Tissue plasminogen activator catalyzed Lys-plasminogen activation on heparin-inserted phospholipid liposomes.

We prepared heparin-inserted phospholipid liposomes as a functional model of heparan sulfate present on the vascular surface and examined tissue plasminogen activator (t-PA) catalyzed plasminogen activation on the liposome surface. Kinetic analyses showed a marked increase in the affinity of t-PA for Lys-plasminogen in the presence of heparin-inserted phosphatidylcholine (PC) liposomes. The catalytic efficiency (kcat/Km) of t-PA for the plasminogen activation on the surface of heparin-inserted PC liposomes was 5.4 times that on the surface of heparin-free PC liposomes. This stimulatory action of immobilized heparin was apparently affected by changing the phospholipid component of liposomes. Phosphatidylethanolamine or stearylamine, having a positively charged group, reduced the catalytic efficiency of t-PA by raising its Km value (10-fold), whereas negatively charged phospholipids, phosphatidylserine and phosphatidylinositol, did not affect the efficiency. t-PA and generated plasmin bound to the liposome surface heparin were protected from inhibition by plasminogen activator inhibitor type 1 and alpha 2-plasmin inhibitor, respectively. t-PA-induced clot lysis of euglobulin or whole plasma, which contained native (Glu-) plasminogen and the above inhibitors, was also accelerated by addition of heparin-inserted PC liposomes. These results suggest that the vascular surface heparin-like molecules may play an important role in modulating fibrinolytic events. The principles of conjugation of t-PA with a biologically active liposome will be applied to the construction of better thrombolytic agents.

Inhibition of proline endopeptidase activity by acyl-coenzyme A esters.

Coenzyme A (CoA), its related compounds and acylcarnitine non-competitively inhibited the activity of proline endopeptidase (PEPase) purified from rat liver cytosol. The degree of inhibition was in the order of acyl-CoA greater than CoA greater than dephospho-CoA greater than or equal to acylcarnitine. However, carnitine did not inhibit the enzyme activity. Among the compounds examined, n-decanoyl-CoA showed the highest inhibitory activity (Ki = 9 microM). These results suggest that both the acyl group and CoA contribute to the inhibition of PEPase by acyl-CoA. The abilities of n-decanoyl-CoA and its related compounds to quench the intrinsic fluorescence at 332 nm from PEPase excited at 280 nm, was used as a probe for the binding affinity of the enzyme for these compounds. The quenching of fluorescence by CoA was nearly equal to that by n-decanoyl-CoA. n-Decanoylcarnitine and carnitine were unable to quench the fluorescence. These results indicate that n-decanoyl-CoA at least binds to PEPase through its CoA portion.

Experimental studies on spinal cord function using evoked action potentials.

Experiments were carried out on cats to determine the use of conductive evoked spinal cord action potentials in diagnosing motor function of the spinal cord. Direct stimulation from the dura produced three negative wave potentials, N1, N2 and N3. The intraspinal pathway of N2 and N3 was the dorsal column. The pathways of N1, determined by dorsal and ventral epidural recording, were the dorsilateral funicle and the extrapyramidal tracts. A collision experiment between potential N1 and pyramidal tract action potential did not reflect the function of the tract as the amplitude of the action potential was too small. Nevertheless, it is considered that conductive evoked spinal cord action potentials could become a valuable method of assessing spinal cord function as they reflect the function of the extrapyramidal tracts, as well as of the dorsilateral funicle and the dorsal column.

The biological characteristics and chemosensitivity of anaplastic thyroid carcinoma transplanted into nude mice.

Three xenografts established from three patients with anaplastic thyroid carcinoma were investigated for their biological characteristics and chemosensitivity. The histological and immunohistochemical findings of these tumors were almost the same as those of the original tumors. Although the growth rate of each xenograft was constant, the tumor doubling time varied from 4.8 per 9.0 days, and the labeling indexes, determined using bromodeoxyuridine pulse labeling, varied from 11.4 to 25.1 per cent. The chemosensitivity tests were performed according to the Battelle Columbus Laboratories Protocol, with adriamycin, cyclophosphamide, cisplatin, mitomycin C and tegafur administered intraperitoneally to tumor-bearing nude mice in maximum tolerable doses. Tumors with slower growth rates tended to be sensitive to more drugs. Furthermore, cyclophosphamide showed antitumor effects against all the tumors tested. Although previous treatments of the original tumors may have affected the results, our results suggest that a more suitable chemotherapy for anaplastic thyroid carcinoma could be developed.

