An electrophysiological method for detecting diabetic retinopathy in rats].

The present study was undertaken to devise an electrophysiological method for detecting diabetic retinopathy in rats. The electroretinogram (ERG) and visual evoked potential (VEP) were recorded from unanesthetized and unrestrained rats rendered diabetic with a single i.v. injection of streptozotocin (STZ) at 35 or 40 mg/kg. The STZ-treated rats showed signs of diabetes: hyperglycemia, glucosuria, hypoinsulinemia, polyuria and increased water intake. Amplitudes of the ERG a- and b-waves and oscillatory potentials (OPs) on the b-wave were decreased and latencies of these waves were prolonged gradually after STZ was administered. Especially, latencies of the OPs became significantly different from the pre-treatment values. Latency of the VEP N1 wave showed a slight prolongation, which might be secondary to the depression of retinal function. Histological examination showed swelling and proliferation of the lens epithelium and swelling and vacuolization of the lens fiber were observed in the eyeball 9 weeks after STZ-treatment. Moreover, thinning of each retinal layer was observed in a few rats. Daily s.c. injection of insulin at 10 units/rat/day started from the 4th week. The ERG values returned to the control values after 2-3 weeks of insulin therapy. These results indicate that the ERG and VEP recording procedure used in the present study is useful for early detection of the diabetic retinopathy in rats and that the OP of the ERG appears to be vulnerable to diabetes in the rat as it is in the human.

[Effects of sodium iodate, iodoacetic acid and ethambutol on electroretinogram and visual evoked potential in rats].

The effects of sodium iodate (SI), iodoacetic acid (IAA) and ethambutol (EB) on the electroretinogram (ERG) and the visual evoked potential (VEP) were examined in unrestrained rats. A single intravenous dose of SI at 25 mg/kg caused depression of amplitudes of the ERG a-wave and oscillatory potentials 24 hrs after dosing. Following these changes, the amplitude of the ERG b-wave decreased. The depression of the VEP was observed in parallel with the depression in amplitude of the ERG. A single intravenous dose of IAA, even at a dose of 60 mg/kg which induced death of the rats, did not cause any significant abnormality in the ERG and VEP. Repeated subcutaneous dose of EB at 500 mg/kg/day depressed the amplitude of the P1-N1 wave and prolonged the peak latency of the P1 and N1 waves of the VEP without affecting the ERG after administration for 5 to 6 weeks. These abnormalities of the VEP appeared almost in parallel with ataxic gait. Neither gross behavioral changes suggesting visual disturbances nor abnormal ocular fundus was revealed in any rat receiving SI or EB even when marked depression of the ERG and/or VEP was observed. These results indicate that SI damages retinal function and EB does the conduction pathways from the retina to the visual cortex. In addition, the simultaneous recordings of both the ERG and VEP in unrestrained rats were found to be useful for evaluating the visual toxicity, and to furnish useful information on the site of toxic action of drugs.

[Experimental cerebral ischemia after bilateral common carotid artery ligation in SHRSP, SHRSR and Wistar rats: correlation between blood pressure and degree of ischemia].

Three different pressure groups of rats, stroke-prone spontaneously hypertensive rats (SHRSP, 200-270 mmHg), stroke-resistant SHR (SHRSR, 160-240 mmHg), and Wistar rats (WR, 120-160 mmHg) were used to investigate the effect of prior existing hypertension on the severity of brain damage induced by ischemia. The cerebral ischemia was induced by bilateral common carotid artery ligation (BLCL) and the survival rate, cerebral blood flow, cerebral energy metabolites (ATP, lactate c-AMP) and water content were measured. Colloidal carbon perfusion was also performed. Sixteen-week-old male rats were used. The survival rate was observed until 24 hours after BLCL. Cerebral blood flow was measured in parietal cortex by hydrogen clearance method. ATP was measured by luciferin-luciferase method, and lactate by enzymatic method using LDH. c-AMP was measured by radioimmunoassay. Brain water content was measured by freeze-dry method. These measurements were done for animals surviving 6 hours of BLCL. Colloidal carbon perfusion was done according to Ames' Method. The survival rate was lower in the hypertension group. The survival of SHRSP and SHRSR were 20% compared to 71% in WR after 24 hours of BLCL. The cerebral circulation of SHRSP fell abruptly and was near to zero after one hour of BLCL. In SHRSR this fall of cerebral blood flow was prominent in the rats of higher blood pressure. On the other hand there was no apparent fall of cerebral blood flow in WR after BLCL. The cerebral energy metabolites. ATP and c-AMP showed the lowest level in SHRSP which had the negative correlation to blood pressure.(ABSTRACT TRUNCATED AT 250 WORDS)

