When K562 cells were made resistant to cisplatin their neutral amino acid transport systems changed. K562 cells cloned in cisplatin developed a 6.7-fold resistance to the drug. The generation times for K562 cells sensitive (S) and resistant (DDP) to cisplatin were similar. The initial uptake and rapid efflux of cisplatin were similar in both cell lines. However, they differed in their sodium dependent neutral amino acid transport properties. In K562S cells the sodium-dependent uptake for methylaminoisobutyric acid was significantly inhibited by threonine (Ki = 10.3 mM), but in K562DDP the uptake of neutral amino acids was significantly lower, and methylaminoisobutyric acid uptake was minimally inhibited by threonine. When K562DDP cells were grown in the absence of cisplatin for 8 weeks, their amino acid transport properties reverted to those of K562S cells. In K562 cells, changes in plasma membrane essential amino acid transport systems were concomitant to both the development of resistance and the redevelopment of sensitivity to cisplatin. Therefore, human malignant cell sensitivity and resistance to cisplatin may be related to membrane nutrient transport systems.
Fifty-seven cases of intrahepatic stone were treated in our department from 1976 to 1984. Fifteen cases out of them were left and right type intrahepatic stone associated with the proximal bile duct stenosis. Treatments and results of these 15 cases are discussed. Percutaneous Transhepatic Cholangioscopy (PTCS) was performed in 10 cases, post operative cholangioscopy (POC) in 7 cases. These cases were divided into three groups according to the treatment received. Six patients were treated only with cholangioscopic lithotomy (Group I). Six patients underwent surgical treatment after cholangioscopic lithotomy (Group II). Other 3 patients were treated with POC (Group III). Clinical course of Group I: One patient died of recurrent liver abscess 5 years after PTCS. One died of another disease. One had suffered from empyema 5 years after PTCS, but he is doing well now. Other 3 patients are doing well now. Clinical course of Group II: All patients are doing well now. Clinical course of Group III: All patients underwent treatments for residual stones after POC. Good results were obtained in group II. Surgical treatment after cholangioscopic lithotomy is the best way for these 15 cases. Adequate operative procedure should be applied after improving the patients' condition and making an accurate diagnosis of biliary tract anomalies by means of PTCS.
Reevaluation was carried out on the surgical treatment for clinically benign thyroid nodules. One hundred and thirty-seven patients underwent conservative resections of the thyroid because of preoperative impression of benign nodules at the First Department of Surgery, Nagoya University Hospital from 1970 to 1984. Permanent paraffin sections of the resected specimen revealed that the nodules in 4 patients were intrathyroidal cancer (3 papillary and 1 follicular) and the nodules in the other 8 patients were associated with an unsuspected small (3 approximately 15 mm) thyroid cancer. All those patients underwent either lobectomy or subtotal thyroidectomy at the initial surgery except for one patient in whom enucleation was performed. No further intervention was carried out when the definitive diagnosis was made, except for one patient in whom the enucleation was followed by lobectomy. No neck dissection was attempted in all of them. Patients have been living and well without evidence of recurrence for 3 to 11 years and one died of unrelated disease. From these results, it is recommended that lobectomy is the least requirement in treating clinically benign nodule and that, when paraffin section reveals the tumor is intrathyroidal cancer, no further surgery is justified unless there are other foci of cancer in the resected lobe.

Recently the technique of imaging diagnosis has been developed and biliary tract carcinomas without any symptoms can be diagnosed by US, CT and cholangiography, but preoperative definitive diagnosis should be carried out by percutaneous transhepatic cholangioscopy (PTCS) or percutaneous transhepatic cholecystoscopy (PTCCS) especially in cases of minute lesions, superficial spreading type carcinomas or biliary tract carcinomas complicated with gall stones. Operative procedures, such as hepatectomy, bile duct resection and/or pancreatoduodenectomy, have to be decided before the operation according to the extent of cancer which is diagnosed by percutaneous transhepatic cholangioscopic biopsy.
Selective middle hepatic venography was performed in 86 patients who had been scheduled to undergo liver resection because of hepatobiliary diseases. Special catheters were used of which tips were bended counterclockwise at an angle of 45 degrees. Successful middle hepatic venograms were obtained in 65 cases (76%). Abnormal findings were observed in 27 cases (42%), and it was difficult to diagnose them by ultrasonography or computed tomography. Selective middle hepatic venogram is very useful to understand surgical anatomy of the liver in each case preoperatively. In 5 cases of giant tumor, middle hepatic venography is necessary to identify the exact site of the tumor. Selective middle hepatic venography is considered to be one of the indispensable, examinations for liver resection.

