Daily walking combined with diet therapy is a useful means for obese NIDDM patients not only to reduce body weight but also to improve insulin sensitivity.

OBJECTIVE: To evaluate the effects of walking combined with diet therapy (1,000-1,600 kcal/day) on insulin sensitivity in obese non-insulin-dependent diabetes mellitus (NIDDM) patients. RESEARCH DESIGN AND METHODS: Subjects were divided into two groups: 10 patients were managed by diet alone (group D), and 14 patients were placed in the diet and exercise group (group DE). Group DE was instructed to walk at least 10,000 steps/day on a flat field as monitored by pedometer (19,200 +/- 2,100 steps/day), and group D was told to maintain a normal daily routine (4,500 +/- 290 steps/day). A glucose clamp procedure at an insulin infusion rate of 40 microU.min-2.min-1 was performed before and after the 6- to 8-week training program. Mean serum insulin concentrations ranged from 720 to 790 pmol/l. RESULTS: While body weight (BW) in groups D and DE decreased significantly (P < 0.01) during the study, the amount of BW reduction in group DE was greater than that in group D (7.8 +/- 0.8 vs. 4.2 +/- 0.5 kg, P < 0.01). After training, glucose infusion rate (GIR) and metabolic clearance rate (MCR) in group D did not significantly increase; however, GIR and MCR increased significantly in group DE, from 17.21 +/- 1.11 to 26.09 +/- 1.11 mumol.kg-1.min-1 (P < 0.001) and from 3.0 +/- 0.3 to 5.3 +/- 0.4 ml.kg-1.min-1 (P < 0.001), respectively. The analysis of variance showed significant effects of exercise (time x exercise, P = 0.0005) for the improvement of MCR. Significant correlations were also observed between delta MCR and average steps per day (r = 0.7257, P < 0.005) in group DE. CONCLUSIONS: Walking, which can be safely performed and easily incorporated into daily life, can be recommended as an adjunct therapy to diet treatment in obese NIDDM patients, not only for BW reduction, but also for improvement of insulin sensitivity.

Thoracic flexion myelopathy. Case report.

A case of thoracic flexion myelopathy is reported in a 36-year-old man. The patient had a history of gradually progressing transverse thoracic myelopathy. Conventional myelography in the neutral position failed to reveal cord compression; however, a severe block of the dye column was evident on studies in the flexed position. Excellent recovery from this condition was obtained with posterolateral fusion of the thoracic spine in the neutral position.

[Expression of GP160 mRNA in renal cell carcinoma: an attempt to predict the sensitivity of IFN-alpha therapy].

BACKGROUND: Alpha-interferon (IFN-alpha) has significant antitumor activity against advanced renal cell carcinoma (RC). Although numerous clinical trials reported major responses in approximately 15% of patients subjected to INF-alpha therapy, it has been impossible to predict which patients will respond to IFN-alpha. RC cell lines could be subdivided into those that are inhibited by IFN-alpha and those that are not. The expression of GP160 (Aminopeptidase a/kidney-associated differentiation antigens) correlated with resistance to IFN-alpha. In RC xenografts using a mouse model, though GP160(+) RC cell lines were resistant to IFN-alpha, GP160(-) RC cell lines were markedly sensitive to IFN-alpha. METHOD: In this study, we assayed 26 RC patients for the expression of GP160 in mRNA level using RT-PCR (reverse transcription polymerase chain reaction). RESULTS & CONCLUSION: In 23 (88.5%) patients GP160 mRNA expression are remarkable as in normal kidney tissue, but in 2 (7.7%) patients, GP160 mRNA expression was weak. And in 1 (3.8%) patient, GP160 mRNA expression was hardly detectable, and in this patient complete remission was achieved with IFN-alpha therapy for advanced lung and mediastinal metastatic RC.

[Primary seminoma of the prostate and embryonal cell carcinoma of the left testis in one patient: a case report].

A 27-year-old Japanese male presented with gross hematuria and right frank pain, and was diagnosed of having a right ureteral stone by IVP. Though the stone was passed spontaneously, gross hematuria continued, and his urinary stream became week. The prostate was moderately enlarged. The prostate biopsy was initially thought to represent prostate rabdomyosarcoma. He underwent a pelvic extenteration followed by chemotherapy with dacarbazing and ADM. Three years after the pelvic exenteration, coin lesions were found on a chest X-ray film. He was diagnosed as embryonal cell carcinoma of the left testis with pulmonary metastasis and received 3 courses of PVB therapy. At this time, the former diagnosis of prostatic rabdomyosarcoma was changed to prostatic rabdomyosarcoma was changed to prostatic seminoma, because placental-ALP staining was positive. This patient had two distinct germinal cell tumors in the prostate and the left testis. He was alive without any tumor recurrence 20 months after PVB chemotherapy. To the best of our knowledge our report represents the first patient of seminoma of the prostate in the world.

