Visual cell profiles for quantitation of premalignant cells in sputum: a preliminary report.

Visual cell profiles were used to analyze the distribution of atypical bronchial cells in sputum specimens from cigarette-smoking volunteers, cigarette-smoking asbestos workers and cigarette-smoking uranium miners. The preliminary results of these sputum visual cell profile studies have demonstrated distinctive distributions of bronchial cell atypias in progressive patterns of squamous metaplasia, mild, moderate and severe atypias and carcinoma, similar to those the authors have previously reported using cell image analysis techniques to determine an atypia status index (ASI). The information gained from this study will be helpful in further validating this ASI and subsequently achieving the ultimate goal of employing cell image analysis for the rapid and precise identification of premalignant atypias in sputum.

Interrupted treatment and forced terminations.

A discussion of "Premature Termination: A Therapist Leaving," by James Beatrice, M.S. The treatment literature offers a plethora of information regarding the beginnings of psychotherapy, but the process of termination is given relatively little attention. The issue of forced terminations, which bring about premature endings, interrupting the treatment in ways often harmful to patients and adding to the negative side of the ambivalence about seeking further help, is even more neglected. This paper examines the nature of forced terminations when they are invoked by the therapist and spells out the variety of circumstances and motivations sparking such endings. Finally, it addresses the nature of forced terminations initiated by patients, and details the conscious and unconscious factors likely to be at work.

Monitoring of IMZ titanium endosseous dental implants by glycosaminoglycan analysis of peri-implant sulcus fluid.

Determination of glycosaminoglycans (GAGs) in peri-implant sulcus fluid (PISF) may monitor tissue changes around implants. This study investigated osseointegrated IMZ titanium implants at five stages from initial exposure to occlusal loading from prostheses. Two peri-implant sulcus fluid glycosaminoglycans (PISF GAGs), hyaluronic acid (HA) and chondroitin-4 sulfate (CS), were determined by cellulose acetate electrophoresis and densitometric scanning. Compared to longer-serving implants, CS contents at early stages was higher and with full occlusal loading was three times greater, without significant changes in HA contents. These observations appeared to reflect predominantly responses in the supporting bone, particularly when compared to our previous studies on ceramic implants and teeth affected by periodontal disease. This study supports the potential of PISF GAG analysis to detect adverse tissue responses, notably bone resorption.

Premature ovarian failure associated with autoantibodies to the zona pellucida.

Immune disturbances are noted in up to 35% of women with premature ovarian failure. Previous investigators have focused on the presence of antibodies directed against steroid secreting cells of the ovary and the presence of lymphocytic infiltration in ovarian biopsy specimens. The authors report two cases of premature ovarian failure associated with antibodies directed against the zona pellucida. One of these two patients conceived while on estrogen replacement therapy.

erbB-2 and myc oncoproteins in sera and tumors of breast cancer patients.

This study compares the prevalence of elevated serological levels of erbB-2 and myc proteins in 36 breast cancer patients and 25 healthy, ambulatory female controls. The controls were frequency matched to the cases by age and ethnicity. Oncoprotein levels were determined blind to the "case-control status" of the individual from whom the specimen was derived. Corresponding tissue levels were examined in tumors of the 13 cases from whom sufficient tissue was available. Serum oncoproteins were elevated as follows: erbB-2 in one control (4%) compared with nine cases (25%; PFisher's exact = 0.03); myc in no control (0%) compared with seven cases (19%; PFisher's exact = 0.02). Elevated serum levels of erbB-2 or myc oncoproteins were detected in four of the seven cases (57.1%) of in situ cancer without evidence of infiltration. In all cases with elevated serum oncoproteins where tumor tissue was available, the corresponding protein was elevated in the tumor. The three cases who had elevated preoperative serum oncoprotein levels and from whom it was possible to procure postoperative specimens had normal postoperative serum oncoprotein levels. We conclude that (a) erbB-2 and myc oncoproteins are elevated in a proportion of breast cancer patients, (b) the tumor seems to be the source of the serum elevation, and (c) these proteins may be useful as part of a panel of biomarkers of early malignant disease.

A randomized vehicle-controlled trial of topical capsaicin in the treatment of postherpetic neuralgia.

A large double-blind, vehicle-controlled study of 143 patients with chronic postherpetic neuralgia (PHN) was performed to evaluate the degree of efficacy of topically applied capsaicin 0.075% cream. In addition, the safety and efficacy of long-term application of topical capsaicin in PHN was assessed by following patients in an open-label study for up to 2 years. In the double-blind phase, 143 patients with PHN of 6 months' duration or longer were enrolled. Since epidemiologic studies of patients who receive no treatment have shown that only 10% to 25% of those with PHN after 1 month will still have pain at 1 year, two separate efficacy analyses were performed: one with all evaluable patients (n = 131) and the other with 93 patients whose PHN lasted for longer than 12 months prior to study startup. All efficacy variables, including the physician's global evaluation of reduction in PHN pain, changes in pain severity on the categoric scale, visual analogue scale for pain severity, visual analogue scale for pain relief, and functional capacity scale, showed significant improvement at nearly all time points throughout the study for both patient groups, based on duration of PHN pain. In contrast, the group receiving vehicle cream remained essentially unchanged. Data from the long-term, open-label phase (up to 2 years, n = 77), which immediately followed the 6-week blinded phase, showed that the clinical benefit in patients treated for a short (6-week) period with topical capsaicin could be maintained or amplified in most patients (86%) during prolonged therapy. There were no serious adverse effects observed or reported throughout the trial; in fact, the only side effect associated with capsaicin treatment was the burning or stinging at local sites of application (in 9% of patients) during exposures of up to 2 years (long-term phase). On the basis of these data, we conclude that capsaicin 0.075% cream is a safe and effective treatment for the pain of postherpetic neuralgia and should be considered for initial management of patients with this condition.