The effect of intense arterial constriction on the survival length of an island flap; an experiment using abdominal flaps in rats.

Using rats' abdominal skin flaps, the effects of markedly decreased arterial inflow was investigated. An intense constriction was created with laminated silastic tubes. Survival length of the island flap was reduced by this manipulation and the differences with the control were statistically significant. It was ascertained experimentally that the flap went into necrosis distally and progressed proximally as the arterial inflow decreased. The possible risks of the sleeve technique are also briefly described.

Scrotal septocutaneous island flap for the reconstruction of the urethral fistula.

We report the reconstruction of the urethral fistula using a scrotal septocutaneous island flap. A scrotal septocutaneous island flap is supplied by vascular networks in the scrotal septum area from both perineal arteries and posterior scrotal arteries. This flap is divided into two flaps, one for urethral lining and the other for the skin defect. We found it to be a simple, useful, and reliable alternative to other local flaps. However, hairless skin of the scrotum must be used for the urethral lining. The musculocutaneous flap is also reliable. However, use of the scrotal septocutaneous island flap can save the musculocutaneous flap for secondary procedures.

Nylon threads used as drains in free skin grafting.

Monofilament nylon threads are used as drains in free skin grafting; 2-0 or 3-0 nylon threads are usually applied. Nylon drains are available in any operating room, and have no detrimental effects on the grafted skin. The drainage effect of nylon threads is shown in clinical cases and in a simple experimental model. The effect may not be dramatic, but it reduces the volume of hematomas and brings good results in free skin grafting.

Avulsion of the scalp: which one is the best artery for anastomosis?

We present a patient who underwent successful replacement of a large avulsed scalp with microvascular anastomosis of one artery and three veins. The left occipital artery in the avulsed scalp and a frontal branch of a right superficial temporal artery in the recipient forehead were anastomosed. Three veins in the occipital area in the avulsed scalp were anastomosed via vein grafts to three veins in the frontal area, respectively. For successful replacement, the selection of the vessels depends on each individual case. To give easy anastomosis, a good cosmetic result, and a comfortable postoperative period, it is not always necessary to reconstruct a previously normal anatomical position.

Osteocutaneous flap from the big toe for repair of osteomyelitis of the second toe.

The free vascularized osteocutaneous flap from the big toe has been offered as a solution for the reconstruction of the distal phalanx of the fingers. As another important application of this flap, it is recommended that the flap pedicled with the plantar vessels can easily cover the defect involving the bone of the second toe.

Combined tensor fasciae latae musculocutaneous flap and sartorius musculocutaneous flap for the repair of wide defects of the lower leg.

The tensor fasciae latae musculocutaneous flap has great advantages for reconstruction of the abdominal wall, but the medial border of its territory is limited to the thigh. In order to expand the territory, a combined tensor fasciae latae musculocutaneous flap and sartorius musculocutaneous flap was devised. This flap was successfully used to resurface a large defect in the lower leg as a distally based musculocutaneous flap. The advantages of this flap are its extremely large territory, the fact that total necrosis of the flap cannot occur, and that as a proximally or distally pedicled flap it is suitable for large defects in the abdominal wall, lower leg, and gluteal region.

A new method for the dressing of free skin grafts.

Using adhesive drapes and a disposable suction drain, a new method for the dressing of free skin grafts has been devised. The graft is compressed by pressure that is equivalent to the negative pressure of the suction drain. This method can apply uniform and constant pressure on the graft. Moreover, the graft can be observed through a transparent drape so that the existence of hematomas can be detected easily.

Ultrasonically guided biopsy of nonpalpable lesions of the breast by the spot method.