[A case of tricuspid valve endocarditis in the drug addict].

We reported a case of tricuspid valve endocarditis in a drug addict. A 25-year-old woman with a history of drug abuse was admitted to our hospital for high fever. Blood cultures revealed staphylococcus aureus, and echocardiography showed vegetation attached to the tricuspid valve and moderate tricuspid regurgitation. Excision of the posterior leaflet including vegetation, and direct suture of the residual posterior leaflet along with annuloplasty was successfully done. Postoperative course was uneventful and endocarditis was eradicated.

[The mechanisms of myeloma cell growth].

The mechanism of human multiple myeloma cell growth was studied utilizing twelve myeloma cell lines established in vitro. Seven of them were derived from extramedullary lesions and had the appearance of immature plasmablast. Five were from bone marrow and showed typical plasma cell morphology. Seven extramedullary lesion derived cell lines grew spontaneously and additively proliferated in response to IL-6 and/or gp-130 related cytokines. Three bone marrow derived cell lines grew dependently on a macrophage feeder layer and then on IL-6. Two bone marrow derived cell lines grew in the presence of bone marrow stromal cells or stromal cell culture supernatant, however, they did not proliferate in response to IL-6. While establishing cell lines, it was found that the proliferating fraction was primarily included in a fraction which was non-adherent to stromal cells and was composed of undifferentiated plasmablasts. Undifferentiated plasmablasts proliferated in response to IL-6, while the adherent and mature form of myeloma cells did not proliferate in response to IL-6. Two established bone marrow derived stromal cell dependent myeloma cell lines were adherent to stromal cells, had the mature form of plasma cell morphology and did not proliferate in response to IL-6. These results imply that multiple myeloma cells are composed of heterogenous populations with different maturities and different growth mechanisms.

[Medulla oblongata compression by tortuous vertebral artery: case report].

We reported a case of a 58-year-old woman who suffered from progressive tetraparesis and sensory disturbance caused by compression of the medulla oblongata brought about by bilateral tortuous vertebral arteries. The neurological examination on admission revealed tetraparesis, sensory disturbance in all modalities below the level of C2, onion-skin pattern sensory disturbance of the face, and motor weakness of the sternocleidomastoid muscles. X-ray films of the cervical spine showed OPLL at the level of C2.3, but on MRI the dural theca was not seen to be compressed at that level. MRI and vertebral angiography demonstrated ventrolateral compression of the medulla oblongata by bilateral tortuous vertebral arteries. After suboccipital craniectomy and C1.4 laminectomy, decompression of the medulla oblongata was performed. Both vertebral arteries were compressing the medulla oblongata at the ventrolateral side. Transposition of the vertebral arteries seemed impossible due to perforating branches to the medulla. After section of the dentate ligaments at C1 level, the medulla oblongata was decompressed and moved backward. Some small pieces of Dacron were inserted between the arteries and the medulla, in order to decrease the pulsatile effect of the vertebral arteries. Postoperatively, the patient's tetraparesis and sensory disturbance gradually improved. There are 6 reported cases concerning compression of the medulla oblongata by the vertebral artery. As MRI becomes more frequently used in neurological situations, it is important to keep the presence of such an entity in mind.

[Pelvic kidney associated with staghorn calculus and renal pelvic cancer: a case report].

A 63-year-old man was admitted to our hospital with complaints of right lower abdominal pain and stone excretions. The patient was diagnosed as having a pelvic kidney associated with staghorn calculus and extended pyelolithotomy was to be performed. However, during the operation, a papillary tumor was found in the renal pelvis occupied with staghorn calculus. As the pathological examination of its frozen section was low grade transitional cell carcinoma, nephro-ureterectomy was performed. Pelvic kidney associated with staghorn calculus or renal pelvic tumor with stones is not rare, but pelvic kidney associated with staghorn calculus and renal pelvic cancer is extremely rare.

[Mitral valve repair for mitral regurgitation--technical contrivance and its further application].

