Cutaneous focal mucinosis.

A 39-year-old man had typical cutaneous focal mucinosis on the left forearm. The fibroblasts in the myxomatous lesion had many cytoplasmic vesicles, and on electron microscopy, these were compatible with condensing vacuoles or secretory granules. The fibroblasts in this case were considered to be mucoblasts involved in the over-production of the ground substance, that was mainly composed of hyaluronic acid.

Expression of Schwann cell characteristics in pigmented nevus. Immunohistochemical study using monoclonal antibody to Schwann cell associated antigen.

A monoclonal antibody to the Schwann cell associated antigen (AHMY1) and a monoclonal antibody to neurofilament proteins (NFP) (AHNF1) each were produced. Using AHMY1, AHNF1, and anti-S-100 antibody, 90 pigmented nevi were examined immunohistochemically to clarify the relationship between nevus cells and Schwann cells. All nevus cells had S-100 protein, but did not have neurofilament proteins. The nevic corpuscle and Type C nevus cell were positively stained with AHMY1 indicating the presence of the Schwann cell associated antigen, while Type A and B nevus cells were entirely negative. This suggests that the nevic corpuscle and Type C nevus cell are closely related to the Schwann cell and differ from Type A and B nevus cells in their development.

Effects of scalding injury on the components of rabbit paw lymph.

A technique for lymph collection has been described. Rabbit paw lymph was collected by this technique before and after scalding injury (75 degrees C for 20 s). The lymph concentrations of zinc, copper, iron, cobalt and lipid peroxide were measured. After scalding, the lymph flow increased rapidly and reached a maximum at 1 h, and then decreased gradually. The lymph concentrations of copper, iron and cobalt increased about two-fold in the first 30 min and the elevated levels persisted until 4 h. However, the increase of zinc was less than that of the other three metals. Total protein concentrations also increased twofold in lymph and the elevated level persisted for 4 h. Using gel filtration, the elution pattern of lymph after scalding was almost the same as that before scalding. Lipid peroxide in lymph increased about three-fold following scalding. However, the plasma concentrations of total protein and lipid peroxide did not change significantly following scalding.

Pseudotail associated with spinal dysraphism.

A 5-year-old girl had a caudal appendage and her left buttock was larger than the righ buttock. X-ray examination revealed spina bifida and bony defect of sacrum; computed tomography demonstrated the extension of the tumor from subcutaneous tissue to the spinal canal. Histologically, the pseudotail contained lobulated fatty tissue which was consistent with lipoma. It emphasizes the fact that even lesions that are not situated in the median line should be carefully explored before excision.

Perineural xanthoma.

A 50-year-old man suffering from cholestatic hepatitis and diabetes mellitus with hyperlipoproteinaemia had small, painful, slightly elevated, reddish, firm indurated plaques on his soles. Histologically, the lesions were composed of a centrally located cutaneous nerve surrounded by concentric layers of xanthoma cells. Electron microscopy showed the cutaneous nerves to be unmyelinated, their axons were vacuolated and contained dense bodies. The xanthoma cells had the same ultrastructural features as those observed in usual xanthomatous lesions. We suggest that this entity be named perineural xanthoma.

Kasabach-Merritt syndrome with disseminated intravascular coagulopathy treated by exchange transfusion and surgical excision.

Kasabach-Merritt syndrome with consumption coagulopathy occurred in a 5-week-old female infant with a hemangioma on her back. Disseminated intravascular coagulopathy was improved by an exchange transfusion. Administration of steroid and radiation therapy was attempted as a treatment for the hemangioma and thrombocytopenia; however, it was unsuccessful. Complete recovery was obtained by surgical excision. Enhanced computerized tomography was advantageous to determine the size of the hemangioma prior to operation.

Superficial spreading capillary hemangioma. A peculiar type of capillary hemangioma.

A 57-year-old Japanese woman had a history of several years of a slowly growing, dark red plaque on the sole of her foot without any subjective symptoms. Histopathologically, the lesion was a capillary hemangioma which showed a lobular structure composed of newly formed capillaries. Each lobule was separated by thin elongated rete ridges. The lesions was definitely circumscribed in papillary and subpapillary layers and appeared to extend laterally.

Posttraumatic occurrence of infantile digital fibromatosis. A histologic and electron microscopic study.

We encountered a patient with infantile digital fibromatosis. The representative lesion was a firm, indurated nodule, which developed in the scar of a previous injury on the skin of the left thigh of an 11-year-old girl. Electron microscopy revealed tumor cells that were strongly reminiscent of myofibroblasts. Histopathologically, the cytoplasmic inclusion bodies in the tumor cells stained positively or negatively with hematoxylin-eosin. Electron microscopically, they were either well-defined dense bodies that often showed a doughnutlike appearance or ill-demarcated dense bodies. Spotty calcified foci were seen in the central area of the tumor mass.

A case of primary biliary cirrhosis associated with generalized morphea.

A 50-year-old female with primary biliary cirrhosis associated with generalized morphea is reported. She had suffered from multiple painful indurated plaques on the trunk which was diagnosed as generalized morphea; subsequently, primary biliary cirrhosis was diagnosed on the basis of her hepatic dysfunction. Although progressive systemic sclerosis has been reported to occur in patients with primary biliary cirrhosis, this localized form of cutaneous sclerosis has not previously been reported. Since generalized morphea is one of the most common manifestations of chronic graft-versus-host disease, and primary biliary cirrhosis is known to have a hepatic histology resembling chronic graft-versus-host disease, this case report may represent evidence in support of a common pathogenesis for the two entities.

A case of bilateral Wilms' tumour of the kidneys and the retrourethral region.

Bilateral Wilms' tumour of the kidneys and the retrouretheral region in a 52-day-old boy is reported. These tumours are of the same histological appearance, similar to that of a malignant rhabdoid tumour of the kidney. We suggest that these tumours have arisen from the mesonephric duct or the mesonephros and Wilms' tumour may have originated from mesonephros or an even earlier developmental stage.