A new biopsy method by injecting dye under ultrasonic guidance is most efficient and accurate for nonpalpable lesions revealed by ultrasonography. It can also minimize the amount of tissue that is resected. The conventional method by needle-wire localization has to be used in instances of calcification which cannot be defined by ultrasonography.

Further characterization of the binding of plasminogen to heparin: evidence for the involvement of lysine residues.

We have previously demonstrated that the heparin-binding site of plasminogen is located in Val442-plasminogen region (kringle 5 domain plus light (B) chain) (Soeda, S., Kakiki, M., Shimeno, H., and Nagamatsu, A. (1987) Biochim. Biophys. Acta 916, 279-287). The chemical modification of Val442-plasminogen with a lysine reagent, pyridoxal 5'-phosphate (PLP), and sodium borohydride resulted in the incorporation of 8-10 PLP moieties per molecule of the zymogen. This PLP-labeled zymogen had no affinity for a heparin-Sepharose column, whereas the non-labeled one bound to the column. Modification in the presence of heparin decreased the extent of labeling by 1-2 mol of PLP per mol of Val442-plasminogen. To further examine the binding site of plasminogen to heparin, functionally active A and B chains were separated from Lys-plasmin after mild reduction and S-carboxymethylation. Only B chain possessed affinity for heparin-Sepharose. Furthermore, plasmin(ogen) bound to heparin was protected from alpha 2-antiplasmin inhibition. These results indicate that one or two lysine residues located in the catalytic region (B chain) of plasmin(ogen) are essential to heparin binding, and that the binding of plasminogen to heparin or heparin-like substance in extracellular matrix environments may be important for the localization and activation of plasminogen and for the prolongation of the resultant plasmin activity.

Study of cell kinetics in anaplastic thyroid carcinoma transplanted to nude mice.

The cell kinetics of the transplantable human anaplastic thyroid carcinoma were studied in athymic nude mice using bromodeoxyuridine (BrdU) pulse labeling. In order to estimate the cell cycle time in this study, the percent labeled mitosis curve was prepared with immunoperoxidase staining using monoclonal antibody to BrdU instead of 3[H]thymidine autoradiography. This technique was considered to be very useful because it can be done for a short term and can still be changed to the autoradiographic method. Comparing the results in this study with already reported data on various xenografts of human carcinomas in nude mice, the tumor doubling time (117.5 hr) and the cell cycle time (23.5 hr) were rather short, but the growth fraction (54%) and the cell loss factor (68%) were nearly equivalent to the median value. These findings suggest that the very rapid growth rate of this tumor was mainly due to the shortness of the cell cycle time.

A case of metastasis from gastric cancer to the thyroid gland.

Metastatic tumors in the thyroid gland are rarely seen in clinical practice. This report describes a case of metastasis from gastric cancer to the thyroid, found five years after removal of the primary gastric lesion. The patient had a large thyroid mass extending to the mediastinum, but there were no obvious metastatic lesions anywhere except in the thyroid. Subtotal thyroidectomy was performed and histological examination revealed the same findings a those of the original gastric cancer. Of additional interest are the findings that led us to believe this metastatic tumor produced alpha-fetoprotein. Seven months following the operation, the patient dies suddenly and, although it is difficult to say whether the patient's survival was prolonged, we believe that the thyroid surgery gene him considerable palliation.

A giant stone in the müllerian duct cyst.

Cystic diseases in the male pelvis are uncommon, and stones found in male pelvic cysts are very rare. We report a case of a giant stone found in the male pelvis. Computed tomography was helpful in locating the stone, and surgery revealed it to be a Müllerian duct cyst.

Inferior epigastric artery skin flaps without rectus abdominis muscle.

The rectus abdominis musculocutaneous flap has many advantages, but its disadvantages are also well-known. These are the possibility of abdominal herniation and, in certain situations, its bulk. To overcome these problems, an inferior epigastric artery skin flap without rectus abdominis muscle, pedicled on the muscle perforators and the proximal inferior deep epigastric artery, have been used in two patients. A large flap without muscle can survive on a single muscle perforator.
