Iloprost, stable analogue of the prostacyclin, is able to improve the tissue resistance to ischaemia.

On 10 patients, suffering from peripheral arterial disease, with critical limb ischaemia, the CO2 production during ischaemia has been evaluated at 1st and at 28th day of treatment with iloprost. The study demonstrated that the drug is able to improve the tissue resistance to ischaemia, with a significant (p < 0.05) reduction of the CO2 production. The authors underline that this study is one of the first confirmations, in vivo and on the man, of the previous experimental findings, made in isolated arterial tissue.

Cefonicid distribution into tonsillar tissue.

The kinetic profile in tonsils of cefonicid has been studied in 30 patients who underwent tonsillectomy after administration of a single intramuscular dose of 1 g. Blood and tissue samples were withdrawn 1, 2, 3, 6, 12 and 24 h after administration of the drug. The analytical determination was performed employing a microbiological method. Peak serum levels appeared at the first hour (94.2 +/- 9.83 mg/l), while peak tissue values were determined in samples collected at the third hour (11.9 +/- 3.68 mg/kg). Serum levels at the 24th hour were 3.82 +/- 1.25 mg/l. Levels in the tonsils were 7.54 +/- 2.96 mg/kg at the sixth hour, 3.34 +/- 1.42 mg/kg at the twelfth hour and 0.40 +/- 0.31 mg/kg at the 24th hour.

Primary non-Hodgkin's lymphoma of the esophagus.

Esophageal involvement of non-Hodgkin's lymphomas is extremely unusual; primary lymphoma of the esophagus is even less common. This report describes a case of malignant small lymphocytic-plasmacytoid lymphoma with primary esophageal localization. Endoscopic diagnosis was confirmed by histological examination of a large portion of tumoral tissue spontaneously expelled after esophagogastroscopy. Lymph nodes, bone marrow, and other gastrointestinal sites were not involved in the disease. We describe the clinical history of the patient, with the remissions induced by chemotherapy, over a 5-yr observation up to the patient's death, which was not directly related to the tumor.

Effects of mesoglycan sulfate on the arterial elastic module.

Ten patients with peripheral obstructive arterial disease (POAD) in stages I and II according to Leriche-Fontaine, were subjected to therapy with mesoglycan sulfate (60 mg/day for twenty days), to evaluate the effect of the drug on the elastic module of the arterial wall. The wall elasticity was deduced from some Doppler velocitographic indices (arterial dynamics index, resistance index, perfusion pressure index, tibial distensibility index); from the analysis of systolic, protodiastolic, and end diastolic velocity variations; and from computerized analysis of the Doppler sound spectrum. The results show a significant improvement of arterial wall elasticity, which suggests a rational use for the drug in the initial stages of peripheral obstructive arterial disease.

Doppler test after microvascular anastomoses and evaluation of somatostatin administration in segmental canine pancreas autotransplantation. A preliminary study.

Segmental pancreatic transplantation still involves a certain number of possible technical complications. The experimental studies in dogs reported here evaluate the effectiveness of vascular microanastomoses using doppler velocimetry in two groups of dogs which underwent segmental pancreatic autotransplantation. One group was protected by intraoperative somatostatin infusion, while the other was kept as control. The obtained results indicate that the doppler-test revealed early vascular complications (2/20 animals) and that somatostatin administration may play a major role in the prophylaxis of pancreatitis.

[Hemorheological changes during exercise].

It is now necessary to study the macro- and microcirculatory modifications of the organism following the stress. We have devoted our attention to the hemorheological parameters, which have been evaluated in a group of ten sedentary subjects, of fourteen athletes under twenty years of age, and of ten subjects whose age was over thirty, who carry on physical unbroken performance. We have evaluated the modifications of total blood viscosity and of red blood cell deformability after an ergometric test. Our results show that the hemorheological pattern gets worse after strenuous muscular exercise; moreover, the advancing age gets worse the hyperviscosity response. The pathophysiological mechanism which causes the hemorheological disorder is yet unknown, so it is clear the importance of studying the hemorheological balance in order to clarify the pathophysiology of stressed muscular function.
