Hemolytic disease of the newborn associated with anti-Jk3.

This report documents a mild case of hemolytic disease of the newborn (HDN) associated with anti-Jk3. The Filipino mother had previously had six children none of whom had been affected by HDN. She had been transfused at the time of her second pregnancy. Anti-Jkb and anti-Jk3 were detected in the maternal serum at the time of her seventh delivery. No prenatal serologic tests for blood group antibodies had been performed. The cord blood was found to have a positive direct antiglobulin test and anti-Jkb plus anti-Jk3 were eluted. The infant was treated with phototherapy.

Down syndrome: nursing interventions newborn through preschool age years.

Down syndrome is the most common cause of mental retardation in children. The children need opportunities to become important members of their families. The preschool years are an excellent time for the children and family to learn about the condition and to establish behaviors that will optimize opportunities for the children to reach their maximum potential. Examples of nursing interventions based on developmental periods are provided.

Quantitative and qualitative home assessment of primary caretaker-child interactions in lower socioeconomic families.

Assessment is an important component of nursing management of at-risk children. Low socioeconomic status and intrauterine growth retardation are often considered to be risk factors for developmental disabilities. Home visitation is often suggested as a way to assess adult-child interactions. In this article, the author provides information about assessments conducted in lower socioeconomic homes and difficulties encountered in conducting them. She raises several questions for consideration as a result of her efforts to assess home environments and primary caregiver-child interactions of children with intrauterine growth retardation.

Nurse case management in a rural parent-infant enrichment program.

Nurse case managers assist families with children with special health-care problems to receive services. This article describes the families and services provided in a parent-infant enrichment program in a rural setting. The interdisciplinary program encourages a parent-provider supportive case management model based on a realistic assessment of the family's ability to actively participate in the case management process. The program empowers families to become advocates for their children.

Case management for young children with special health care needs.

Case management for young children and their families with special health care needs is an organized system of health care. Fifty-eight families were surveyed to assess their experience with case management or determine their need for case management. Suggestions are provided for nurses to participate actively in this role.

Strategic planning for a Nursing Information System (NIS) in the hospital setting. Development of a Nursing System Model.

As nursing departments become more involved in automation, strategic planning for computer applications gains importance. Making the switch from manual to automated systems requires an understanding and analysis of the present system. This knowledge allows participants to examine the current information processing routines in relation to how well they support departmental goals. Priorities for projects can be established and alternatives for more efficient and effective information management schemes can be considered. The development of a Nursing System Model to serve this purpose is described. The model provides an overview for planning and identifies data classes created or used in nursing, organized according to the processes which act upon the data during the accomplishment of work activities.

Home management of URI in children with Down syndrome.

There is an increased risk of upper respiratory infection (URI) for the child compromised by Down Syndrome. Nurses can educate parents to care for the child experiencing mild URI without the need for medical intervention.

Cranial CT in children and adolescents with diabetic ketoacidosis.

To determine the effects of diabetic ketoacidosis on the brain, we studied nine consecutive patients between the ages of 6 and 17 years by performing cranial CT on three occasions: on admission, before treatment; 6-8 hr after beginning treatment; and 7 days after admission. Both the pretreatment scans and those at 6-8 hr showed significantly reduced size of the lateral and third ventricles in comparison with the 7-day scans. A comparison of the pretreatment scans with those done at 6-8 hr showed no statistically significant differences. We conclude that the cerebral swelling of diabetic ketoacidosis is usually present before treatment.

Nurse and parent collaborative case management in a rural setting.

A collaborative parent and nurse case management model can facilitate meeting the variety of needs of children who have special health care problems. When families live in depressed social and economic conditions in rural areas, services can be difficult to obtain. Families can be assisted to be advocates for their children by a close interaction with a nurse case manager.