To evaluate the mitral reconstructive technique from the pathological and surgical points of view, 33 consecutive cases were analyzed. Eighteen patients (54.5%) were men, and the mean age was 40.9 +/- 19.8 (range 5 to 72 years). Ten patients (30.3%) had atrial fibrillation. The causes of mitral regurgitation were torn chordae tendinae in 17, elongated choradae tendinae in 14, annular dilatation in 8, papillary muscle dysfunction in 2, and congenital cleft in 4. The mitral regurgitation was due to prolapse of the anterior leaflet in 16, prolapse of the posterior leaflet in 10, and prolapse of both leaflets in 5. Resection and reconstruction of the leaflet was performed on anterior leaflet in 15, and on posterior leaflet in 15. The anterior mitral cleft was sutured in 3. The newly contrived wrapping and shortening chordoplasty was performed on anterior leaflet in 6, on posterior leaflet in 3, and on both leaflets in 2. Two patients had replacement of artificial chordae tendinae with EPTFE suture. Commissuroplasty was performed at anterolateral commissure in 5, at posteromedian commissure in 15, and at both commissures in 5. Thirty patients with dilated annulus underwent ring annuloplasty by Carpentier ring. There was no hospital death. Two patients required mitral valve replacement for redeveloping mitral regurgitation 2 weeks and one year after initial operation. All patients were in NYHA functional class I after surgery. The Doppler echo cardiographic study revealed no mitral regurgitation in 27, mild or trivial in 3, and moderate in 3. LVEDVI and LVESVI significantly decreased, and left ventricular volume overload was markedly reduced.

[Acquired amylase production induced by radiotherapy in a myeloma patient].

A 55-year-old patient with multiple myeloma (IgG-lambda) diagnosed in November 1988 was admitted because of bone pain throughout the body. After plasmapheresis and several courses of chemotherapy, a massive tumor of the left thoracic wall involving the rib appeared. Radiotherapy was performed to ameliorate the severe chest pain, after which myelomatous pleural effusion appeared on the left side. The serum, urine and pleural effusion revealed increased activity of amylase of the salivary type. Amylase activity was also detected in the supernatant of myeloma cells cultured from pleural effusion. We reviewed 12 cases of ectopic amylase-producing multiple myeloma. All the cases except one have been reported from Japan, and hyperamylasemia in these cases was detected at diagnosis or during course of the illness. Moreover, cytogenetic analysis of myeloma cells of previous reports revealed structural abnormalities including chromosome 1, near the amylase gene locus. This case also showed t (1; 10) (q 21?; q 26) by examination of 8 metaphase derived from bone marrow. These observations suggested that ectopic amylase production was induced by irradiation to the plasmacytoma of thoracic wall.

alpha-Tocopherol enhances the hypocholesterolemic action of sesamin in rats.

The effect of alpha-tocopherol on the hypocholesterolemic action of sesamin was examined in rats given a cholesterol-enriched diet. When different levels (0.05 and 0.2%) of sesamin were fed, the supplementation of 1% alpha-tocopherol significantly accentuated the hypocholesterolemic action of sesamin, particularly with the higher sesamin level, although alpha-tocopherol alone did not affect the concentration of serum cholesterol. The dose-dependent promoting effect of alpha-tocopherol on the hypocholesterolemic action of sesamin was confirmed by supplementing different levels (0.2 and 1%) of alpha-tocopherol to a fixed level of sesamin (0.2%). alpha-Tocopherol was still effective at the 0.2% level. The metabolism of sesamin in the liver S9 fraction appeared to be interfered with alpha-tocopherol in vitro, suggesting a possible role of alpha-tocopherol in maintenance of the availability of sesamin.

Thermostable N-carbamoyl-D-amino acid amidohydrolase: screening, purification and characterization.

A thermostable N-carbamoyl-D-amino acid amidohydrolase was found in the cells of newly isolated bacterium. Blastobacter sp. A17p-4. The bacterium also showed D-specific hydantoinase activity. The N-carbamoyl-D-amino acid amidohydrolase activity of the cells exhibited a temperature optimum at 50-55 degrees C, and was stable up to 50 degrees C. The N-carbamoyl-D-amino acid amidohydrolase of Blastobacter sp. A17p-4 was purified to homogeneity and characterized. It has a relative molecular weight of about 120,000 and consists of three identical subunits with a relative molecular weight of about 40,000. N-Carbamoyl-D-amino acids having hydrophobic groups served as good substrates for the enzyme. It has been suggested that D-amino acid production from DL-5-substituted hydantoin involves the action of a series of enzymes involved in pyrimidine degradation, namely amide-ring opening enzyme, dihydropyrimidinase, and N-carbamoylamide hydrolyzing enzyme, beta-ureidopropionase. However, the purified enzyme did not hydrolyze beta-ureidopropionate; suggesting that the N-carbamoyl-D-amino acid amidohydrolase coexisting with D-specific hydantoinase, probably dihydropyrimidinase, in Blastobacter sp. A17p-4 is different from beta-ureidopropionase.
