Radiographic study of gastric hyperplasia induced by polychlorinated biphenyls in the rhesus monkey.

Polychlorinated biphenyls (PCBs) are a group of compounds which have many industrial uses and have become an environmental pollutant. In the rhesus monkey, an unusual type of gastric hyperplasia has been produced by feeding the animals small amounts of these chemicals. By using a double contrast gastrogram with simultaneous pneumoperitoneum, we studied the development of the lesions radiographically. Over a period of several months the gastric abnormality progressed from minimal thickening of the gastric wall, with normal appearing mucosa, to extensive nodularity and irregularity in the area of involvement. This technique provides a method for studying the characteristics of the gastric hyperplasia over an extended time, and therefore has several advantages over autopsy studies or surgical biopsies. Although no similar lesions related to PCB exposure have been seen in humans, no survey of possible gastric changes has been done among persons exposed to these materials.

Carbon dioxide laser resection of superficial oral carcinoma: indications, technique, and results.

A retrospective medical record review was done of 51 consecutive patients with 56 biopsy-proven squamous cell carcinomas of the oral cavity and oropharynx treated for cure by one of the authors (H.H.D.) using the carbon dioxide laser via microdirect pharyngoscopy. All 51 patients were followed for at least 3 years unless they died, and 27 were followed for 5 years. The tumors treated were superficial, accessible lesions without bone involvement or clinically apparent metastases. Eighteen patients had had irradiation or surgery at the same site, so were having salvage CO2 laser surgery. Indications and technique are discussed. Local recurrence and disease control rates were comparable to those of conventional local surgical excision at 3 and 5 years. Complication rate was low. Carbon dioxide laser resection with microscopic control is, we propose, an excellent modality for transoral excision of carefully selected lesions, and it offers increased precision, better visualization, minimal bleeding, and decreased postoperative morbidity compared to other surgical techniques.

Rationale for the potential use of calcitonin in osteoarthritis.

This review provides evidence that osteoarthritis (OA) or a major subset of OA is not only a disease of cartilage but also a disorder of subchondral bone. This review also discusses the potential efficacy of a bone and cartilage active agent, calcitonin, and discusses how calcitonin might be useful in the pharmaceutical treatment of OA.

Findings in 50 AIDS virus-infected patients with positive oral Candida cultures.

Oral candidiasis is a common complication of HIV-infected-individuals. The purpose of this study was to evaluate clinical and laboratory findings to assess the impact and efficacy of antifungal treatment. This preliminary report describes findings in 50 HIV-positive, candida culture-positive subjects (49 males, 1 female; mean age: 39 years). The group had been known HIV-positive for a mean of 28 months, and 19 met the CDC/WHO definition for AIDS (63%, KS, 21% PCP). Thirty-four of the fifty patients had oral signs of candidiasis, with almost half having both atrophic (red) and pseudomembranous (white) components. In quantitating the cultures, the higher colony forming unit counts in general were correlated with clinical signs and pain. The other most common oral manifestations were periodontal disease, hairy leukoplakia and xerostomia. The most common candida species was albicans (84%). Response to initial antifungal therapy was satisfactory clinically, but erratic regarding CFU quantitation, species changes, and bacterial emergence. In summary, oral candidiasis is a complex infection with uncertainties as to the significance of quantitation and achieving control.
