Syncarcinogenic effects of methyl methanesulfonate with methylazoxymethanol acetate in rat small intestine and liver.

The carcinogenic potency of methyl methanesulfonate (MMS) and its combination effect with methylazoxymethanol (MAM) acetate were evaluated in rats. MMS was suggested to be weakly carcinogenic for the small intestine. Syncarcinogenic effects of MMS and MAM acetate which could be due to summation of their genotoxic influences were obtained in the small intestine and liver.

Reduced DNA repair response of cells from carcinogen-induced rat liver altered foci exposed to chemical carcinogens in culture.

Cells were cultured from rat livers containing enzyme-altered foci induced by N-2-fluorenylacetamide and the level of DNA repair synthesis in response to several carcinogens was compared in gamma-glutamyl transpeptidase-positive foci cells and in enzyme-negative hepatocytes from the same livers. The level of unscheduled DNA synthesis elicited in foci cells by either the activation-dependent carcinogens N-2-fluorenylacetamide and diethylnitrosamine or the activation-independent carcinogens N-methyl-N'-nitro-N-nitrosoguanidine and 1-methyl-1-nitrosourea was significantly lower than that in hepatocytes. The results suggest that the repair of DNA damage by these altered cells is abnormal.

Genotoxicity of a variety of pyrrolizidine alkaloids in the hepatocyte primary culture-DNA repair test using rat, mouse, and hamster hepatocytes.

Seventeen pyrrolizidine alkaloids were studied with the hepatocyte primary culture-DNA repair test using rat hepatocytes. DNA repair synthesis was elicited by 15 alkaloids, including 11 of unknown carcinogenicity, i.e., senecionine, seneciphylline, jacobine, epoxyseneciphylline, senecicannabine, acetylfukinotoxin, syneilesine, dihydroclivorine, ligularidine, neoligularidine, and ligularizine. The positive results with these alkaloids of unknown carcinogenicity suggest that they are possibly genotoxic carcinogens. The two pyrrolizidine alkaloids that did not elicit DNA repair were retronecine which lacks a necic acid component and ligularinine which lacks the unsaturated double bond at the 1,2-position of the pyrrolizidine ring. Five pyrrolizidine alkaloids, retronecine, monocrotaline, seneciphylline, senkirkine, and clivorine, were also tested in the DNA repair test with hamster or mouse hepatocytes. These alkaloids, except retronecine, showed a positive response in the test with hamster hepatocytes, but in the test with mouse hepatocytes clivorine in addition to retronecine was also negative. The results indicate a species difference in liver bioactivation of pyrrolizidine alkaloids, implying that there could be species differences in their carcinogenic activities.

Genetic toxicity of several antihypertensive drugs possessing a hydrazine group.

Mutagenicity and genotoxicity of antihypertensive drugs, ecarazine, budralazine, benzerazide, and carbidopa were compared with those of hydralazine whose genetic toxicity and carcinogenicity were well established. Ecarazine and budralazine as well as hydralazine showed apparent mutagenicity in Salmonella/microsome test using a strain TA 100 and weak mutagenicity in strains TA 97 and 2637. Benzerazide and carbidopa showed merely weak mutagenicity in TA 100. None of tested drugs except hydralazine exerted any positive result in hepatocyte primary culture (HPC)/DNA repair test, indicating no genotoxic activity of these hydrazine drugs.

Thanatophoric dysplasia.

Two autopsy cases of thanatophoric dysplasia in male newborn infant are described. They have markedly short-limbs and narrow thoraxes. Histologically, generalized disruption of endochondral ossification with normal resting cartilage was seen at the epiphyseal region of femur. Differential diagnosis from other 3 types of lethal chondrodysplasia is described and the histogenesis of thanatophoric dysplasia is considered with a survey of the literature.

Measurement of unscheduled DNA synthesis on alkylating carcinogens in rat intestinal mucosal cells from different sites.

Unscheduled DNA synthesis (UDS) induced by in vitro exposure to five alkylating carcinogens was measured in the mucosal cells from three different sites of the rat intestine. The yield of UDS by N-methyl-N'-nitro-N-nitrosoguanidine or N-ethyl-N'-nitro-N-nitrosoguanidine or methylazoxymethanol acetate was higher in the cells of colon & rectum than in those of the jejunum or ileum. Conversely, UDS induction by N-methyl-N-nitrosourea or methyl-methanesulphonate was seen at a rather higher level in the cells from the small intestine compared with those from the colon & rectum.

A cytochemical study of lactic dehydrogenase (LDH) isoenzymes in human lung cancer.

The cytochemical and electrophoretic LDH isoenzyme patterns in cells from lung cancer tissues were examined with 2.6 M urea treatment and the correlation between the LDH isoenzyme pattern and histopathological entities of lung cancer was also studied. The zymograms of the nonlesional lung tissues indicated the main peak at LDH3, with the M/H ratio 0.74. In tumor tissues, epidermoid carcinoma, adenocarcinoma, and large cell carcinoma showed almost similar patterns of isoenzymes with LDH4 peak. Though small cell carcinomas had a peak at LDH3, they indicated lower activity of %LDH1 and %LDH2 than those in nonlesional lung tissues. Most of the tumor tissues showed a high M/H ratio, more than 1.0. In cytochemical stain for LDH, no inhibitory effect of urea treatment was seen in the nonlesional lung tissues. On the other hand, in the cells obtained from three types of lung cancer, except for small cell carcinoma, urea inhibition was noticed. Cytochemical stainability for LDH-M subunits corresponded well to the results of LDH zymogram. These results suggest that the LDH stain with urea treatment is a useful method for detecting malignancy in cytological specimens.

[A case of paraganglioma of the urinary bladder].

