A controlled clinical trial of fluspirilene, a long-acting injectable neuroleptic, in schizophrenic patients with acute exacerbation.

A 4-week double-blind controlled clinical trial was carried out in which fluspirilene, an injectable diphenylbutylpiperidine neuroleptic given weekly, was compared to chlorpromazine in the treatment of 40 newly admitted schizophrenic patients with acute exacerbation. Similar therapeutic improvement was obtained with both drugs, but men needed a significantly higher mean dose of fluspirilene (23 mg/week) than women (13 mg/week). Fluspirilene induced more parkinsonism than chlorpromazine, but less drowsiness, dizziness, and dry mouth. The difference between the sexes in the potency of fluspirilene and its greater potential to induce parkinsonism may be related to its lesser presynaptic and D1-dopamine receptor blocking properties. The low incidence of autonomic side effects confirms the relative specificity of fluspirilene for dopamine receptors.

Mortality among employees engaged in chemical manufacturing and related activities.

A retrospective cohort mortality study was conducted among men employed for one or more years, between 1940 and 1969, at an operating division of a large chemical company. Vital status follow-up for the cohort of 1,919 men was determined through 1979 and identified 390 deaths. Overall mortality in the study group and in each of eight employment subgroups was less than that of the corresponding United States white male population. Additionally, standardized mortality ratios were not significantly elevated for any of the examined cause-of-death categories. Cause-specific mortality comparisons were also made among the employment subgroups and by duration of employment in the company division using an internal analysis method. There were no relationships observed for employment duration. Several significant differences (p less than 0.05) by employment subgroup were noted; however, neither the decreases nor increases presently could be ascribed to identifiable environmental factors.

Teaching psychiatry in the health maintenance organization.

Psychiatric residents training in a health maintenance organization (HMO) rapidly encounter the conflict between high demand for services and limited supply of time. In one HMO setting, the Harvard Community Health Plan, supervisors assist trainees in managing their practices to recognize their avoidances and conflicts and to respond therapeutically in keeping with the setting.

A multicentre trial of live attenuated varicella vaccine in children with leukaemia in remission.

Two hundred forty children with acute leukaemia in remission for at least 1 year were immunized with live attenuated varicella vaccine. All were susceptible to varicella before immunization. There was a seroconversion to varicella-zoster virus in approximately 85% after 1 dose, and in 97% after 2 doses. The major side effect was mild to moderate rash, seen mainly in children with maintenance chemotherapy suspended for 1 week before and 1 week after vaccination. Vaccinees with rash were at some risk (10%) to transmit vaccine virus to varicella susceptibles with whom they had close contact. Twenty-nine vaccinees were subsequently exposed to varicella in their households. The attack rate of clinical varicella in these vaccinees was 21%, which is significantly lower than the 80%-90% attack rate occurring in varicella susceptibles after household exposure. All these breakthrough cases of varicella were mild, even in leukaemics receiving chemotherapy. Varicella vaccine was approximately 80% effective in preventing clinical varicella in children with leukaemia and completely effective in preventing severe varicella in this high-risk group.

Live attenuated varicella vaccine in children with leukemia in remission.

One-hundred-ninety-one children with acute leukemia in remission for at least one year were immunized with 1 or more doses of live attenuated varicella vaccine. All were susceptible to varicella prior to vaccination. The only significant side effect was mild to moderate rash, seen especially in children with maintenance chemotherapy temporarily suspended for one week before and one week after vaccination. Children with rash were at some risk (10%) to transmit vaccine virus to varicella susceptibles with whom they had close contact.

Indomethacin improvement of septic acute respiratory failure in a porcine model.

The effects of indomethacin, a prostaglandin synthesis blocker, were tested in a porcine model of septic acute respiratory failure (ARF) produced by continuous infusion of live Ps. aeruginosa. Control groups received either indomethacin boluses (2 mg/kg) intravenously at 20 and 210 minutes or Ps. aeruginosa infusion (2 X 10(8) CFU/20 kg/min). The treatment group received both continuous Ps. aeruginosa infusion and indomethacin. Indomethacin alone transiently but significantly decreased cardiac indices, heart rate, and PaCO2. With Ps. aeruginosa infusion, significant pulmonary hypertension, hypoxemia, increased intrapulmonary shunt fraction, and systemic hypotension occurred. In this porcine model of septic ARF, indomethacin doubled the survival period and improved cardiopulmonary parameters.
