Alkalosis and renal excretion of ammonia by rat kidney.

Upon sulfate administration, UpH falls more in alkalotic rats than in controls. Alkalosis can lead to a reduction in UNH3 V at highly acidic urine. The significance of this process is doubtful at UpH ranging from about 6 to 7. At lower UpH less NH3 would be excreted, thereby less H+ would be trapped in urine and some acid would be conserved.

Isolation and structure of corticostatin peptides from rabbit fetal and adult lung.

A 34-amino acid peptide and three other structurally related peptides were isolated from rabbit fetal and adult lung. These cationic arginine- and cysteine-rich peptides inhibit corticotropin (ACTH)-stimulated rat adrenal cell corticosterone production. The peptide was called corticostatin (CSI). CSI was purified by reverse-phase HPLC and was shown to be homogenous from its amino acid analysis. Its sequence was determined on a gas-phase sequenator. The structure of CSI is Gly-Ile-Cys-Ala-Cys-Arg-Arg-Arg-Phe-Cys-Pro-Asn-Ser-Glu-Arg-Phe-Ser-Gly- Tyr-Cys - Arg-Val-Asn-Gly-Ala-Arg-Tyr-Val-Arg-Cys-Cys-Ser-Arg-Arg. CSI was found to markedly inhibit ACTH-stimulated corticosterone production by rat adrenal cells in vitro but did not affect basal levels. CSI did not affect the stimulation of aldosterone synthesis by angiotensin II in rat zona glomerulosa cells but it did suppress ACTH-stimulated aldosterone synthesis in whole adrenal cells, demonstrating that CSI is a specific inhibitor of ACTH-stimulated corticosteroid synthesis. The minimum effective concentration of CSI inhibiting ACTH-stimulated (33 pM) corticosterone production was 5 nM (20 ng/ml), the ED50 (50% effective dose) was 25 nM and steroidogenesis was completely inhibited at concentrations greater than 500 nM (2 micrograms/ml).

Outbreak of echovirus 11 infection in hospitalized neonates.

Between July 18 and August 5, 1986, a cluster of echovirus 11 infections occurred in hospitalized neonates. Ten infants were affected and one died. All cases occurring after the index case were infants who were in the nursery for at least 1 day when the index patient was also present. Risk factors for secondary infection included low birth weight or gestational age and receipt of antibiotics, red blood cell transfusions, nasogastric intubation or gavage feedings. Because viral infection had not been suspected in the index patient, isolation measures were not instituted until after onset of secondary cases. We conclude that more severely ill infants receiving intensive levels of care are at increased risk for nosocomial enteroviral infection. These infants may have a greater likelihood of exposure to the virus and/or increased host susceptibility. Outbreaks caused by cross-infection may be preventable by early recognition of patients colonized or infected with potentially pathogenic agents and prompt institution of appropriate isolation measures.

A new syndrome with features of the Smith-Lemli-Opitz and Meckel-Gruber syndromes in a sibship with cerebellar defects.

The Smith-Lemli-Opitz syndrome (SLOS) and the Meckel syndrome (MS) have been regarded as separate autosomal recessive entities. Recently, overlap of these two syndromes has been discussed. A sibship containing a probable new syndrome with features reminiscent of the SLOS and the MS is presented. The literature is reviewed with regard to the frequency of various malformations in these syndromes. Clinical manifestations and cerebellar abnormalities in these sibs are similar to those described in the Joubert syndrome (JS). These three cases may represent a new syndrome with features in common with SLOS, MS, and JS resulting from the same mutant gene, which exhibits considerable pleiotropy.

The corticostatic (anti-ACTH) and cytotoxic activity of peptides isolated from fetal, adult and tumor-bearing lung.

The possibility that corticosteroids influence lung maturation was suggested in 1968 by Buckingham et al. and supported by studies mounted in humans and in rabbits by Liggins and Howie and by a collaborative study from the N.I.H. To our knowledge no reverse process, i.e. the regulation of adrenal function by the lung, has been postulated. In attempting to isolate ACTH from fetal lung we found peptides which depressed corticosterone production. Eventually, we isolated a group of peptides from rabbit fetal lung which showed potent inhibition of corticosterone production in rat adrenal cell suspensions. Using high-resolution reverse-phase HPLC and high-sensitivity gas-phase sequencing techniques the primary structure of one of these peptides has been determined and it was called corticostatin. The adult rabbit lung contains a large quantity of a very similar peptide, which elutes close to corticostatin but whose amino acid composition differs by a single glycine and possibly a serine. This peptide is also potently corticostatic. We have isolated homologous human peptides but these show no apparent corticostatic activity in rat adrenal cell suspensions. They do, however, show interesting effects on in vitro growth of lung cell lines.