Herein we report a case of primary paraganglioma of the urinary bladder. A 21-year-old woman was admitted to an emergency hospital, because of gross hematuria, dysuria and complete urinary retention. Pelvic CT scan and ultrasonic examination revealed a bladder tumor and coagulated blood mass, then she was referred to our clinic for further treatment. On cystoscopy a dark brown lobular tumor was seen on the anterior wall. The bladder tumor was removed by partial cystectomy. The operative specimen measured 6 X 4 X 3 cm and weighed 50 g. Histological examination showed it to be a paraganglioma of the urinary bladder, which was chromaffin positive. Her serum and urinary catecholamine level were within the normal range after operation. She is now ten months postoperative, free from hematuria, and showing no other symptoms or signs of disease including metastasis.

Adenoma with multiple hyperplastic nodules in noncirrhotic liver possibly related to long-term administration of a hypolipidemic drug.

Described here is an autopsy case of a 76-year-old woman with preneoplastic and other adenomatous hepatocellular lesions. Multiple small hyperplastic liver foci with PAS positive reaction and adenomatous nodular lesions were noted in a non-cirrhotic liver. These lesions resembled altered foci or neoplastic nodules which are currently regarded as premalignant changes in experimental animal models. It is suggested that the liver lesions of this woman may be related to long-term administration of hypolipidemic drug clofibrate for hypercholesteremia and the lesions could be one of the precursor changes of human hepatocellular carcinoma.

[Multihospital randomized study on the adjuvant chemotherapy with mitomycin-C and Futraful for gastric cancer V. Estimation of 5-year survival rate].

The addition of Futraful to MMC therapy of gastric carcinoma was evaluated. Nationwide 297 institutions participated in a randomized controlled study and cumulative five-year survival rate was calculated. A total of 1,673 cases was studied. These were divided in two groups: Group A, without Futraful, and Group B, with Futraful. Futraful was administered postoperatively for 3 months. The MMC treatment was done by moderate intermittent administration (Method I) or by large dose administration (Method II). Futraful improved survival rates regardless of the method of MMC treatment. Especially, Group B' (more than 60 g of Futraful) showed significantly better results in patients with metastases and positive serosal invasion in both methods, and beta type infiltration in Method I. Furthermore the Group B' patients had higher survival than Group A patients in Method II for patients in stage III and gamma type infiltration. Method II, a large dose MMC, plus Futraful was suggestive of particular effectiveness in preventing peritoneal metastases in curatively resected cases.

[Effect of postoperative long-term chemotherapy of stomach cancer using mitomycin C and futraful--the secondary study (1)].

The Cooperative Study Group of Surgical Adjuvant Chemotherapy for Gastric Cancer in Japan with the participation of 334 institutes nation-wide had conducted the secondary study in order to investigate the usefulness of futraful in long term treatment, based on the results obtained by the primary study. The following method was taken for the study and patients were randomly divided into three groups: A group-given a high dose of MMC immediately after operation; B group-given futraful for one year after MMC administration; C group-given futraful alone for long-term. The number of eligible cases for data analysis was 3,o30. Side effects observed during the long term treatment with futraful were considered to be minor. Although the occurrence of hematocytopenia was slightly frequent because of possible increase of influence by the concomitant use of MMC, no tendency of hepatic disorders and any subjective side effects to be strengthened by MMC combination was observed at all. As for 2-year survival, an elevated survival was found in the group of concomitant use of MMC and futraful; Especially, a significantly high survival was found in the cases of stage III and n (+) . ps (+). Furthermore, the survival was elevated in proportion to the increase of total dosage of futraful. This suggested the usefulness of futraful for long term treatment.

DNA content of liver cell nuclei of N-2-fluorenylacetamide-induced altered foci and neoplasms in rats and human hyperplastic foci.

The DNA content of cell nuclei of liver lesions induced in inbred male ACI rats by administration of 0.02% N-2-fluorenylacetamide (2-FAA) diet was measured with a microspectrophotometer in Feulgen-stained specimens. Altered liver foci of the eosinophilic, clear, and simple basophilic (toluidine blue weakly positive or negative) types had modal DNA values with a wider deviation (2C-10C) than the euploid pattern of normal hepatocytes (2C-6C). However, the DNA histograms of cells in hyperbasophilic (intensely toluidine blue-positive) foci were aneuploid (2C-18C) and resembled those of cells in hepatocellular carcinomas (2C-23C). Conversely, the modal DNA values of neoplastic nodules showed a narrower range (2C-12C). These results confirm that the hyperbasophilic foci may be a population similar to carcinoma in situ or may be a direct precursor lesion of hepatocellular carcinoma. In the livers from 4 humans, 3 of whom had a history of long-term use of some drugs, multiple hyperplastic foci resembling eosinophilic or clear cell foci in rats were identified. The modal DNA values of the cell nuclei in these foci showed a wider deviation (2C-10C) than in surrounding hepatocytes (2C-6C). In 2 patients, the lesions contained neoplasms and, therefore, hyperplastic foci in humans may be related to tumor development.

Wall reinforcement with highly porous Dacron mesh in aortic surgery.

Wall reinforcement with highly porous synthetic Dacron mesh was devised and evaluated in various aortic operations. Excellent attachment of the mesh to the aortic wall, good preservation of pliability of the aorta, and efficient pressure resistance with minimal scar tissue formation were the usual findings. This type fo wall reinforcement was employed in 60 patients for the following indications: 1) prevention of suture line complications; 2) wall reinforcement of aortic aneurysm without resection; 3) reinforcement of adventitia or autogenous vein following thromboendarterectomy. It is concluded that reinforcement with highly porous synthetic Dacron mesh should be superior to reinforcement with conventional prosthetic Dacron graft.