[A case of amaurosis fugax with anomalous origin of the ophthalmic artery and atheromatous stenosis of the external carotid artery].

Amaurosis fugax associated with anomalous origin of the ophthalmic artery without occlusion of internal carotid artery is very rare. We are reporting such a case of amaurosis fugax with anomalous origin of the ophthalmic artery and atheromatous stenosis of the external carotid artery. A 49-year-old-man was referred to our neurosurgical clinic because of transient monocular blindness. The patient was diagnosed as carotid TIA and was treated within aspirin and dipyridamole. In spite of this treatment, he experienced further transient dimness of vision in the right eye, several times during a period of one month. On admission, general, physical and neurological examinations disclosed no abnormality except for a bruit over the right carotid artery. Platelet adhesiveness and aggregation were slightly elevated. Right common carotid angiogram showed marked stenosis of the origin of the external carotid artery and small "sail-like" plaque in the internal carotid sinus. The angiogram also demonstrated that the right ophthalmic artery was originated from the middle meningeal artery through the superior orbital fissure. Since choroid crescent was observed, all the branches of the ophthalmic artery including the central retinal artery were filled by the external carotid system. The right ophthalmic artery was never visualized by the selective internal carotid angiography. Subsequently right carotid endarterectomy was performed without complication. One year later he remains asymptomatic.(ABSTRACT TRUNCATED AT 250 WORDS)

[Combined therapy of ventriculo-cisternal irrigation with external decompression in acute ruptured aneurysm].

Vasospasm after clipping of an acute ruptured aneurysm plays an important role in its prognosis. An effective therapy for vasospasm has not been established. We had attempted to remove the subarachnoid clot, which probably caused vasospasm, by ventriculo-cisternal irrigation and had not so good results (A: a group without decompression). Therefore, we have performed a therapy combined ventriculo-cisternal irrigation with external decompression (B: a group with decompression). We wish to report the efficacy of external decompression for an acute ruptured aneurysm. Evaluation of the outcome is due to Table 2. 55% of A group and 80% of B group have a better grade than "fair". Furthermore, a significant difference lies between A and B group associated with symptomatic vasospasm (brain ischemia caused by vasospasm). Namely, A group has all poor or dead cases but B group has 80% of the cases better than "fair". We concluded that relieving ICP by external decompression could suppress the aggravation of the post operative symptomatic vasospasm.

A procedure for recording electroretinogram and visual evoked potential in conscious dogs.

A procedure for recording the electroretinogram (ERG) and the visual evoked potential (VEP) in conscious, nonstressed dogs was investigated. The animals were trained to adapt to a semirestraining support that permitted free head movement. The ERG and VEP were recorded, respectively, through contact lens-type electrodes consisting of double gold rings and previously implanted cortical electrodes. Just before the light stimulus was given, the eye of the dog was directed toward the lamp by gentle manipulation of the head. Using this recording procedure, typical waveforms of the ERG consisting of an a-wave followed by a b-wave and oscillatory potentials on the rising slope of the b-wave were recorded from fully dark-adapted dogs. During the course of dark-adaptation, the amplitudes of these waves increased and reached a fully developed shape 15-30 min after start of the adaptation. Stable ERG and VEP were recorded repeatedly for a long period of time. These results indicate that this procedure for recording the ERG and VEP is suitable for long-term visual toxicity studies of drugs in conscious and relatively nonstressed dogs.