Postoperative hospital course of patients with history of severe psychiatric illness.

The postoperative hospital course of 54 patients with a past history of psychiatric illness was studied through chart review. Both chronic schizophrenics and chronic depressives tolerated surgical procedures well, without any unusual difficulties or exacerbation of psychiatric illness. They represented no management problems. Patients with acute, severe upset in the preoperative period (regardless of diagnosis) presented most of the management problems postoperatively.

Impotence and bicycling. A seldom-reported connection.

A man riding a stationary bicycle experienced transient tight sensations around the head of his penis during the exercise and progressive impairment of sexual potency over a period of more than one year. Lowering the bicycle seat terminated the bouts of impaired penile sensation, and one month after the patient discontinued the bicycle exercises, sexual potency returned. The course supported an ischemic mechanism for the abnormal penile sensation and a neural mechanism for impotence. Impotence has been associated with both vascular and neural lesions, and there is evidence for both vascular and neural compression in the perineal area during bicycle riding. A relationship between sexual dysfunction and bicycling may be more common than formerly suspected.

Logical memory subtest of the Wechsler Memory Scale: age and education norms and alternate-form reliability of two scoring systems.

The Logical Memory (LM) subtest of the Wechsler Memory Scale has been characterized by imprecise scoring instructions which can make data interpretation and study comparisons difficult. A total of 339 adults, from 18 to 83 years old, took either Form I or Form II of the LM. Verbal recall of the story passages was evaluated using gist and verbatim scoring systems. Interrater reliability was very high for both scoring approaches. The two forms were equivalent for gist recall. However, verbatim recall of Form I was more difficult than Form II because the former consists of more words to remember. Recall was related more to educational level than to age. For both gist and verbatim scoring, age and education norms were generated for immediate, delayed, and 24-h recall.

The headache of temporal arteritis.

Patients with temporal arteritis may be misdiagnosed because the headache does not affect the temples. The records of 24 patients with biopsy proven temporal arteritis were reviewed to determine the site and other qualities of headache. Past reports of the sites of headache in this disease were also reviewed. The temporal area was the sole site of headache in only six of 24 patients; the temple was included in pain that affected the frontal, vertex, and occipital areas in another seven patients. Two patients had generalized headaches. The headache did not involve the temple in seven patients and two patients did not experience headaches. Temporal arteritis should be considered in every elderly individual who begins to complain of headache, no matter what the site, quality, severity, or time pattern.

Selection of patients for clinical drug trials in migraine.

General concepts and specific factors to be used in the selection of patients for clinical drug trials in migraine are discussed. The definition of common migraine has been unsatisfactory and new diagnostic operational criteria are recommended. Patients with headaches that are a mixture of tension headache and migraine, and migraineurs who have interval headaches which are not clearly differentiated from migraine should be excluded. The headaches should be of moderate to severe degree, in the range of 2-6 per month, and should last from 3 h to 3 days. It would be best for migraine to have been present for at least 1 year, with 3 months retrospective and 2 months prospective observation prior to drug trial. The age of onset of migraine should be below 50, and the age of entry into the study less than 60. Migraineurs in good health, of either sex, are to be included in the study. Occasional use of minor tranquilizers and sedatives as well as of contraceptive drugs is acceptable, but patients who abuse drugs, who are allergic to compounds related to the trial drug and who require major psychotropic medication should be excluded. Also excluded are those whose compliance with the drug trial is doubtful for intellectual or, more often, psychological reasons.

Renal effects of atrial natriuretic factor in the rats of different ages.

Renal effects of an atrial natriuretic factor preparation were compared in 15, 28 and 66-day-old rats. This factor, prepared from atrial tissue of adult rats, was more effective in 28 and 66-day-old rats than in 15-day-old rats. There was a 6 fold increase of sodium excretion in 15-day-old rats and a 60 fold increase in 28-day-old rats. There was also a 15 fold increase in renal sodium excretion in 66-day-old rats receiving a higher dose (0.1 ml/animal). As indicated by the sodium potassium ratio, the increase in renal excretion of sodium was distinctly more pronounced than the increase in renal potassium excretion. In 15, 28 and 60-day-old rats, the increase of urine volume was 2 fold, 4 fold and 5 fold, respectively. The increase of fractional sodium excretion (FE) in rats receiving an atrial factor preparation was distinctly more pronounced than the increase of GFR. In all experiments, the preparation from ventricular tissue of the same animals was ineffective in producing natriuresis or diuresis.