A 41-year-old man was admitted to the hospital complaining of back pain and progressive jaundice. Ultrasonography, CT and hypotonic duodenography revealed a large and well-defined tumor in the head of pancreas. The tumor was hypervascular on angiography. Total pancreatectomy was performed, and the examination of the resected specimen disclosed that the tumor was 50 X 45 mm in size and extrapancreatic tumor-forming type. Histopathologically, the tumor comprized mostly bizzare mono- and multinucleated giant cells with sarcomatous growth pattern. The patient died 6 months after operation due to liver metastasis and peritoneal dissemination. The pathological feature of the pleomophic carcinoma of the pancreas is well-known, but the clinical feature is vague because the resected cases are rare. The pathological finding of the specimen of this case is typical and it is considered that the clinical findings of this case, such as well-defined and hypervascular tumor, are characteristic of the pleomorphic carcinoma of the pancreas.
Palliative treatment have been performed in 50 cases of unresectable pancreatic carcinoma during these nine years. There were 36 in the head and 14 in the body and tail on the location of the tumor. Operation was performed in 35 cases (70%) and about 60% of which were diagnosed as unresectable for liver metastases. Prophylactic gastroenterostomy concomitant with biliary bypass is considered to be the most suitable procedure and should be performed routinely regardless of duodenal obstruction. Choledochojejunostomy (Roux-en-Y) and gastrectomy (Billorth-II) were considered advantageous for favorable long-term bile drainage and to prevent hematemesis. Splanchnicetomy proved to alleviate pain in 87.5% and is recommended to perform at the time of initial laparotomy. Postoperative adjuvant chemotherapy was performed in 40% of the cases. Arterial infusion chemotherapy was found most effective to prolong survival. The patients who received palliative treatment survived 6.5 months on the average: Mean survival was 7.3 months in the former cases and 3.8 in the latter. Six patients (12%) survived more then one year, and no significant difference in location or size could be found.
Sera obtained from seven normal volunteers and 11 patients with aplastic anemia were assayed for two types of colony-stimulating activity (CSA) using monolayer agar cultures containing human unfractionated and monocyte-depleted bone marrow cells. Sera obtained from normal volunteers had low CSA for unfractionated bone marrow cells and no CSA for monocyte-depleted bone marrow cells. On the other hand, sera obtained from patients with aplastic anemia (patients' sera) had moderate CSA for both unfractionated and monocyte-depleted bone marrow cells. The patients' serum CSA titer for unfractionated bone marrow cells rose markedly with the addition of a small amount of diluted control rabbit serum (control serum) into the CSA assay system, while it did not rise with addition of the same amount of diluted rabbit antiserum against partially purified human urinary colony-stimulating factor (CSF) (antiserum). The patients' serum CSA titer for monocyte-depleted human bone marrow cells rose with the addition of control serum as well as antiserum. These findings indicate that the addition of rabbit serum enhances colony formation by human bone marrow cells in the presence of aplastic anemia sera as a source of CSA, and that sera of patients with aplastic anemia contain two antigenically different types of CSA: one that is active on human unfractionated bone marrow cells and partially neutralized by the addition of antiserum, and another that is active on human monocyte-depleted bone marrow cells and is not neutralized by the addition of antiserum.

A case of T cell chronic lymphocytic leukaemia (CLL) with red cell hypoplasia and neutropenia is reported. WBC was 10.0 X 10(9)/l with 78% being T lymphocytes. These T lymphocytes were positive for Fc gamma receptor and had OKT8 and Leu 2a antigens on the cell surfaces. They suppressed both erythroid and granulocyte-macrophage colony formation in normal bone marrow cultures and suppressed immunoglobulin production by normal B lymphocytes in vitro. Though the myeloid cells were preserved in the bone marrow and the values of serum immunoglobulins were within normal limits, it can be estimated from these results that red cell hypoplasia and neutropenia were partially due to the suppression of haemopoietic precursor cells by leukaemic T lymphocytes.
In the present study the technique and the reliability of indirect blood pressure measurement at the common femoral level was described. The blood pressure was measured by using a long cuff, which encircled the hips at the level of the groin, and determining the flow signals by Doppler technique applied at the ankle. In order to transmit the cuff pressure to the artery, a slender bag with air, which was a blood pressure cuff used in children, was placed over the common femoral artery under the cuff. A 14 cm. wide cuff-bladder was usually used, but in obese subjects with the size of hips more than 90 cm, a 16 cm. wide cuff-bladder was applied. In patients with collateral vessels around the groin due to the iliac disease, the blood pressure could be measured by placing the Doppler flat probe over the common femoral artery or the collateral vessel. In observation of 24 subjects, a close correlation was obtained between the femoral cuff pressures and common femoral intra-arterial pressures. Furthermore, blood pressure values at the common femoral level measured indirectly showed similar reproducibility as those at the ankle and toe levels.

During the last 8 years, arterial reconstructive operations such as bypass grafting, thrombendarterectomy and percutaneous transluminal angioplasty were performed on 282 lower extremities with chronic arterial occlusion. Operative indications were intermittent claudication in 57.1% and limb salvage in 42.9%. Early and late failure rate following operations was 31.2%: for claudication group 24.0% and for limb salvage group 40.7%. In 30.6% of claudication group and in 45.7% of limb salvage group, ischemia of limbs, compared to their preoperative status, worsened following occlusion. These limb worsening-rates were high and not negligible. Main causes of failures were poor run off and poor graft. Because chronic arterial occlusion is not a malignant disease, it is not forgivable to worsen limb status by surgery. Therefore, the operative indication for claudicant should be the need of patients in their daily life. For femoro-popliteal bypass grafting in claudicant, not artificial graft but auto vein graft should be used. When no suitable vein is available, conservative therapy is the choice of treatment. In case of limb salvage, artificial graft may be used.
