Comparison of the effects of leaving needle, direct current electrical acupuncture, and low-frequency electrical acupuncture therapy.

During a 3 year period from 1978 to 1980, one hundred and seventeen patients who came to the Pain Clinic of the Department of Anesthesiology of the Osaka Medical College complaining of pain, were treated with leaving needle(LN), direct current electrical acupuncture (EAP), or low-frequency electrical acupuncture(LFEA). The immediate effects of these therapies were investigated in a period of 3 days after treatment, on the basis of the patient's subjective evaluation of his relief from pain on a scale of 10 before treatment. Appearance patterns of therapeutic effects of these therapies were divided into four types: persistent (therapeutic effects persist during the 3 day period immediately after treatment), downward (therapeutic effects gradually decrease immediately after treatment), upward (therapeutic effects gradually appear after treatment), and unaltered (therapeutic effects were fair or poor during the 3 days after treatment). In addition, it was seen that of the three therapies LFEA was the best one in producing a persistent therapeutic effect and a good rate of effectiveness in the first day immediately after treatment.

[Intracranial hemorrhage associated with pregnancy--4 case reports and a review of the literature].

Four cases of intracranial hemorrhage associated with pregnancy are reported. They were a ruptured anterior communicating aneurysm, a ruptured AVM of cerebellar hemisphere, a lt-thalamic and putaminal hemorrhage and brain stem hemorrhage. As the mode of hemorrhage, one case was a subarachnoid hemorrhage, one case was a subarachnoid hemorrhage combined with a cerebellar hemorrhage and other two cases were intraparenchymal hemorrhage. The mean age of these 4 cases was 29.5 years of age from 25 to 35. One case was occurred the intracranial hemorrhage during the first trimester but other 3 cases was occurred during the third trimester. It was, thus, considered the intracranial hemorrhage associated with pregnancy was more frequently occurred during the third trimester. Primigravida and multigravida were equally occurred on each two cases. It was not related to parity. Consciousness levels were drowsiness; one case, semicoma; two cases and deep coma; one case. A case of ruptured anterior communicating aneurysm underwent neck clipping of aneurysm through the rt-fronto-temporal craniotomy on the first day of the attack. This case fully recovered, but the other three cases of severely impaired consciousness level were dead. Maternal prognosis was related to consciousness level. We mentioned the preferred neurosurgical and obstetrical management to the intracranial hemorrhage associated with pregnancy.

[A case of metastatic brain tumor from prostatic cancer showing remarkable shrinkage following CDDP administration].

A patient with brain metastases from prostatic cancer received chemotherapy using CDDP alone. A dose of 80 mg of CDDP was administered twice intravenously, followed by intra-arterial injections of 70 mg and 50 mg for the primary lesion over the course of three months. The main brain tumor showed a 70% decrease in size when estimated by CT, along with improvement of clinical symptoms related to the brain metastases. Intravenous injection of CDDP may thus have potential therapeutic value against metastatic brain tumors.

Iodine-131 metaiodobenzylguanidine intra- and extravesicular accumulation in the rat heart.

In order to establish the appropriate time for [123I]MIBG human myocardial imaging to assess the adrenergic nerve activity, the time courses of metaiodobenzylguanidine (MIBG) intra- and extravesicular accumulation in the rat heart were estimated by using [131I]MIBG and reserpine. In the heart, the intravesicular accumulation was relatively constant, while the extravesicular accumulation decreased rapidly from 5 min to 6 hr. The intravesicular percentage of the total cardiac tissue concentration reached a plateau value of 50% at 4 hr after i.v. injection of [131I]MIBG. In the spleen, similar time courses were observed as those in the heart, both of these organs being richly innervated by adrenergic nerves. Along with the time activity difference previously observed in the human hearts, these results suggest that at 4 hr post i.v. injection, [123I]MIBG myocardial imaging will best express the neuronal accumulation of the tracer and may be useful for the assessment of adrenergic function in various pathological conditions of the human heart.
