[Concentration of cefotaxime and its metabolite in human bone marrow blood].

Cefotaxime (CTX) was intravenously administered in an amount of 2.0 g to each of 34 adult patients before the surgery mainly of the hip joint. Samples of the blood from the bone marrow around the trochanter were taken at the time of the operation. At the same time blood samples were taken from peripheral veins. The sample was centrifuged and the supernatant was analyzed for CTX and desacetyl-CTX. The concentration of CTX in the marrow blood was 150.9 micrograms/ml and that in the blood was 182.5 micrograms/ml in the earliest samples taken at 20 minutes after injection. In the 44 pairs of samples, the concentration of CTX in the marrow blood was lower than that in the peripheral blood in all the cases except 4. The concentration of desacetyl-CTX (Des-CTX), however, in the marrow blood was higher than in the peripheral blood in 33 of the 44 pairs of specimens. Since the degradation of the drug progresses with time, the ratio of Des-CTX to CTX increased with time. This trend was particularly marked in the bone marrow blood and can be expressed as Y = 113.0 + 0.32 t, when Y is the ratio percentage (Des-CTX/CTX) and t is time after the injection of the drug in minute. Thus, CTX transferred into the bone marrow tends to remain there and transformed into the desacetyl form.

Isolated avulsion fracture of the lesser tuberosity of the humerus. A case report.

An extremely rare isolated avulsion fracture of the lesser tuberosity, not complicated with other proximal humeral fractures or posterior shoulder dislocations, occurred in a 12-year-old girl. The avulsion fracture of the upper part of the lesser tuberosity appears to have been caused by hyperextension and hyperexternal rotation of the shoulder. In this position, the coracohumeral ligament, superior glenohumeral ligament, and subscapularis tendon could have pulled out the upper part of the lesser tuberosity. She gained full recovery of left shoulder function after three months of conservative treatment, although follow-up roentgenograms show nonunion of the fracture. An isolated avulsion fracture of the lesser tuberosity is extremely rare. Only 12 cases, none in children, have been found in the literature since Hartigan's report in 1895.

Pathological studies on esophageal varices treated with injection sclerotherapy.

Injection sclerotherapy is one of the most effective treatments for esophageal varices. To investigate the fate of esophageal varices embolized with a sclerosant (ethanolamine oleate), we have examined at autopsy the esophageal wall of 14 patients with esophageal varices. When the sclerosant was injected into the varices, at first thrombi formed. After two weeks, granulation tissue appeared and the thrombi were gradually replaced. The granulation tissues were organized slowly after 3 months and in the organized granulation tissue, microscopic recanalization of vessels were seen in 8 out of 9 cases. There was no recurrence of the original varices and de novo varices occurred in one patient. These observations indicate that the varices embolized with ethanolamine oleate become organized in three months after the treatment, and accordingly the esophageal varices are cured. It became also clear that recurrence of the original varices did not occur after the organization.

Duchenne dystrophy: reduced density of orthogonal array subunit particles in muscle plasma membrane.

Using freeze-fracture, we analyzed the density of orthogonal arrays and subunit particles in muscle plasma membrane of six patients with Duchenne muscular dystrophy and six control boys. The group median density of orthogonal arrays per 1 micron2 and the group mean density of orthogonal array subunit particles per one orthogonal array were significantly lower in Duchenne plasma membrane. The results suggested the possible impairment of orthogonal array function in the muscle plasma membrane of Duchenne muscular dystrophy.

A case of mesangial IgM nephropathy with decreased renal function.

A case of mesangial IgM nephropathy associated with chronic renal failure is described. The patient did not reveal nephrotic syndrome during the clinical course. Renal biopsy specimens revealed typical features of mesangial IgM nephropathy when studied by light microscopy, electron microscopy and immunofluorescence staining. Mesangial IgM nephropathy is considered to be a heterogenous disorder, because this disorder is clinically characterized by nephrotic syndrome, mild proteinuria and/or hematuria or renal failure.

Synthesis and antiherpesviral activity of 5-C-substituted uracil nucleosides.

Reaction of 2'-deoxy-5-hydroxyuridine with some Wittig reagents gave 5-alkoxycarbonylmethyl-, 5-carbamoylmethyl-, 5-cyanomethyl-and 5-acetonyl-2'-deoxyuridines. The corresponding 1-beta-D-arabinofuranosyl derivatives were prepared from 1-beta-D-arabinofuranosyl-5-hydroxymethyluracil. The latter was also converted to 1-beta-D-arabinofuranosyl-5-vinyluracil via triphenylphosphonium salt, and then to 5-(2-halogenovinyl)derivatives. Some of these compounds showed marked antiherpesviral activity in vitro.

Magnetic circular dichroism and circular dichroism of some nucleosides.

The magnetic circular dichroism and circular dichroism spectra of cytidine, isocytidine, 2,5'-O-cyclo-2',3'-O-isopropylidine uridine, and 2,2'-O-cyclouridine were measured in the wavenumber region of 30000 -50000 cm-1. On the basis of the experimental results, the tautomerism of cytidine and isocytidine was discussed. The transition energies, the oscillator strengths, and the Faraday parameters were calculated within the framework of the INDO approximation. The calculated results are in good agreement with the experimental results. And then the spectral assignment of the pyrimidin nucleosides, together with those of cytosine and isocytosine, were discussed on the basis of the experimental and theoretical results.

Mycotic aneurysms of the internal carotid artery. Case report.

The authors report a case with two mycotic aneurysms in the cavernous portion of the internal carotid artery, presumably secondary to a transient bacteremia from pneumonia. The strikingly rapid development of the aneurysms was demonstrated by angiography. Painful total ophthalmoplegia and extophthalmos were the main clinical features.

[The bone scintigraphy in eosinophilic granuloma (author's transl)].

The significance of bone scintigraphy using 99mTc-polyphosphate or 99mTc-diphosphate in the diagnosis of eosinophilic granuloma was discussed on the basis of our experience with four cases. The summary is following: 1. On diagnostic procedure of eosinophilic granuloma, it was found that abnormal RI accumulation in the bone scintigram was more marked than that in the ordinary brain scintigram. 2. The bone scintigram of eosinophilic granuloma showed more accurately the extent of skull involvement of cellular infiltration than the plain skull X-rays or brain scintigram. Accordingly, the bone scintigram was helpful for neurosurgeon in planning the operative procedure and determining the extent of removal of the involved skull. 3. The plain skull X-rays show the size of skull defect, and the brain scintigram reveals the size of the size of granuloma per se, as well as soft tissue involvements. We can conjecture the stage of the diseases by comparison between the findings of bone scintigram and those of above procedures: plain skull X-rays and brain scintigram. Viz, the bone scintigram showed more extensive areas of RI accumulation including the surrounding cellular infiltration in the acute stage, but the same size of the lesions shown by the ordinary brain scintigram and plain skull X-rays is chronic stage. Furthermore, we discussed the possibility of the differential diagnosis between eosinophilic granuloma and other neoplastic bone diseases, such as intracranial metastatic tumors and multiple myelomas etc., by further experience with the bone scintigram.
